Dementia is characterized by gradual deterioration of cognition, function, and communication, and these factors are considered decisive disability in making decisions. Most dementia patients lose their ability to make important decisions as the disease advances, and they often include family caregivers who know their preference in decision making. Shared decision making can be more useful for dementia patients who have lowered decision making capacity and need help as their cognition function gets harmed. However, our current society is given little information necessary for evaluating dementia patients' decision making capacity which is important for share decision making. As a result, family caregivers justify participation by patients based on their perception about patients' decision making capacity. When dementia diagnose is made and symptoms show, there are many cases where dementia patients are hastily excluded from decision making. As right understanding about shared decision making reduces burden of family caregivers and minimize violation of self determination right of dementia patients, securing self determination right of dementia patients requires knowledge of identifying perception about shared decision making by family caregivers of early dementia patients. In addition, from the early stage before starting adult guardianship system which considers respect for self determination right, if family caregivers and dementia patients make efficient decisions together, the purpose of the system would be supported and the efficiency of the system use would be enhanced later. Moreover, for even dementia patients with deteriorated cognition function to be respected for their dignity and to enjoy healthy and happy later life based on their predetermined decisions, it is necessary to confirm perception of family caregivers who help dementia patients with shared decision making. However, most of the existing previous researches are conducted in foreign countries and they are about shared decision making process and inclusion, and proxy decision making. There is no research on perception about shared decision making by family caregivers of early dementia patients of actual cases.
Therefore, this study aims to identify perception about shared decision making by family caregivers of early dementia patients and contribute to education and support for better decision making.
This study was conducted on a total of 12 family caregivers of early dementia patients in a Seoul-based dementia center. They were made to make a statement about share decision making through in-depth interviews. The details of the interviews were made into transcripts and analyzed through qualitative content analysis in this qualitative study.
This study's analysis led to 6 categories and 17 subcategories, and specific findings are like the following. Perception about shared decision making by family caregivers of early dementia patients are categorized into 6 including communication facilitating measure with dementia patients, measure to secure autonomy of dementia patients, treatment facilitating chances, cause for increase in burden of family caregivers, cause for worsening relationship with dementia patients, and choices depending on changes of priorities. The subcategory found in this study regarding shared decision making include 'communication facilitating chance about daily life with dementia patients' , 'increased chance for dementia patients to express their will' , 'enhanced emotional satisfaction through consideration of dementia patients' thoughts and feelings, 'chance for dementia patients to meet their desire' , 'playing the role as a guide for desirable choice by dementia patients' , 'preparation for conflicts among family members by planning for future together' , 'chance for increasing will and effort regarding healing' , ' chance for increasing cognition by supplementing memory decline' , and 'chance for identifying changes of disease'. In addition, there are 'sense of failure regarding limits in having in-depth communication with dementia patients' , 'sense of burden regarding proxy decision making' , 'disappointment regarding limits in the role as communication helper' , 'increase in distrust about opinion of dementia patients' , 'stimulating negative emotion of dementia patients' , 'accumulation of negative perception about dementia patients due to symptoms of dementia patients during decision making' , 'induction of conflicts because of symptoms of dementia patients after making decisions' and 'judgement of necessity of shared decision making depending on changing situations'.
The findings of this study can help health care experts in their clinical efforts to identify perception about shared decision making by family caregivers of dementia patients and to provide education and support for helping better decision making by adding new knowledge to a nursing department. Moreover, the findings can be utilized in the base data important for developing practical knowledge of nursing and educational data development regarding shared decision making for family caregivers of early dementia patients, and contribute to development of a nursing department with regard to dementia patients. In terms of policies, instead of hastily excluding dementia patients in decision making upon diagnosis of dementia, having dementia patients to make efficient shared decision before the start of the adult guardianship system would lead to respect for dementia patients' self determination right and reduction in burden of family caregiver, supporting the purpose of adult guardianship system.
