본 연구의 목적은 발달장애 아동에게 구강감각자극 프로그램을 제공하였을 때 식습관 문제행동과 편식행동에 미치는 영향을 알아보고자 하였다. 연구대상은 만 3세부터 7세의 발달장애 진단을 받은 아동 12명을 선정하였다. 대상자들은 촉각과 고유수용성감각, 다감각을 활용한 구강놀이 활동으로 구성한 구강감각자극 프로그램을 받았으며 중재는 총 20회기로 회당 20분 동안 실시하였다. 중재 전-후의 구강감각처리 변화를 알아보기 위해 감각프로파일의 구강감각처리 영역을 사용하였고, 식습관 문제행동을 알아보기 위해 한국형 아동 섭취행동 검사를 실시하였다. 또 편식 행동 변화를 알아보기 위해 식품 섭취 빈도표를 사용하여 평가하였다. 결과 분석을 위해 윌콕슨 부호 순위 검증을 실시하였다. 검사결과 구강감각자극 프로그램 중재 전·후 대상군의 감각프로파일 검사 상 구강감각처리 영역에서 유의한 차이가 있었다. 한국형 섭취행동 검사에서 총 점수의 유의미한 변화가 있었으며 그 중 식사 중 부모스트레스 점수가 감소하고 충분한 양 섭취, 적절하게 씹기, 음식 즐기기 등 전반적 식사문제에서 개선을 보였다. 또한 중재 전·후 편식행동 점수 변화 측정 결과 중재 후 육류, 난류 및 두류, 뼈째 먹는 생선, 유지류, 해조류, 채소류, 과일류, 곡류 및 감자류, 김치류 편식행동에서 유의미한 변화를 나타냈다. 본 연구의 결과를 통해 구강감각자극 프로그램이 발달장애 아동의 식습관 문제행동과 편식행동 개선에 효과적인 중재방법이며, 발달장애 아동의 작업치료 프로그램으로 사용될 수 있음을 나타낸다. 또한 발달장애 아동의 식습관 문제행동 개선에 대한 구강감각자극 프로그램 활용 연구의 기초자료로 활용될 수 있을 것으로 사료된다.

This study is evaluated that a oral sensory stimulation program had effect on ability of oral sensory processing, eating and feeding problem, picky eating behavior with developmental disorder children. The study enrolled 12 children with diagnosed developmental disorder. Participants were provided oral sensory stimulation program that was constituted tactile, proprioceptive and multi-sensory using oral activities. The intervention was carried out for 20 minutes and 20 session. The result of the intervention were evaluated by comparing the oral sensory processing before-after the using Oral Sensory Processing area of Sensory Profile, the Korean Children Eating Behavior Inventory(K-CEBI) for feeding problems, and the food intake checklist for picky eating behavior. The Wilcoxn signed-rank test was used to compare pre-post test. As a result, Oral sensory processing improved in pre-post test. Eating and feeding problem improved in pre-post test, including parent’s stress and global mealtime problems on K-CEBI. Also on before-after interventions, there were significant changes picky eating behavior – eggs and beans, fish with bones, fats, seaweeds, vegetables, fruits, grains and potatoes, Kimchis. - on food intake checklist. The study found that Oral sensory stimulation program was effective intervention on developmental disorder childrens’ oral sensory processing, eating and feeding problem, picky eating behavior improvement, and offered occupational therapy program for children with developmental disorder.
