1. 일반적인 특성에서 성별은 남자가 52.6%, 연령은 65세 이상이 23.6%, 내원 당시 의식은 명료가 95.5%, 내원 동기는 질병이 72.2%, 진료 결과는 귀가 78.7%로 가장 높았고, GCS의 중앙값(사분범위)은 15(15,15)점이었다. 진료결과 결정까지 소요시간의 중앙값(사분범위)은 3시간 02분(118, 291)이었고, 재실시간의 중앙값(사분범위)은 2시간 28분(94, 235)이었다.
3. 중증도 단계별 진료결과는 KTAS의 중증도가 높을수록 입원, 중증도가 낮을수록 귀가가 유의하게 높았고, NEDIS 응급은 입원, 비응급은 귀가가 유의하게 더 높았다. 진료결과 결정까지의 소요시간은 KTAS 단계별 통계학적 차이가 있었고, NEDIS 응급이 비응급보다 유의하게 더 높았다. 응급의료센터 재실시간은 KTAS 단계별 통계학적 차이가 있었고, NEDIS 응급이 비응급보다 재실시간이 유의하게 더 높게 나타났다.
4. KTAS의 최초분류와 최종분류의 일치도는 유의한 상관관계를 보였고, KTAS와 NEDIS 단계 간의 일치도에도 유의한 관련성이 있었다. 또한 KTAS 1-3단계를 NEDIS 응급으로 재분류시 40.3%에서 24.7%으로 응급환자관리 대상의 비율이 15.6% 감소함을 알 수 있었다.

The subject of this study was 2,590 patients who visited the Emergency Medical Center in Seoul from April 1, 2016 to April 30, 2016. And their the national emergency medical information network and electronic medical records collected. The collected data were analyzed by frequency, percentage, mean, standard deviation, median, interquartile range(IQR), X2-test, t-test, ANOVA and Scheffe post-test. Correlation was analyzed by Pearson correlation analysis, Kendall's tau-b and statistical analysis was performed using SAS and SPSS(version 18).
The results of this study are as follows.
1. In general characteristics, the gender was 52.6% for male and more than 65-year-old was 23.6%. Consciousness at the time of visit, alert was 95.5% and Cause of visit to the hospital, the disease was 72.2%. In the medical care result, discharge was the highest at 78.7% and the median(IQR) of GCS was 15(15,15) points. In the clinical decision making time, the median(IQR) was 3 hours and 02 minutes(118, 291) and the median(IQR) of the length of stay in the hospital was 2 hours and 28 minutes(94, 235).
2. In case of the severity classification results, the KTAS level 1 was 0.9%, the level 2 was 7.4%, the level 3 was 6.4%, the level 4 was 51.1%, and the level 5 was 24.2%. NEDIS emergency was 40.3% and non-emergency 59.7%.
3. In the severity step-by-step medical care results, the higher the severity of KTAS, the higher the admission and the lower the severity, the higher the discharge. In the case of NEDIS emergency, admission was high and in the NEDIS non-emergency, discharge was significantly high. There was statistically difference between KTAS clinical decision making time and NEDIS emergency was significantly higher than non-emergency. In the length of stay, there was statistical difference between KTAS level and NEDIS emergency was significantly higher than in non–emergency.
4. The agreement between the initial classification and the final classification of KTAS showed a significant correlation. There was also a significant correlation to the agreement between the KTAS and NEDIS. In addition, when the KTAS level 1-3 was reclassified as NEDIS emergency, the ratio of emergency care patients decreased from 40.3% to 24.7% by 15.6%.
Based on the above results, we could confirm that the KTAS classification as a severity classification tool is a major indicator to predict the clinical outcome of the medical care results, clinical decision making time and length of stay. The KTAS classification has a low ratio of under-triage and over-triage plus is more objective and reasonable based on emergency patient management standards. Thus if the standardized KTAS is applied to the hospital in the future, the emergency medical system will be further improved. Also continuous education and program development need to increase the reliability among the severity classifiers.
