1. 조사대상자의 가구형태별 분포는 1인 가구 9.6%(584명), 2인 가구 33.9%(2,063명), 3인 이상 가구 56.5%(3,441명)로 나타났다. 성별은 남성 39.3%, 여성 60.7%로 여성의 비율이 높게 나타났다. 연령은 19-29세 1.1%, 30-39세 12.8%, 40-49세 18.6%, 50-59세 21.9%, 60-69세 22.6%, 70대 이상 23.0%로 나타났다. 소득 및 교육수준의 비율은 각각 23.3%, 31.8%로 조사되었으며 거주지역은 동 지역에 76.6%가 거주하고 있었다. 결혼상태에서는 기혼 84.1%, 미혼 15.9%로 나타났다.
2. 남성 대상자의 평균신장은 1인 가구 168.4±0.66㎝, 2인 가구 169.4±0.24㎝. 3인 이상 가구 169.3±0.18㎝ 이었으며, 여성 대상자의 평균신장은 1인 가구 155.7±0.31㎝, 2인 가구 155.9±0.22㎝, 3인 이상 가구 156.3±0.15㎝ 인 것으로 나타났다. 평균체중은 남성 대상자에서 1인 가구는 68.0±1.06㎏. 2인 가구 69.5±0.46㎏. 3인 이상 가구는 69.8±0.30㎏ 으로 나타났으며, 여성 대상자의 평균체중은 1인 가구 56.6±0.58㎏, 2인 가구 57.7±0.30㎏, 3인 이상 가구 57.2±0.24㎏으로 나타났다.
3. 가구형태에 따른 식생활을 분석한 결과 식사 빈도, 외식 빈도, 동반식사 여부, 영양표시 인지 및 이용 여부, 식생활 지원프로그램 수혜경험 여부에서 1인 가구와 비 1인 가구 간에 유의적인 차이를 보였다. 건강설문조사를 통해 대상자들의 건강관련 요인을 살펴보았을 때, 현재흡연 여부, 음주 빈도, 주관적 건강상태, 걷기운동, 근력운동, 하루 평균 수면시간, 활동 제한에서 가구형태에 따라 그룹 간에 유의적인 차이가 나타났다.
4. 대상자의 식품안정성을 분석한 결과, 식생활형편을 묻는 문항에서 가구형태에 따라 그룹 간에 유의적인 차이를 나타내었으며, 식비가 부족하여 먹을 것인 떨어진 경험, 균형 잡힌 식사를 할 수 없던 경험 등을 묻는 문항에서도 가구형태에 따른 그룹 간의 유의한 차이를 나타내었다. 이는 1인 가구는 낮은 교육수준, 저소득 및 빈곤이 식품미보장에 노출될 가능성이 높기 때문인 것으로 사료된다.
5. 1인 가구는 비 1인 가구에 비해 총 에너지 섭취랑과 단백질, 칼슘, 인, 나트륨, 칼륨, 티아민, 리보플라빈, 나이아신의 섭취비율이 유의적으로 높은 것으로 나타났으며, 조섬유와 칼슘에서는 섭취기준에 미치지 못하였다.
6. 영양소의 질적 평가를 위해 영양소적정섭취비 및 평균영양소적정섭취비를 살펴본 결과, 영양소적정섭취비 평균값인 0.78 미만인 영양소는 칼슘, 비타민 A, 리보플라빈, 비타민 C 이었다. 식사의 질과 다양성을 평가하는 영양질적지수 값이 1 이상인 영양소는 단백질(1.26±0.02), 인(1.49±0.02), 철(2.61±0.67), 나트륨(2.92±0.08), 비타민 A(1.05±0.07), 티아민(1.71±0.03)으로 가구형태에 따라 그룹 간에 유의적인 차이를 보였고(p<0.001), 영양질적지수 값이 1 미만인 영양소는 조섬유(0.34±0.01), 칼슘(0.63±0.02), 칼륨(0.86±0.02), 리보플라빈(0.91±0.02), 나이아신(0.98±0.02), 비타민 C(0.88±0.05)로 권장섭취량을 충족하지 못하는 것으로 나타났다.
7. 1인 가구의 가구원은 높은 혈중 중성지방 농도와 낮은 HDL-콜레스테롤 농도를 나타내었다. 성별로 본 가구형태의 혈액 분석치를 보면 남성 대상자에서 1인 가구는 높은 중성지방 농도를 나타냈으며, 여성 대상자에서 낮은 HDL-콜레스테롤 농도와 높은 LDL-콜레스테롤 농도를 가진 것으로 조사되었다. 또한 1인 가구 그룹에서 혈중 중성지방 농도가 정상 기준치 이상(150mg/dL 이상)인 분율과 당화혈색소 수준이 5.7% 이상인 분율이 높은 것으로 나타났다.
