This study investigated effective methods for prevention of dental erosion caused by fermented milk, which is an acidic beverage. Three methods were set as experimental groups: (1) Adding high- or low-concentration calcium to fermented milk (Ca 2% and Ca 0.5% groups); (2) High-concentration fluoride application (once) or low- concentration fluoride application (everyday) on teeth prior to exposure to fermented milk (APF gel and NaF 0.05% groups); (3) Mixed method of adding low-concentration calcium to fermented milk and high- or low-concentration fluoride application on teeth (APF gel+Ca 0.5% and NaF 0.05%+Ca 0.5% groups). These six experimental groups and two control groups (mineral water as the negative control and untreated fermented milk as the positive control) were prepared, and 12 bovine teeth specimens were assigned to each group. The specimens were immersed for five minutes four times a day for five days, and stored in artificial saliva in-between immersion cycles. Dental erosion prevention effects were evaluated by measuring the enamel surface microhardness and roughness of the specimens after the 5-day immersion treatment. Statistical analysis of the measurement data and surface morphology observation yielded the following findings:
1. Intergroup comparison of the changes in the enamel surface microhardness of the specimens before and after the 5-day immersion treatment revealed statistically significant intergroup differences (P0.05). And the Ca 0.5%, APF gel, APF gel+Ca 0.5%, NaF 0.05%, and NaF 0.05%+Ca 0.5% groups showed significant differences in changes of surface microhardness from that of the negative control group (mineral water) and positive control group (fermented milk) (P<0.05).
2. Intergroup comparison of the differences in the enamel surface roughness of the specimens between the immersed and non-immersed parts after the 5-day immersion treatment revealed statistically significant intergroup differences (P0.05). And the Ca 0.5%, APF gel+Ca 0.5%, and NaF 0.05% groups showed significant differences in changes of surface roughness from that of the negative control group (mineral water) and positive control group (fermented milk) (P0.05).
3. In the atomic force microscope (AFM) observation of 2D and 3D images of the surface morphology of the immersed and non-immersed parts after the 5-day immersion cycles, no differences were found in the negative control group (mineral water) and Ca 2%. The NaF 0.05%+Ca 0.5% group did not exhibit any differences in 2D images, but slightly damaged surfaces were observed in 3D images. The remaining five groups exhibited clear surface damage.
4. In the scanning electron microscope (SEM) observation of the surface morphology of the immersed and non-immersed parts after the 5-day immersion treatment, no differences in surface morphology were found in the negative control group (mineral water), Ca 2%, and NaF 0.05%+Ca 0.5% groups after the treatment, which maintained smooth and regular surface morphology. However, the remaining five groups were observed to have undergone morphological damage on the enamel surface, including cracks and irregular crystals.
We assessed preventive methods of dental erosion due to fermented milk using sound enamel. There were no significant differences in surface microhardness, surface roughness, and surface morphology after immersed the specimens in fermented milk containing 2% calcium. However, considering that 2% calcium is highly concentrated, adding low-concentration calcium to fermented milk and daily low-concentration fluoride application on teeth lowered the reduction of surface microhardness and reduced the increase of surface roughness without significant changes in surface morphology, confirming its potential preventive effects against dental erosion. We believe that these results would contribute to proposing effective preventive methods and appropriate alternatives to individuals at high risk of dental erosion.
