Metabolic syndrome is a chronic disease in which hypertension, abdominal obesity, hyperlipidemia, and impaired fasting glucose levels occur simultaneously. Furthermore, metabolic syndrome is known to have an association with periodontal disease. Periodontal disease is highly prevalent among Korean adults (37%), and chronic gingivitis leads to the destruction of periodontal tissues and alveolar bones that support the teeth. In this study, we examined a possible mediating effects of metabolic syndrome on the relationship between factors related with pregnancy and childbirth and periodontitis. This study used raw data from the 5th (2010, 2012) and the 6th (2013, 2014) National Health and Nutrition Surveys. No oral examinations were conducted in 2011; therefore, data for 2011 were excluded from the analysis. A total of 32,584 subjects completed the health and medical examination survey. Adults aged 19 years and over, who answered the pregnancy and childbirth questionnaire, performed periodontal tissue examinations. Information on metabolic syndrome was available in 8802 of these adults, who were thus included in the study analysis. Periodontitis was defined as having 3 or more score in CPI. Risk of periodontitis was estimated by using multiple logistic regression adjusted for potential confounders and covariates, i.e., age, area, household income, smoking status, tooth brushing frequency, and metabolic syndrome. Compared to women who have been pregnant one time or none, those with more pregnancies were more likely to have periodontitis: risk of periodontitis (Odd Ratios (OR) and 95% CI (Confidence Intervals)) were 1.50 (1.12 - 2.01) for those with 2-3 times, and 1.44 (1.06 - 1.94) for those with 4 times or more pregnancies. The risk of metabolic syndrome was higher in postmenopausal compared to premenopausal women 1.28 times (95% CI: 1.05 - 1.55), females with lactation periods of 13 - 36 months and 37 months, compared to those with less than 12 months, 1.17 times (95% CI: 1.03 - 1.34) and 1.31 times (95% CI: 1.09 - 1.57), respectively, and women who took oral contraceptives compared to those who did not (1.23 times (95% CI: 1.06 - 1.42)). Moreover, the risk of metabolic syndrome was higher in women aged 25 years or less compared to those aged 26 – 30 years 0.82 times (95% CI: 0.71 - 0.93), whilst the risk of metabolic syndrome decreased as age at first birth increased; women over the age of 31 at first birth were 0.76 times (95% CI: 0.60-0.93) less likely to have metabolic syndrome, compared to women having first birth at <25 years. Metabolic syndrome patients had a significantly higher prevalence of periodontitis 1.32 times (95% CI: 1.14-1.52). Metabolic syndrome patients were then separated from the normal group, and the relationship between factors related with pregnancy and childbirth and periodontitis was examined. Women with metabolic syndrome were at greater risk of periodontitis if they were taking oral contraceptives compared to those who were not (OR (95% CI), 1.28 (1.02 - 1.61)), and at less risk of periodontitis if they were aged 26 - 30 years at first birth, compared to those aged under 25 years (OR (95% CI), 0.74 (0.58 - 0.95)). In the normal group, the risk of periodontal disease was 1.41 (95% CI: 1.02-1.97) in the group of pregnancies 2-3 times, compared to the group of pregnancy 0-1 time. Analysis of the mediating effects of metabolic syndrome on the relationship between pregnancy and childbirth and periodontitis revealed significant direct effects between age at first birth (p=< 0.024) and gestation frequency(p=<0.001). Meanwhile, significant indirect effects were found between periodontitis(p=<0.001), mediated by metabolic syndrome and the all five factors related with pregnancy and childbirth. The total effect was significant in the increasing risk of gingivitis associated with first delivery age(p=<0.013) and childbirth frequency(p=<0.001), showing significant direct effects, despite indirect effects. Our study revealed that: 1) the younger the first childbirth age and greater the number of pregnancies, the greater the direct risk of periodontitis, 2) menopausal status, oral contraceptive use, and duration of breastfeeding are not directly related to periodontitis, and 3) risk of periodontitis is indirectly increased by increasing risk of metabolic syndrome. Therefore, the management of metabolic syndrome, which is a systemic disease, is very important for the prevention and management of periodontitis in women although the number of pregnancies and the age at first birth should also be carefully considered.
