The purpose of this study is to analyze the indirect effects of COVID-19 on the treatment of patients with non-infectious disease by comparing the incidence of complicated appendicitis before and after the pandemic. METHODS: The target group included patients aged above 16 years and diagnosed with acute appendicitis between February 23 and July 31, 2020. The same group of patients diagnosed during the same period in 2019 was selected as the control group. Complicated appendicitis was defined according to the laparoscopic grading system of acute appendicitis stages 3 to 5. Based on the collected data, the difference in the incidence of complicated appendicitis before and after COVID-19 was investigated, and the association with various variables was analyzed using odds ratios and 95% confidence intervals (CI). RESULTS: The study included 120 subjects from February 23 to July 31, 2019 (pre-COVID group) and 119 cases from February 23 to July 31, 2020 (post-COVID group). The pre-COVID group included 25 cases (20.8%), and the post-COVID group included 48 cases (40.3%) of complicated appendicitis. The time from symptom onset to visit (pre-hospital time) and the time from visit to surgery (in-hospital time) were compared. The median pre-hospital time increased from 15 h (IQR, 8.00-34.25) to 22 h (IQR, 9.00-47.00), but was not statistically significant (p=0.058). The median in-hospital time increased from 7 h (IQR, 4.00-10.00) to 11 h (IQR, 7.50-17.00) and was statistically significant (p<0.001). Based on multivariate regression analysis of the three variables, the odds ratios of pre-hospital time, erythrocyte sedimentation rate, and post-corona group were: 1.48 (95% CI 1.28-1.70, p<0.001), 1.79 (95% CI 1.25-2.57, p=0.002), and 2.73 (95% CI 1.44-5.14, p=0.002), respectively. CONCLUSION: During the COVID-19 pandemic, the treatment of patients in need of emergency medical care such as acute appendicitis is being affected. Therefore, an efficient infection management system is needed to resolve patients' anxiety about contagion in medical institutions and minimize delays involving treatment of non-infectious emergencies in hospitals.
