근육은 월등한 자가 치유능력을 지녔지만, 자동차 사고, 전쟁으로 인한 극심한 손상을 입었을 경우, 회복이 불가능하며, 이는 대체적 근육 손상이라 불리운다. 임상적 치료방법으로, 자가 근육을 이식하는 시술이 이루어지고 있으나, 공여부의 합병증, 이식 가능한 근육의 부족, 이식 후 기능 저하 등의 한계를 보이고있다. 현재 조직공학적 접근을 통한 3차원하이드로젤 기반 근육 구조체가 대체적 치료 방법으로써 제시되고 있으나, 구조체가 산소와 영양분을 확산에 의존하고 있기 때문에, 대면적을 갖는 구조체를 제작할 수 없는 한계가 지적되고 있다. 탈세포화 세포외기질 인공지지체는 우수한 재생능을 기반으로 현재까지 28명의 대체적 근육 손상 환자에게 적용되었고, 근기능 및 근질량 향상을 보였다. 하지만 근육의 완전한 기능회복 및 재생을 달성하지는 못하였고, 이는 이식부의 매우 낮은 수의 위상세포와 정렬되지 않은 인공지지체의 구조로 인한 상처조직의 형성 때문이란 연구들이 보고되고 있다. 따라서 본 연구에서는 우수한 재생능을 지닌 탈세포화 세포외기질을 세포를 봉입할 수 있는 바이오 잉크로 제작하고, 세포를 원하는 형상으로 배열 할 수 있는 3차원 세포 프린팅 기술을 이용하여 3차원 대면적 근육 구조체를 제작하고 이를 대체적 근육 손상에 적용하여 그 재생능과 가능성을 평가하였다. 본 연구의 상세내용은 다음과 같다. 근육 세포에게 적절하고 효과적인 미세환경을 모사하기 위해 근육 조직에서 유래된 근육 탈세포화 세포외기질 바이오잉크를 개발하였다. 개발된 바이오잉크로 제작된 3차원 근육 구조체는 세포의 높은 생존, 증식, 분화, 성숙을 보였다. 한편, 3차원 세포 프린팅 기술을 통해 근육 구조체 내부의 세포의 배열을 효과적으로 조절하여 본래 조직과 유사한 정렬된 근관 섬유의 형성을 달성하였다. 탈세포화 세포외기질 바이오잉크는 낮은 점도, 기계적 강도 등으로 인해 정밀하고 대면적을 갖는 구조를 제작하기 매우 어렵다. 이를 극복하기 위해 과립 기반 프린팅 지지 수조를 개발하였다. 탈세포화 세포외기질 바이오잉크가 과립 기반 프린팅 지지 수조 내부에서 프린팅 될 때, 수조에 포함된 응고제는 바이오잉크를 즉시 고형화 시켜 적층을 가능하게 하였다. 응고제로 인한 바이오잉크의 화학적 변화는 일어나지 않았으며, 바이오 잉크의 기계적 강도 향상을 촉진하였다. 과립 기반 프린팅 지지 수조를 통해 다양한 형태의 대면적을 갖는 3 차원 세포 구조체를 제작 할 수 있었다. 앞서 개발된 바이오 잉크와 과립 기반 프린팅 지지 수조를 이용하여 세포 프린팅 된 대면적 근육 구조체를 제작하였다. 세포 프린팅된 대면적 근육 구조체에서는 저산소증이 발생 하지 않았고, 높은 세포 생존능과 정렬된 근관 섬유 형성을 확인하였다. 대면적 근육 구조체의 재생능을 검증하기 위해 쥐의 대체적 근육 손상 모델에 이식하였다. 세포 프린팅 대면적 근육 구조체에서 새로운 근육 섬유 재생을 확인하였으나, 정상 근육에 비해 상대적으로 낮은 근기능 회복을 보였다. 근기능 회복을 촉진하기 위해, 동축 노즐을 이용하여 혈관화된 근육 구조체를 제작하였다. 동축 노즐을 통해 제작된 혈관화된 근육 구조체는 근육과 혈관이 구획화 된 구조를 보였으며, 근육 세포와 혈관세포는 각각 근관 섬유, 혈관세포네트워크를 형성하였다. 혈관화된 근육 구조체는 대체적 근육 손상에서 매우 뛰어난 신생 근육 형성, 혈관화, 및 신경화를 이루었고, 정상 근육의 약 85 %에 해당되는 높은 근 기능 회복을 달성하였다. 본 연구에서 개발된 조직 유래 바이오잉크와 대면적 구조체 제작 방법, 그리고 혈관 생성 방식은 대체적 조직 손상 치료의 적용뿐만 아니라, 약물 개발 및 독성 평가를 위한 인체 규모의 조직 개발의 청사진을 제공할 것이다.

3D printing is an emerging technology that seeks to organize biomaterials (bioinks) and cells into physiologically relevant geometries within supportive tissue microenvironments. Bioinks refer to cell-encapsulating biomaterials (usually hydrogels) that allow the printed mass to be constructed into a 3D form, as well as provide a cell matrix to substitute or mimic native tissue. Therefore, a suitable microenvironment providing physical, chemical, and biological cues for accelerating the tissue formation can be controlled by the selection of bioinks. Decellularized extracellular matrix (dECM) bioinks, which directly reconstituted from decellularization of native tissues, has shown great promise for supporting cell adhesion, proliferation, and maturation by providing a natural 3D microenvironment in which cells could grow and function. Although dECM bioinks support robust cellular function for tissue regeneration, their relatively weak mechanical integrity and low shape fidelity are considered challenges to build large-volume dECM-based tissue constructs. This thesis delineates development of skeletal muscle-derived dECM (mdECM) bioink and its 3D cell printing application. The present work also shows that development of a granule-based printing reservoir which enables to create a large-volume construct using soft dECM bioinks without losing structural fidelity. Finally, the present dissertation describes the evaluation of the regenerative capacity of the large-volume constructs by applying volumetric muscle loss (VML) model of rat tibialis anterior muscle. Details on the research are given below. The goal of decellularization is to minimize cellular components and maximize ECM components after processing. Tissues were primarily derived from porcine tibialis anterior muscles, and they are treated with several detergents, enzymes, and solvents. The histological and biochemical analyses showed the mdECM retained major ECM components, such as laminin, collagen, and GAGs, while cellular components were removed. Moreover, the myogenic potential of the mdECM bioink was demonstrated by mechanical and biological analyses. To overcome the weak mechanical integrity and low resolution of dECM bioink and to fabricate large-volume constructs, a granule-based printing reservoir was developed. A coagent solution contained in a granule-based printing reservoir supported rapid precipitation of collagenous proteins in dECM bioink and allowed the building of large free-standing 3D printed tissue constructs. The regenerative capacity of the 3D large-volume muscle constructs was demonstrated by treating the rat model of VML injury. The muscle cells and endothelial cells were printed through a coaxial nozzle to generate prevascularized muscle constructs. The prevascularized muscle construct exhibited 85% of functional recovery as well as newly formed organized muscle tissue at 4 weeks of post-implantation. The present dissertation demonstrates the therapeutic potential of the 3D cell printed large-volume muscle construct and the dECM bioink for VML injuries. In addition, the granule-based printing reservoir can be applied not only to mdECM bioink, but also to various soft tissue based bioinks such as fat, skin, and cartilage, enabling the construction of large-volume constructs while maintaining the superior functionality of dECM bioinks
