KTAS와 ISS를 5단계 그룹(1-중증, 5-비응급)으로 나누어서 분류별 일치도, 응급실 재실시간, 중환자실 입원율, 수술 시행률, 병원 내 사망률 등을 비교분석하였다. 연구 결과는 다음과 같다. KTAS에 의한 중증도 분류에서는 1단계 0.2%, 2단계 5.6%, 3단계 31.2%, 4단계 57.9%, 5단계 5.1%이었다. ISS에 의한 중증도 분류에서는 1단계 0.1%, 2단계 0.3%, 3단계 6.9%, 4단계 21.4%, 5단계 71.2%이었다. 응급실 진료결과 비교에서 KTAS와 ISS 모두 중증도가 낮은 4, 5단계에서 귀가율이 높았고(KTAS=66.7%, ISS=97.0%), KTAS는 중증도가 높은 1, 2, 3단계에서 중환자실 입원율이 높았다(KTAS=92.1%, ISS=49.0%). 응급실 재실시간은 KTAS와 ISS 모두 중증도가 높을수록 길어졌고, ISS에 비하여 KTAS는 모든 분류단계에서 응급실 재실시간이 짧았다. KTAS와 ISS의 분류별 일치도의 상관계수는 τ = 0.21로 약한 양의 상관관계를 보였다. KTAS와 ISS의 진료결과 예측력 비교 분석결과, KTAS와 ISS는 중환자실 입원율, 병원 내 사망률 측면에서 외상환자의 중증도를 높은 수준으로 예측하였다. 결론적으로 KTAS는 외상환자에서 중증도 분류를 통해 적정한 진료를 6시간 이내에 받게 하여 응급실 재실시간을 줄여 과밀화 해소에 기여하며, 특히 중환자실 입원율과 사망률을 예측할 수 있는 좋은 분류 도구임을 알 수 있었다. 그러나 본 연구에서 KTAS와 ISS의 상관도 계수는 τ = 0.21 로 나타나 중증도 분류 단계 불일치와 외상환자 중증도 분류 시 과소, 과대평가문제가 있음을 재확인할 수 있었다. 따라서 KTAS 중증도 분류자의 역량 강화를 위한 체계적인 교육과 심화학습이 필요하며 외상환자 분류에 있어 외상환자의 특성이 반영된 분류지표에 대한 이해와 지속적인 연구 역시 필요할 것으로 사료된다.

Injury Severity Score (ISS) is a representative triage system for trauma patients, but it has a disadvantage in which it is difficult to be used to determine the severity of patients immediately after they enter the Emergency departments (ED). Since 2016, the Korea Triage and Acuity Scale (KTAS) has been used in all ED in Korea. However, the adequacy of KTAS remains to be tested in actual clinical performance. The purpose of this study is to verify the adequacy of KTAS to determine the severity of trauma patients through the comparative analysis between ISS and KTAS. We retrospectively reviewed the NEDIS(National Emergency Department Information System) data and medical records of trauma patients with the age of 15 or higher who visited the Emergency Medical Center in Seoul from January 1, 2016 to June 30, 2017. KTAS and ISS divided patients into 5 groups (1 severe, 5 non-emergency) and analyzed agreement by methods, ED length of stay(LOS) time, ICU admission rate, operation performance rate and hospital mortality. The results of this study are as follows. A total of 6,639 subjects participated in this study. According to KTAS, 0.2 %, 5.6 %, 31.2 %, 57.9 % and 5.1 % were assigned to the levels 1, 2, 3, 4, and 5, respectively. According to ISS, 0.1 %, 0.3%, 6.9 %, 21.4 %, and 71.2 % were assigned to the levels 1, 2, 3, 4, and 5, respectively. When outcomes in ED were compared, discharge rate was high in levels 4 and 5 with low severity of both KTAS and ISS (KTAS=66.7%, ISS=97.0%). ICU admission rate was high in levels 1 through 3 with high severity (KTAS=92.1%, ISS=49.0%). ED LOS got longer in lower levels of both KTAS and ISS. ED LOS of KTAS at all levels was shorter than that of ISS. The concordance correlation coefficient between KTAS and ISS was τ = 0.21. A positive weak correlation was shown. When prediction performance of KTAS and ISS was compared, ICU admission rate and hospital mortality in KTAS and ISS were good. Based on the above results, KTAS reflects the severity of trauma patients and helps them receive proper treatments within the appropriate length of time. Thus, it contributes to resolving the overcrowding issue by reducing the ED LOS. In particular, it is a good tool to predict ICU admission rate and hospital mortality. However, there are still some problems such as group inconsistency, under-triage and over-triage. Thus, systematic training and further studies are required to strengthen abilities of personnel using KTAS. In addition, understanding of indicators reflecting characteristics of trauma patients and further studies are required for classification of trauma patients.
