The Nutrition Care Process (NCP) and International Dietetic and Nutrition Terminology (IDNT) are used internationally as standard model. The Accreditation Council for Education in Nutrition and Dietetics (ACEND) suggested that the educational contents of NCP should be included in all clinical nutrition education programs. In South Korea, clinical dietitians are certified by the Ministry of Health and Welfare in 2010. Since 2012, clinical dietitian training institutes have been designated, where clinical dietitians receive level of education in and accordance to national qualifications. The education of NCP has to be included in the regular curriculum. Because there is limited published data on the status of implementation of NCP and IDNT in Korea, this study was conducted to understand the implementation and education status regarding the NCP in Korea.
Clinical nutrition managers and dietitians in tertiary hospitals located nationwide and university hospitals in the capital city (over 500 beds)and professors and students in clinical dietitian training institutes located nationwide from March, 2016 to April, 2016.
As the result of study from responses of 30 hospitals, 60% had adopted NCP in their clinical nutrition practice. Clinical managers (100%, 92.5%) and dietitians (97.4%, 90.8%) recognized both NCP and IDNT, and answered that they had experience on education (clinical manager 95%, 82.5%, dietitian 88.2%, 69.7%) and high degrees of knowledge. Managers and dietitians responded that the major barrier to implementing NCP is “Increase of the time spent.” However, distribution of response rate on “Increase of time spent” was less in hospitals 6 years after implementation than those under 3 years after implementation. Three quarters of managers and dietitians thought systematic training programs are needed to implement NCP.
As the result of the study from responses of 17 clinical dietitian training institutes, ‘clinical nutrition therapy 1,’ including the contents of NCP, was adopted by teaching method of lecture (88.2%) and homework type (64.7%). In addition, 70.6% of the institutions included case studies on their NCP classes. All students of educational institutions were all aware of the NCP and IDNT; however, 36.7% of students answered that they lack knowledge applicable in practical settings through practical NCP lessons. Difficulties in handling assignments on NCP in practical settings were “lack of knowledge” and “lack of practice/experience.”
In conclusion, our study suggest that ongoing education and training programs are needed to be improved to give adequate knowledge and help dietitians to overcome the barriers to effective implementation of NCP. Furthermore, toolkits and systemic educational programs, which could be applied in clinical settings beyond the education, are warranted. In addition, preparing students to become dietetic professionals, adequate curriculums and toolkits development are needed for students to apply NCP in practical setting through the education for students.
