The purpose of this study was to examine the effects a spinal manipulation combined with nerve mobilization aimed at the low back pain patients with lower limb radiating pain for six weeks on pain index, low back pain disability index, isometric lumbar muscle strength and postural balance ability. A total of 30 subjects were randomly participated in this study. They were divided into joint mobilization group of 15 subjects applied to spinal manipulation and 15 subjects in the comparison group applied to lumbar stabilization exercise with a physical therapy. All intervention programs were implemented for 30 minutes and three times per week for six weeks. A pretest was conducted before intervention programs began, and a post test was conducted after six weeks of intervention in the same method with the pretest in order to investigate pain index, low back pain disability index, isometric lumbar muscle strength and postural balance ability. Kolmogorov-Smirnov tests were conducted to test the normality of collected data in order to examine changes in measurement variables in the two group between before intervention and six weeks after the beginning of intervention, independent sample t-tests were used to test homogeneity in physical characteristics between the groups, chi-square tests were conducted to test homogeneity between genders, Wilcoxon signed rank tests were used to analyze differences in visual analog scale and low back pain disability index between before and after intervention in each group, and Mann-whithey U-tests were used to analyze differences between the groups following the experiment. Since the measurement variables isometric muscle strength and postural balance ability were normally distributed, paired sample t-tests were used to analyze differences between before and after intervention in each group and independent sample t-tests were used to analyze differences between the two groups. In addition, the magnitudes of effects based on differences between before and after intervention in each group were calculated. When visual analog scale and low back pain disability values in each group were compared, after the implementation of sciatic nerve mobilization and joint mobilization for six weeks, both groups showed statistically significant decreases in the values and in comparison between the groups, the sciatic nerve mobilization and joint mobilization group showed more significant differences. When changes in the isometric muscle strength of flexor muscles and extensor muscles in each group were compared, both groups showed statistically significant increases and in comparison between the groups, the sciatic nerve mobilization and joint mobilization group showed more significant differences only in changes in the isometric muscle strength of the lumbar extensor muscle. When stability index values out of postural balance abilities were compared within each group, the sciatic nerve mobilization and joint mobilization group showed statistically significant decreases and in comparison between the groups, the sciatic nerve mobilization and joint mobilization group showed more significant differences. When changes in weight distribution index values were compared, both groups showed statistically significant decreases in states where the subjects were standing on a pillow with the eyes open, in states where the subjects bent their head back with the eyes closed, and in states where the subjects leaned their head forward with the eyes closed and only the sciatic nerve mobilization and joint mobilization group showed statistically significant decreases in states where the subjects were standing on a pillow with the eyes closed and in states where the subjected turned the head rightward with the eyes closed. Although the lumbar stabilization exercise and physical therapy group in states where the subjected turned the head leftward with the eyes closed, in comparison between the groups, no statistically significant difference was shown in the same states. However, after intervention, the sciatic nerve mobilization and joint mobilization group showed larger effect size values and both groups showed values between 4∼6 which are normal values after intervention. In conclusion, it had a particularly positive impact on a postural stability index in the four sectors such as pain index, low back pain disability index, isometric lumbar muscle strength and postural balance ability to apply spinal manipulation combined with nerve mobilization to the patients with low back pain accompanied by lower limb radiating pain. If such manipulation is widely applied to many low back pain patients, it will improve the quality of their lives by letting them return to their daily lives and economic activities more quickly.
