연구목적 : 지속적으로 증가하고 있는 제2형 당뇨병 환자를 대상으로 영양관리과정(NCP)을 적용한 영양상담을 통해 환자의 식사요법 순응과 혈당 조절 효과를 분석하고자 한다. 연구방법 : 구미소재 S병원 외래에 내원한 30-60대의 제2형 당뇨병 남녀 환자 중 인슐린 처방 환자를 제외하고 당화혈색소가 6.5%-10%미만 대상자 총 49명(남 29명, 여 20명)에게 1차 영양상담 전 사전설문조사를 한 후 영양관리과정을 적용한 영양상담을 실시하였고, 2-3개월 뒤 2차 영양상담 전 사후설문조사가 이루어졌다. 설문지는 5가지 영역에서 총 60문항으로 구성되었고, 영양상담은 1:1로 이루어졌으며, 영양관리과정 평가지를 활용하였다. 신체계측 및 생화학검사 자료는 병원 EMR자료를 활용하였고, 조사기간은 2016년 12월 1일부터 2017년 4월 30일까지였다. 연구결과 : 연구대상자의 연령은 50-59세가 36.7%로 가장 많았고 유병기간은 평균 5.26년이었다. 대상자의 73.5%가 영양상담 및 교육의 경험이 없었고, 당뇨식 처방 열량에 대해 98%가 모른다고 응답하였다. 혈당 자가측정과 운동의 시행 및 횟수가 영양상담 후 증가하였고, 식사요법의 실천률이 영양상담 후 100%로 조사되었다. 연구대상자의 영양진단에서 영양문제(P)로 당질섭취 과다가 31.37%로 가장 많은 부분을 차지하였고, 영양문제에 따른 원인(E)으로 식품영양지식 부족이 29.25%로 가장 높았다. 이에 따른 영양중재의 내용은 식사요법의 중요성 교육 20.34%, 체중감량을 위한 식사요령 12.99% 등의 순서로 조사되었다. 영양중재 후에 당질섭취 감소와 채소군 및 어육류군의 섭취가 크게 증가하였고, 대상자의 식사요법관련 지식, 식사요법 수행도가 증가한 것으로 조사되었다. 혈액 측정치 결과에서 공복혈당, 식후 2시간 후 혈당은 감소하였지만 유의하지는 않았고 당화혈색소(HbA1c)는 유의하게 감소하였다. 2차 영양상담 시 연구대상자의 영양모니터링 및 평가를 한 결과 목표일부 달성한 대상자 48.04%, 목표달성 대상자 29.41%로 나타났다. 결론 : 제2형 당뇨병 환자를 대상으로 영양관리과정(NCP)을 적극 반영한 영양상담 후 모니터링 및 평가를 통해서 체중과 당화혈색소(HbA1c)가 감소하였다. 또 수행도를 알아보는 설문지의 모든 문항에서 장애요인이 개선되어 수행도가 증가하였고, 양양지식 점수가 높아진 것으로 보아 체계적인 영양상담이 효과가 있는 것으로 사료된다.

The purpose of this study is to analyze the dietetic therapy adjustment and glycemic control effects of a diabetic patient through the dietary consultation applying the nutrition care process (NCP) targeting patients with diabetes mellitus type 2 that are on the increase continuously. For the method of study, the preliminary survey targeting 49 male and female patients (29 male patients, 20 female patients) whose glycated hemoglobin was below 6.5%-10% among patients in 30s-60s with diabetes mellitus type 2 who visited the outpatient department of S hospital located in Gumi except for patients prescribed with insulin was conducted before the 1st dietary consultation and the dietary consultation applying the NCP was carried out, and then the follow-up survey before the 2nd dietary consultation was conducted 2 or 3 months later. The questionnaire was composed of 60 questions in 5 fields, the dietary consultation was carried out on the basis of one on one, and the NCP questionnaire was used. Hospital EMR data was used for anthropometric dimension and biochemical examination data, and the investigation period was between December 1, 2016 to April 30, 2017. The result of study showed that the age between 50 and 59 accounted for 36.7% of total target patients and the average disease duration was 5.26 years. 73.5% of the target patients answered that they had no experience in dietary consultation and education, and 98% of the target patients answered that they did not know the calories of prescribed diabetic diet. After the dietary consultation, the number of self blood sugar measurement and exercise and the practice rate of dietetic therapy was 100%. In the nutritional diagnosis of target patients, excessive glucoside intake due to nutritional problem (P) accounted for 31.37% that was the largest portion, and a lack of food and nutrition knowledge accounted for 29.25% as the biggest reason for nutritional problem (E). The contents of nutritional intervention was shown in order of education regarding the importance of dietetic therapy (21.05%) and dietary habits preferring foods containing a lot of sugar. After the nutritional intervention, the intake of glucoside decreased and the intake of vegetables and fish meats increased significantly, and dietetic therapy-related knowledge and dietetic therapy performance of target patients increased. The blood measurement result showed that the level of blood sugar when 2 hours passed after meals decreased but it was not significant, and the level of glycated hemoglobin (HbA1c) decreased significantly. The nutritional monitoring and evaluation result of target patients at the time of the 2nd dietary consultation showed that the patients who achieved the goal only partially accounted for 48.04% and the patients who achieved the goal accounted for 29.41%. In conclusion, the monitoring and evaluation result after the dietary consultation actively reflecting the nutrition care process (NCP) targeting patients with diabetes mellitus type 2 showed that the weight and glycated hemoglobin (HbA1c) of target patients decreased. Since all questions in the questionnaire reviewing the level of performance showed that the level of performance increased due to the removal of obstacle factors and the nutrition knowledge score became higher, it is considered that systematic dietary consultation is effective, and it is necessary to apply continuous nutrition care process (NCP) to the business practice.
