Background: Schizophrenia is a recurrent, debilitating disease that is rarely curable. Especially, relapse in patient with first-episode schizophrenia lowers their quality of life and raises social costs. Rapid intervention has been shown to positively affect the prognosis and pharmacotherapy is considered to be the most important element of schizophrenia treatment. On the other hand, there is a concern that the use of antipsychotic drugs may affect the incidence of physical illness in schizophrenia patient. In Korea, however, studies are scarce on incidence of physical illness and relapse in patient with first-episode schizophrenia.
Methods: This is a retrospective study using health insurance claims data provided by the Health Insurance Review & Assessment Service from January 2011 to December 2015. The Patient with first-episode schizophrenia was defined patients who were not diagnosed with F20-29 in 2011-2013 but were then diagnosed with F20 after 2014. To identify the factors affecting relapse, we explored gender, age, geographic location, medical benefits, type of medical institution, type of medication used, medication adherence, and the severity of symptoms. Data analysis was performed using the Cox proportional hazard model.
Results: The number of patients diagnosed with first-episode schizophrenia in Korea over a 2-year period was 4,567, and 1,265 patients experienced a relapse during the observational period. Factors affecting relapse varied according to age, type of medical institution, type of medication used, medication adherence, and type of treatment (inpatient or outpatient). The incidence of metabolic disorder varied according to sex, and the incidence of back pain varied according to gender, type of medical institution, and medication adherence.
Conclusions: The results showed that the use of antipsychotic drugs and medication adherence were the main factors in preventing relapse in patients with first-episode schizophrenia. In the incidence of physical illness, however, the use of antipsychotic drugs was a major risk factor for musculoskeletal disorders such as back pain even during the short observation period. The results of this study can be utilized as source material for improving policies on patients with psychiatric disorders and directing therapeutic interventions in the future.
