Coffee is one of the most popular beverages worldwide, and one of the primary active ingredients in this beloved beverage is caffeine. This study was aimed 1) to measure the extents of caffeine in cosmetic products commercially available, 2) to estimate the systemic exposure of caffeine following an application of each product, and 3) to investigate the percutaneous absorption and metabolism of caffeine in rats and the relevance of terpinyl acetate co-administered. Following a dilution with distilled water, the extents of caffeine in 15 cosmetic products were analyzed by using high-performance liquid chromatography with an ultraviolet detection (HPLC-UV). For percutaneous absorption and metabolism of caffeine, caffeine dissolved in distilled water, and in a 60% ethanol aqueous solution in both the presence and absence of terpinyl acetate (10%, v/v) was applied to the dorsal skin of rats after shaving a section of their fur. After a serial blood sampling, plasma concentrations of caffeine and its three primary metabolites (paraxanthine, theobromine, theophylline) were measured by using high performance liquid chromatography-tandem mass spectrometry (HPLC-MS/MS). A body cream from company S emphasizing an anti-cellulite function represented the highest caffeine level (12.2 mg/g) among cosmetic samples. The amount of caffeine could be equivalent to about 54 mg, when the cream is applied to the skin once, and the systemic exposure of caffeine can be estimated as a 7.3 mg considering the absolute percutaneous bioavailability (14%). The levels of caffeine and its three primary metabolites were monitored following intravenous administration of caffeine to calculate the absolute percutaneous bioavailability of caffeine and the metabolism ratios. No percutaneous absorption was observed when caffeine was dissolved in water. Absolute percutaneous bioavailability of caffeine dissolved in the 60% ethanol solutions was about 14%, which was 3.5 times greater than through oral administration. The generation of the three metabolites was also significantly reduced (p<0.05). Terpinyl acetate (10%) enhanced the percutaneous absorption of caffeine by 40%. This study provided data on the absolute percutaneous bioavailability of caffeine in rats for the first time. Both the increase of caffeine absorption and the decrease in its metabolism through percutaneous application may be attributable to the bypass of first-pass metabolism. Further study is needed to confirm the clinical significance and applications for terpinyl acetate as well. Also, additional researches would be valuable on the percutaneous absorption of caffeine following application of cosmetic products produced in various formulations.
