The purpose of this study is to analyze the effect of Drug Utilization Review(DUR) on drug prescription and the claim adjustment on the drug prescription by the Health Insurance Review and Assessment Service(HIRA) since the DUR system was adopted in March 25 2011 to prevent drug-related accidents and promote safe use of drugs.
First, this study analyzes the number of drug prescription changes and the number of patients taking these drugs before and after adopting the DUR, focusing on contraindication drugs (drug–drug interactions, age precautions, pregnancy contraindication) and duplication drugs(ingredient duplication and therapeutic duplication). This research is based on the HIRA-NPS data set from 2010 to 2013 using multiple regression analysis.
Secondt, this study analyzes how the provision of information on contraindication and duplication drugs by DUR has affected drug prescription changes. This research is based on the monthly DUR operation data of HIRA from April 2011, which is shortly after the adoption of the DUR system, to the end of 2014 using time series analysis.
Last, this study analyzes how the provision of information on contraindication and duplication drugs by the DUR has affected claim adjustments on such drugs. This research is based on the assessment monitoring data of HIRA from April 2011, which is shortly after the adoption of the DUR system, to the end of 2014 using time series analysis.
The result of this study can be summarized as follows;
First, this research findings show that the number of prescriptions of contraindication(drug–drug interactions, age precautions, pregnancy contraindication) and duplication drugs(ingredient duplication and therapeutic duplication) as well as the number of patients taking these drugs declined after the introduction of the DUR system, albeit to different degrees.
Second, it was found that the adoption of the DUR system raised the rates of prescription modification for all contraindication and duplication drugs in the early stages. The rates of prescription modification climbed initially with the adoption of the DUR system before starting to decline gradually. It is presumed that this is attributed to the increased awareness of DUR information.
In the meantime, higher-grade healthcare institutions are less likely to change prescriptions than lower-grade hospitals or clinics when provided with DUR information on contraindication and duplication drugs. It is supposed that this is because higher-grade hospitals tend to have more patients with severe conditions.
Third, the provision of contraindication information through the DUR system raised the rates of prescription modification at the point of prescribing. Hence fewer claim adjustments were made during the review process by HIRA.
In conclusion, this paper found that the DUR system was adopted by almost all healthcare institutions in Korea at a relatively fast rate after its nationwide introduction in March 25, 2011. As a result, there was a decline in the number of prescriptions of contraindication and duplication drugs and the number of patients taking these drugs due to the information on the DUR.
In addition, with the provision of information on contraindication and duplication drugs by DUR, the rates of prescription modification at the point of prescribing rose in the early stages, but gradually declined with the increased awareness of DUR information. This led to a drop in claim adjustments by HIRA.
The introduction of the DUR system reduced prescriptions of contraindication and duplication drugs all together, while increasing prescription changes early, and thus, decreasing claim adjustments later, by effectively providing information on contraindication and duplication drugs in real time. Furthermore, the DUR system significantly contributed to preventing drug-related accidents and promoting drug safety nationwide.
