본 연구는 건강신념 모델을 적용하여 고혈압 관리 및 예방을 위한 영양교육 프로그램 및 교육 자료를 개발하고 그 교육 효과를 분석하는 한편, 고혈압 관리에 영향을 주는 요인을 파악하여 앞으로 고혈압 예방 및 관리 교육자료 개발을 위한 기초자료 제공을 목적으로 하였다. 연구 1은 건강신념 모델을 적용한 고혈압 영양교육 프로그램 개발로 보건소에 내원하는 고혈압 환자 및 고혈압 전단계 환자들을 대상으로 포커스그룹 인터뷰(FGI)를 실시 한 후 이를 토대로 고혈압 영양교육 프로그램 및 교육 자료를 개발하였다. DFQ 총점과 인식된 위협, DFQ 총점과 인식된 이득과 장애 그리고 자아효능감을 교육 기간 동안 점수를 비교한 결과 수축기 혈압과 마찬가지로 DFQ에 유의한 영향을 주지는 않았다. DFQ 총점에 영향을 준 건강신념 모델 요인 결정을 위한 상관분석의 경우 교육 시점에서 DFQ와 수축기 혈압, 인식된 위협, 행동의 계기, 인식된 이득과 장애, 자아효능감와의 관계를 본 결과 DFQ 총점수와 관련이 있는 요인은 인식된 장애(0.3630, p<0.001)와 자아효능감(-0.2190, p<0.05)이 유의한 관련성을 보였다. 그러므로 교육 전후 변화에 DFQ 총점에 뚜렷한 영향을 준 요인은 없었으나 교육 시점에서의 점수에 영향을 준 요인은 인식된 장애이며, 그 외에 인식된 장애와 자아효능감이 서로 유의한 관련을 보여 장애가 낮을수록 자아효능감이 커지는 것을 나타냈다. 본 연구는 건강신념 모델을 기반으로 고혈압 예방 및 관리를 위한 영양교육 프로그램의 개발과 교육 효과평가를 실시한 논문으로 고혈압 영양교육을 이론에 적용하여 고혈압 프로그램을 개발한 것에 의미가 있다. 기존 연구와 다르게 고혈압 관리를 위한 저나트륨 식습관을 건강일기 모니터링과 영양상담을 통해 지속적으로 관리하였으며, 다양한 활동지를 통해 대상자가 적극적으로 교육에 임할 수 있도록 구성하였다. 건강신념 모델요인 간 상관관계에서 인식된 장애와 자아효능감이 유의한 관련을 보였으므로, 장애를 낮추고 자아효능감을 높여 고혈압 예방 및 관리를 유지할 수 있는 프로그램 보완이 필요하다고 생각되며, 본 연구에서 개발한 프로그램이 고혈압 식생활 관리를 위한 행동 유발로 자리매김이 되길 기대한다. This study was performed to develop nutritional education program for management and prevention of hypertension based on Health belief model and evaluate the effect of the program. Also, factors affecting hypertension management were assessed to analyze the educational effect for hypertension. In study 1, Focus group interview(FGI) was conducted on 23 hypertensive or prehypertensive participants aged over 45 years to investigate main factors of Health belief model with regard to hypertension management. ‘Perceived susceptibility' to hypertension was family history, neglect of health care, preference for salty foods, broth of soup and stew. Lifelong medication, complications, and medical costs were reported as ‘perceived severity’ of hypertension. ‘Perceived benefits'of hypertension management were decrease of medicinal dose, reduction of medical costs, and healthy eating habits of the family, while ‘perceived barriers’ were lack of palatability of low salt diet, convenience-oriented dietary habits, and limited choice of foods when eating out. participants mentioned TV health programs, public health center programs, and advice from doctors and family as 'cues to action' of hypertension management. These qualitative information provided basis for developing a nutrition education program for hypertension which could be implemented in the public health center. Eight week program was developed which consisted of understanding hypertension, risk factor management such as eating habits, bldy weight, Principles and practice of cooking low salt diet, advanced management for healthy diet in 2 sessions, and summary. Each session was designed to alert the susceptibility and severity, to emphasize the benefits, and to reduce the barriers by providing dietary monitoring, practical advice, and action tips. In study 2, Nutrition education program for hypertension management was performed and the effect was evaluated at three public health centers in Seoul. Among the participants aged over 45 years with high risk of hypertension, and 115 women completed questionnaire, anthropometic and biochemical measurement during the 16 week study period (8 week education and 8 week follow up) were used in the analysis. Systolic blood pressure was decreased in hypertensive group and normal group after 8 weeks of education, and the decrease was effective in normal group even additional 8 weeks follow up period. After 8 weeks of education waist circumference decrease in hypertensive group (-4.6%) and normal group (-2.8%) both