8. 연구대상자의 만성질환 유병률을 분석하였을 때 고혈압, 당뇨, 이상지질혈증(고콜레스테롤혈증, 고중성지방혈증), 뇌졸중, 심근경색/협심증, 신부전의 유병률이 1인 가구에서 유의적으로 높은 것으로 나타났다. 고혈압 유병률은 1인 가구 47.3%, 2인 가구 36.5%, 3인 이상 가구 18.6%로 나타났으며, 당뇨병 유병률에서는 1인 가구 19.9%, 2인 가구 14.0%, 3인 이상 가구 7.4%의 비율을 나타냈다. 이상지질혈증의 유병률은 1인 가구에서는 22.6%, 2인 가구 16.7%, 3인 이상 가구에서는 9.0%의 결과를 보였다. 가구형태에 따라 그룹 간을 비교했을 때 1인 가구의 고혈압, 당뇨병, 이상지질혈증의 유병률이 높게 나타났다. 가구형태에 따른 대상자들의 대사증후군 위험도 분석에서는 1인 가구의 37.2%가 대사증후군으로 진단되었으며, 2인 가구는 35.1%, 3인 이상 가구에서는 25.8%가 대사증후군을 가지고 있는 것으로 조사되어 1인 가구에서 대사증후군의 위험성이 높은 것으로 나타냈다.
본 연구결과에서 식생활 및 건강상태는 가구형태에 따라 영향을 받는 것으로 나타났다. 대부분의 1인 가구는 식생활이 불규칙하고, 외식에 대한 의존도가 높은 것을 알 수 있었다. 외식 빈도가 높은 식생활 행태로 인해 나트륨을 목표섭취량 이상으로 섭취하는 비율이 높게 나타났으며, 식품안정성은 상대적으로 낮게 나타났다. 또한 영양소 섭취상태는 비타민 C, 칼슘 섭취량에서 영양부족의 위험이 있는 것으로 조사되었다. 본 연구에서는 전국 규모의 대표성을 지닌 국가조사인 국민건강영양조사 자료를 이용하여 1인 가구의 식생활 및 건강상태를 분석하여 그 특성을 파악하였다. 최근 1인 가구가 크게 증가되고 있는 시점에서 1인 가구의 건강 및 식생활에 대한 특성을 전국 규모로 파악한 것은 의미가 있으리라 사료되며, 이는 1인 가구의 식생활 문제를 개선하기 위한 기초자료가 될 수 있을 것으로 여겨진다. 그러나 본 연구에서는 국민건강영양조사 자료 중 식생활 및 영양소 관련 지표만을 분석하여 식사습관 등 통합적인 식생활 지표 분석과는 연계하지 못했다는 한계점을 가지고 있다. 본 연구는 국민건강영양조사 자료에서 가구형태별 가구원의 건강 및 식생활 상태를 알 수 있으나, 대상자의 연령 등 세분화된 요인에 따른 분석이 다양하게 이루어지지는 못하였다. 따라서 향후에는 다양한 연령층을 일정 비율로 포함하여 건강 및 식생활 변인들과의 관련성을 심층 분석하는 후속 연구가 이루어져야 할 것으로 사료된다.

This study analyzed the health and dietary life of members according to household type, using the 6th National Health and Nutrition Survey data (2003-2004). The subjects of this study were 6,088 adults aged at or over 19(2,392 males and 3,696 females), and were classified according to household type. General characteristics, anthropometric values, nutrient intake and prevalence of chronic disease of the subjects were analyzed according to the household type. The main results of the analysis are as follows.
1. The distribution of household type is shown as 9.6%(584 persons) for single household, 33.9%(2,063 persons) for two-person household, and 56.5%(3,441 persons) for more than three household members. Female subjects are far more than males, accounting for 60.7% of the total and 39.3% for males. The age bands are categorized as 19-29(1.1%), 30-39(12.8%), 40-49 (18.6%), 50-59(21.9%), 60-69(22.6%) and over 70(23.0%). The income and educational level ratios are 23.3% and 31.8%, respectively, and 76.6% of the total subjects live in 'dong' area. 84.1% the total subjects are married and 15.9% single.
2. The average height of male subjects is shown as 168.4±0.66㎝ for single household, 169.4±0.24㎝ for two-person household and 169.3±0.18㎝ for three-person & over household or larger. The average height of female subjects is shown as 155.7±0.31㎝ for single household, 155.9±0.22㎝ for two-person household and 156.3±0.15㎝ for three-person & over household or larger. The average weight of male subjects is shown as 68.0±1.06kg for single household, 69.5±0.46kg for two-person household and 69.8±0.30㎏ three-person & over household or larger. The average weight of female subjects is shown as 56.6±0.58㎏ for single household, 57.7±0.30kg for two-person household and 57.2±0.24kg three-person & over household or larger.
