Previous studies on stress experienced by nursing students during clinical practice showed that stress related to the attitude of health professionals, patients, and family caregivers as well as interpersonal stress among fellow students, was high. Despite these previous reports, no study has specifically identified the factors influencing interpersonal stress in clinical practice. Accordingly, interpersonal stress in nursing students needs to be examined during clinical practice, and related factors need to be identified. As a subcategory of interpersonal stress in clinical practice, interpersonal stress related to patients, family caregivers, nurses, clinical practice instructors, and fellow students was explored. Therefore, this study aimed to explore factors related to interpersonal stress among nursing students in clinical practice. This is a descriptive study, and 95 nursing students who had over 6 months of clinical experience in Y. University were included as subjects. A structured questionnaire survey was conducted from September 2 to 8, 2016. The measurement scales used in this study included the nursing professionalism scale developed by Yoon(2005) for measuring nursing professionalism, emotional intelligence scale developed by Jeong(2007) for measuring emotional intelligence, social support scale developed by Kim(2008) for measuring social support, and physical symptom scale developed by Han, Jeon, Tack, Lee & Lee(1992) for measuring perceived health condition. Finally, the clinical nursing practice stress scale, originally developed by Hwang(2002) and then modified by other researchers, was used to measure interpersonal stress in clinical practice.
Data were analyzed using SPSS statistics 23.0 to conduct descriptive statistical analysis, which included independent t-test, one-way ANOVA, Pearson’s correlation, and multiple regression analysis.
The main results of the research were as follows.
1) Approximately 82(86.3%) of the surveyed subjects were found to be female students. A total of 53(55.8%) and 42(44.2%) subjects were third and fourth grade students, respectively, with and average age of 22.11(±1.53). Personality types included 53(55.8%) introverted and 42(44.2%) extroverted types. In addition, 53(55.8%) received social support from their families, whereas 25(26.3%) received social support from friends. The average of perceived health condition was 3.53(±3.90). 2) The average interpersonal stress of nursing students in clinical practice was 67.11(±18.60), perceived health condition was 105.47(±11.95), and emotional intelligence was 78.88(±13.87). Finally, the average social support was 89.33 (±12.08). 3) Interpersonal stress of nursing students in clinical practice, according to general characteristics and perceived health condition, showed a significant difference in grade(t=−3.77, p<.001) and perceived health condition(r=.24, p=.019). 4) Factors related to interpersonal stress with patients included nursing professionalism(β=−.28) and emotional intelligence(β=−.25). Moreover, nursing professionalism was a factor related to the interpersonal stress with family caregivers. Factors related to the interpersonal stress with nurses were nursing professionalism(β=−.23), grade(β=.40), and perceived health condition (β=.20). Finally, factors related to the interpersonal stress with fellow students were nursing professionalism(β=−.23) and social support (β=.29).
The results of this study confirmed that nursing professionalism was the factor that influenced interpersonal stress in clinical practice involving patients, family caregivers, nurses, clinical practice instructors, and fellow students. However, this study was conducted on Y. University’s nursing students to control for differences in clinical practice teaching systems. The results of this study suggest the improvement of emotional intelligence, enhancement of nursing professionalism, and establishment of social support for managing interpersonal stress of nursing students in clinical practice.