groups showed significant increase in HDL cholesterol levels (hypertensive group 9.3%; normal group 8.9%, p<0.001) and decrease in serum triglyceride levels (hypertensive group –15.7%; normal group –14.9%, p<0.01). In addition, LDL cholesterol level and total-cholesterol level were decrease in hypertensive group and normal group, respectively, after completion of the program. Further, eating habits, nutrition knowledge, self efficacy associated wit reducing sodium intake, and dish frequency questionnaire(DFQ) on high sodium food were improve in both groups. Therefore, the education program showed positive effects on improving health indices and diet behavior in middle-aged women with high risk of hypertension. In study 3, we examined the relationship between factors of Health belief model and outcomes of the education to delineate the meaningful factors to induce the effect of education. The questionnaire used in the study was utilized to extract the factors of Health belief model and 104 participants completed the questionnaire and outcome measurements were included in this study. Correlation analysis showed that perceived threat at baseline was only factor to associate with systolic blood pressure(r=0.2165, p<0.05). However, none of the changes in factors of Health belief model was not correlated with the change of systolic blood pressure during the education periods. Considering the significant decrease of systolic blood pressure with the decrease in DFQ, change in DFQ was assessed as another outcome of the study. Although any factors of Health belief model was not correlated with the change of DFQ, correlation analysis showed that perceived barrier and self-efficacy were interrelated at all the time points(r=-0.393, p<0.001 at baseline, r=-0.317, p<0.001 at 8 weeks and r=-0.454, p<0.001 at 16 weeks) and perceived barrier was positive correlation with DFQ score (r=0.363, p<0.001 at baseline, r=0.359, p<0.001, 0.254, p<0.01). Therefore, strategies lowering perceived barrier is necessary to enhance self-efficacy to practice low sodium intake. In Conclusion, the developed nutrition education program for hypertension management was effective to improve health indices and dietary behavior when implemented at public health center, and lowering perceived barrier was a critical factor to enhance self-efficacy to reduce sodium intake.

Correlation analysis showed that perceived threat at baseline was only factor to associate with systolic blood pressure(r=0.2165, p<0.05). However, none of the changes in factors of Health belief model was not correlated with the change of systolic blood pressure during the education periods. Considering the significant decrease of systolic blood pressure with the decrease in DFQ, change in DFQ was assessed as another outcome of the study. Although any factors of Health belief model was not correlated with the change of DFQ, correlation analysis showed that perceived barrier and self-efficacy were interrelated at all the time points(r=-0.393, p<0.001 at baseline, r=-0.317, p<0.001 at 8 weeks and r=-0.454, p<0.001 at 16 weeks) and perceived barrier was positive correlation with DFQ score (r=0.363, p<0.001 at baseline, r=0.359, p<0.001, 0.254, p<0.01). Therefore, strategies lowering perceived barrier is necessary to enhance self-efficacy to practice low sodium intake. In Conclusion, the developed nutrition education program for hypertension management was effective to improve health indices and dietary behavior when implemented at public health center, and lowering perceived barrier was a critical factor to enhance self-efficacy to reduce sodium intake.