3. As a result of the dietary pattern analysis by household type, a significant difference showed, between single households and multiple-family households, in meal frequency, frequency of eating out, family meal time, awareness of nutritional information and experience in a support program. When looking at health factors of the subjects through a health survey, some significant differences showed by household type in smoking status, drinking frequency, subjective health status, walking exercise, strength exercise, average daily sleeping hours and activity limitation.
4. As a result of food stability of the target subjects, a significant difference showed among household sizes on 'food afford-ability' questions, e.g., not enough money for food or unable to keep well balanced healthy diet. This is because a single household is likely to show a low education level, income level and poor financial condition.
5. Compared to multiple-family households, a single household significantly showed a high intake of protein, calcium, phosphorus, sodium, potassium, thiamin, riboflavin, and niacin as well as the total calorie while crude fiber and calcium far below the standard intake.
6. As a result of looking at appropriate nutrient intake ratio and average nutrient intake ratio for quality assessment of nutrients, nutrients with a nutrient titration value less than 0.78 were calcium, vitamin A, riboflavin and vitamin C. Nutrients with a nutritional quality index higher than 1 to evaluate the quality and variety of meal are protein (1.26±0.02), phosphorus (1.49±0.02), iron (2.61±0.67), sodium (2.92±0.08), vitamin A (1.05±0.07) and thiamin (1.71±0.03), respectively. There were significant differences between the groups by household type (p<0.001). Nutrients with a nutritional quality index less than 1 are crude fiber(0.34±0.01), calcium(0.63±0.02), potassium(0.86±0.02), riboflavin(0.91±0.02), niacin(0.98±0.02) and vitamin C(0.88±0.05), all of which are unmet to the daily recommended intake.
7. Single households showed a high blood triglyceride level and low HDL-cholesterol concentration. Looking at the blood analysis of each household type by gender, the survey data showed that male subjects of single households had a high triglyceride level, and females had low HDL-cholesterol and high LDL-cholesterol. Also single households showed 5.7% of blood triglyceride level, above the normal level(150mg/dL or higher) and glycated hemoglobin level above 5.7%.
8. Analyzing the prevalence of chronic diseases among subjects, single households significantly showed the high prevalence of hypertension, diabetes, dyslipidemia (hypercholesterolemia, hypertriglyceridemia), stroke, myocardial infarction/ angina and renal failure. The prevalence of hypertension showed 47.3% in single households, 36.5% in two-person households, 18.6% in households of more than three while the prevalence of diabetes showed 19.9% in single households, 14.0% in two-person households, 7.4% in households of more than three. The prevalence of dyslipidemia resulted in 22.6% in single households, 16.7% in two-person households, 9.0% in households of more than three. Comparing with different household types, single households showed the high prevalence of hypertension, diabetes and dyslipidemia. In the risk analysis of metabolic syndrome by household type, 37.2% of single households were most highly diagnosed with metabolic syndrome, followed by 35.1% of two-person households and 25.8% of three-person & over households or larger.
This study showed that dietary habits and health are influenced by household type. Most single households were found to have irregular eating habits and heavily rely on eating out. Their frequent eating out causes a higher sodium intake than a recommended daily amount and relatively low food stability. They were also found to be at risk of deficient vitamin C and calcium.
This study analyzed the dietary life and health of single households, using National Health and Nutrition Survey data representing the whole nation in sample size. It is certainly significant to identify the characteristics of the health and dietary habits of single households on a national scale amid recent increase in single-person households. This study can provide primary data for improving the health and dietary habits of single households. However, this study analyzed only the dietary and nutritional indicators of the National Health and Nutrition Survey data, limitedly unable to associate with the integrated dietary index analysis. In this study, the health and dietary status of household members were identified by household type through the National Health and Nutrition Survey data, but other specific factors such as age were not included for a broad analysis. Therefore, an in-depth analysis needs to be carried out for corelation between health and other dietary variables in further studies, including a wide range of age groups at a certain rate.

It is certainly significant to identify the characteristics of the health and dietary habits of single households on a national scale amid recent increase in single-person households. This study can provide primary data for improving the health and dietary habits of single households. However, this study analyzed only the dietary and nutritional indicators of the National Health and Nutrition Survey data, limitedly unable to associate with the integrated dietary index analysis. In this study, the health and dietary status of household members were identified by household type through the National Health and Nutrition Survey data, but other specific factors such as age were not included for a broad analysis. Therefore, an in-depth analysis needs to be carried out for corelation between health and other dietary variables in further studies, including a wide range of age groups at a certain rate.
