Purpose : This study seeks to identify the relation among patient safety culture, nursing practice environment and safety nursing activities to develop measures to improve patient safety nursing practices.
Method : The study was conducted on 150 nurses working at hospitals with 300 beds or more, located in Gyeongsangnam Province. Data were collected through a self-reported questionnaire from March 23 to April 1, 2018. The tools used were the 'Questionnaire on Patient Safety' translated into Korean by Jeongeun Kim et al(2007) which is a translation of the Hospital Survey on Patient Safety Culture developed by AHRQ(2004),‘Korean Version of the Practice Environment Scale of Nursing Work Index for Korean Nurses' translated by Eunhee Cho et al(2011) and originally developed by Lake(2002), and the 'Questionnaire on safety nursing activities' developed by Korea Institute for Healthcare Accreditation and revised by Miyoung Han and Myeonsuk Jeong (2015). The collected data were analyzed using SPSS/WIN ver 24.0 for technical statistical analysis, t-test, ANOVA and Pearson's Correlation Analysis.
Results: 1) The patient safety culture score for subjects was an average of 3.39(±0.03) points on a scale of 5. In terms of the sub-categories, ‘Frequency of Events reported' scored the highest at 3.92(±0.66) points, and ‘Work Area/Unit' scored the loweset at 3.26(±0.31) points. 2) The perception of subjects towards nursing practice environment was a positive 2.56(±0.38) points on a scale of 4. In terms of the sub-categories, ‘Nurse manager ability, leadership, and support of nurses’ and ‘Nursing foundations for quality of care’ scored 2.73(±0.46) points and 2.73(±0.39) points, respectively, while ‘Staffing and resources adequacy' scored the lowest at 2.12(±0.53). 3) Safety nursing activities of subjects scored 4.43(±0.47) on a scale of 5. In terms of the sub-categories, ‘Identification of patient’ was the highest at 4.54(±0.54), while ‘Communication’ was the lowest at 4.19(±0.63). 4) There was statistically significant difference in patient safety culture across different ‘age’ (F=4.75, p=.001) and ‘clinical career’ (F=3.80, p=.003). Of the general characteristics of subjects, there was statistically significant difference in nursing practice environment across ‘age’ (F=4.06, p=.004) and ‘clinical career’ (F=5.55, p<.001) as well as in safety nursing activities across different ‘age’ (F=2.58, p=.040) and ‘clinical career’ (F=3.19, p=.009). 5) Subjects’ nursing practice environment and patient safety culture had a positive correlation (r=.531, p<.001), while the nursing practice environment and safety nursing activities had a positive correlation (r=179, p=.029). Conclusion : Based on the above findings, it was verified that a higher perception of nursing practice environment was correlated with a higher score on safety nursing activities, while a higher patient safety culture was correlated with a higher safety nursing activities. Moreover, among the sub-categories of nursing practice environment, ‘Nursing foundation for quality of care’, ‘Nurse manager ability , leadership, and support of nurses’ was correlated with higher safety nursing activities. A higher age and longer clinical career was associated with a higher level of patient safety culture, nursing practice environment and safety nursing activities. Therefore, improved safety nursing activities through better nursing practice environment and patient safety culture can help improve the quality of healthcare and the safety of the healthcare environment.
