3개월간 외래환자의 DUR전체항목의 경고 대상 처방전수 1,416,153장과 경고대상 처방의약품수 2,506,844건에 대해 분석한 결과 DUR5개 점검항목에 해당하는 예외사유기재 필수 약품의 경고 약품수는 62%인 1,554,978건으로 나타났으며 그중 47%에 해당하는 처방약품이 사유를 기재하여 계속 처방한 것으로 나타났다. 따라서 본 연구는 DUR 경고 약품 47%에 해당하는 724,503건에 대해 예외사유코드로 기재한 경우, 약 95%에 해당하는 688,610건과 나머지 5%에 해당하는 35,893건의 텍스트로 예외사유를 기재한 내용을 분석하였다.
처방의의 예외사유 기재 방식은 사유코드로 입력하는 방식과 텍스트로 입력하는 방식이 있다. 따라서 심평원에서 제시한 기설정된 사유코드가 적절한지 그리고 처방의의 텍스트기재 사유가 기설정된 사유와 유사성이 있는지 분석하여 경고대상 처방의약품을 무시(Override)하여 계속 처방하는 경우 의미가 명확한 사유를 기재할 수 있는 방안을 제시하였다.
총 텍스트로 기재한 사유건수 중 심평원의 11개 예외사유코드와 유사한 사유가 약 42%, 기존 코드와 부합하지 않고 의미 있는 사유가 약 41% 그리고 의미 없는 사유가 약 17%로 나타났다. 즉 기존코드와 부합하지 않는 텍스트 사유는 전체 예외사유 기재 약품 건수(724,503건)의 약 2%에 불가하여 현재 텍스트 기재를 의무화한 병용금기, 연령금기, 임부금기 의약품 및 동일성분 중복처방의약품을 불가피한 사유로 계속하여 처방 시 코드로 변경하는 것을 제시하였다.
그리고 기설정된 11개 코드 중 가장 빈도가 높았던 코드 A(장기 출장이나 여행 등의 이유로 미리 처방을 받는 경우)와 코드 F(처방일과 투약일이 다른 경우)그리고 코드 C, H, I, J는 함께 병용 또는 복용하지 않는 의미이므로 한 개의 코드로 조정하는 방안을 제시하였다.
결과적으로 기설정된 사유코드 중 유사항목으로 인해 혼란을 초래할 수 있는 코드를 통합하였고 텍스트기재 사유 방법은 삭제하는 대신 가장 사용 빈도가 높았던 사유를 DUR 항목별로 신규코드로 추가함으로써 처방의는 예외사유 선택 시 해당 모듈별 예외사유 코드를 선택하는 것으로 제안하였다. 병용금기 의약품의 예외사유코드로 9개(DD1~DD9), 연령금기 의약품의 예외사유코드로 5개(AC1~AC5), 임부금기 의약품의 예외사유코드로 6개(PG1~PG6) 그리고 동일성분 중복의약품의 예외사유코드로 15개(SD1~SD15)가 제시되었고 총 14의 코드가 신설되었고 기존 코드는 11개에서 5개로 통합되었다.

Objective: To investigate overall patterns of prescriber’s rationales for overriding the national DUR alerts and to determine whether these reasons were helpful in verifying orders. By analysing the problems of existing method using 11 types of Health Insurance Review and Assessment Service (HIRA) codes and free text entry, this paper suggests a new overrides reasons codes structure that improves prescribers’responses to DUR alerts without unclear and meaningless reasons.
Methods: Prescription data electronically submitted to the HIRA with mandating online prospective DUR alerts system were obtained from January 1to March 31, 2015 from all medical institutions in Seoul. The prescribers’reasons for overriding alerts were collected from both established 11 codes by government agency and free text entry contents. All the free text entry of prescribers’ reasons for overriding DUR alerts were validated manually by expert pharmacists and statistically analysed to determine the patterns by SQL query and EXCEL Macro.
Results: Of 1,554,978 alerted medications by 5 DUR categories, 47% (724,503) of medications were prescribed continuously with override reasons. Among these, 688,610 (95%) medications were utilized the HIRA codes, and 35,893 (5%) utilized free text reasons. According to the free text content analysis, 42% of contents were equal or similar to the existing HIRA codes. 41% were useful reasons but unclassifiable in HIRA codes which outlines the need to develop new codes by DUR categories. Also 17% were identified as no meaning or not useful reasons. Total 14 new codes were suggested by analysing the free text contents and existing 6 codes (A, C, F, H, I, J) were merged into one code, 2 codes (G, P) were merged into one because they were too detailed and difficult to differentiate among them. The suggested new codes for drug drug interaction were 9 codes such as no substituted drug (DD4), need for therapy (DD5) etc. The new codes for age contraindication were 5 codes such as need for therapy (AC1), physically adult weight (AC3) etc. The new codes for pregnant contraindication were 6 codes such as labor induction (PG1), external fertilization (PG2) etc. The new codes for duplicated substance were 15 codes such as need for therapy (SD4), no substituted drug (SD6) etc. The top 3 responses for 5 DUR categories were “ not taking together” and “as needed, PRN”and “ not contact with previous prescriber.
Conclusions: When given the opportunity to provide a reason for overriding a national DUR alerts, prescribers rarely enter justifications that are useful in verifying the orders. These new suggested codes by DUR category may facilitate the use of alert override in the current national online DUR system. For further study, the reason codes for pharmacists to verify dispensing should be designed.
