본 연구의 목적은 환자 및 보호자와 가장 많은 접촉을 하고 업무 수행 과정에서 환자의 의료정보를 직접적으로 다루는 직종인 간호사의 환자 의료정보 보안유지 강화 실천도에 영향을 미치는 요인을 도출하고, 환자 의료정보보안유지 강화를 위한 대책을 제시함에 있다. 이를 토대로 의료기관에서 간호사를 대상으로 의료정보 보호 교육을 하는데 필요한 기초자료를 제공함에 있다. 연구모형은 독립변수인 환자 의료정보 보호 인식도, 환자 의료정보 보안에 대한 법·제도적 장치, 조직윤리풍토와 종속변수인 환자 의료정보 보안유지 강화 실천도로 설계되었으며, 이를 토대로 세 가지 가설을 설정하였다. 연구대상은 서울시에 위치한 C대학병원에 근무하는 간호사를 대상으로 설문조사를 실시하였다. 설문지는 총 150부의 설문지를 배포하여 미회수 설문지와 작성이 미흡한 설문지를 합한 7부를 제외하고 143부를 분석에 이용하였으며, 주요 연구 결과는 다음과 같다. 첫째, 의료정보 보안교육경험이 있는 응답자들이 환자의료정보 보호에 대한 인식도가 의료정보 보안교육경험이 없는 응답자들보다 더 높음을 알 수 있었다. 의료정보 보호에 대한 권리를 높게 인식하고 있었으며, 환자의 의료정보를 안전하게 처리하고, 보호해야 한다는 책임감이 높게 나타나고 있음을 알 수 있었다. 둘째, 가설 검증결과 환자의료정보 보안유지 강화 실천도에 환자의료정보 보안에 대한 법·제도적 장치와 조직윤리풍토가 영향을 미치고 있으며(가설 채택), 환자의료정보 보호 인식도는 영향을 미치지 않는 것을(가설 기각) 알 수 있었다. 셋째, 환자의료정보 보안에 대한 법·제도적 장치 요인의 평균값을 분석한 결과 대부분의 평균값이 4점 이상으로 높게 나타나 환자의료정보 제공을 규정과 지침에 의해 엄격하게 제한하고 있음을 알 수 있었다. 넷째, 조직윤리풍토 요인에 대한 평균값 분석 결과 응답자들은 병원이용 고객을 위한 서비스를 가장 중시하는 것으로 나타났으며, 병원은 직원들의 규칙과 직업윤리강령 등을 엄격하게 중시하는 경향이 나타나고 있음을 알 수 있었다. 또한 응답자 각자의 판단보다는 병원 내 규정을 더욱 중시하고, 규정에 의하여 우선적으로 결정한다는 것을 알 수 있었다. 다섯째, 종속변수 환자의료정보 보안유지 강화 실천도 요인의 평균값 분석결과 의사소통 영역의 평균값이 특히 높게 나타났다. 응답자들은 환자의료정보 보안유지를 위해 ...

The research purpose of this thesis is to identify the impact factors on the Implementation level of patients’ medical information security reinforcement by the nurses and to suggest the policy remedies for their strengthening the security maintenance of patients’medical information. The research model consists of three independent variables (perception of the protection of patients’ medical information, legal/institutional system for patients’ medical information security, organizational ethical climate) and one dependent variable (strengthening the Implementation level of patients’ medical information security maintenance). Based on this model, three hypotheses were established. To test the hypotheses, a questionnaire survey was conducted targeting nurses working at Hospital of C University of Korea. Among the 150 questionnaires distributed, 143 questionnaires were used for analysis. The major research findings are as follows: Firstly, the respondents who have experienced medical information security education training have shown the higher perception level of patient‘s medical information security than those who have not. They have highly perceived the right to protect patients’ medical information along with a high sense of responsibility to safeguard and protect patients’medical information. Secondly, as results of the hypotheses test, the legal/institutional system for patient medical information security and the organizational ethical climate have significantly affected implementation level of strengthening the patient medical information security maintenance (Adoption of H2 and H3), whereas the perception level of patients’ medical information protection has not affected it(rejection of H1). Thirdly, most of the average value of the legal/institutional system, was higher than 4 points in the five-point Likert scale, which has shown that the provision of patients’ medical information was strictly restricted by the regulations and guidelines. Fourthly, the average value of the organizational ethical climate has shown the most importance of hospital customers-oriented services. Also it shows that hospitals have tended to strictly emphasize staff rules and codes of professional ethics. Furthermore, the respondents have perceived more importantly the hospital regulations or guidelines than their own judgment through the decision-making process based on the principles of hospital regulations. Finally, the average value of the implementation level of strengthening patients’ medical information security maintenance as a dependent variable has shown the high implementation level in the communication area. The respondents have shown the positive implementation level. They have been conducting patients’ medical information security maintenance in accessing medical information systems and strictly prohibiting disclosure to visitors or in public places for the security of patients’ medical information. Based on these empirical research results, the major policy implications have been recommended as follows. First of all, the regular education of patients’ medical information security should be conducted, which will result in enhancing the responsibility to safely handle and protect the patient's medical information as the ethical consciousness of the nurse's job and affecting the perception level of patients' medical information protection. Secondly, the legal/institutional systems of patients’ medical information security should be revised and newly established. The current laws and systems of patients’ medical information security are partially regulated by the Medical Law and the Pubic Health Law and they have been scattered on the individual laws with limited contents. Therefore, an independent and integral legal system of patients’ medical information security should be revised and established for unified regulations. Finally, the organizational ethical climate for strengthening the patient medical information security should more be fostered. As shown in the causal relationship (hypotheses testing) of implementation level of strengthening patients’ medical information security maintenance organizational ethical climate is more influential than the legal and institutional factors of patient information security. This implies that organizational ethical climate might affect the ethical judgment of a group. Therefore, the organizational ethical climate for strengthening the patients’medical information security maintenance should be managed well in a nurse group where ethical judgment is more important than other occupations.
