본 연구는 관절경하 회전근개 봉합술을 받은 남성 환자 중 전방 어깨 자세를 가진 환자를 대상으로 운동 프로그램에 따른 수술 후 어깨관절의 통증, 어깨관절 가동범위, 기능적 회복, 근력, 및 회전근개 근육의 단면적 변화를 비교 분석하여 자가 교정 운동프로그램의 효과성을 검증하고 전방 어깨 자세를 가진 회전근개 봉합술 환자들의 수술 후 빠른 기능적 회복에 도움을 주는데 그 목적이 있다. 본 연구는 전방 어깨 자세를 가진 관절경하 회전근개 봉합술을 받은 남성 환자 총 84명을 대상으로 자가 교정운동 집단 29명, 자가 회전근개 근력운동 집단 27명, 회전근개 봉합술만 받은 통제 집단 28명을 대상으로 실시했다. 수술 후 10주째 시점에서 운동 교육 1회 50분간 실시하였다. 어깨관절의 근력과 어깨관절의 가동범위 및 기능 평가는 회전근개 봉합술 후 6개월, 12개월 시점에서 실시하였으며 근단면적은 측정을 위해 MRI촬영은 수술 전과 수술 후 6개월 시점에서 평가하였다. 1회 운동 교육 후 집에서 운동 프로그램의 참여에 대한 순응을 높이기 위해 운동 프로그램 동영상을 배포하였으며 전화 교육(Telerehabilitation)을 실시하였다.
본 연구의 결과는 다음과 같다.
1. 운동 프로그램에 따른 통증 및 기능 척도의 변화

1. 운동 프로그램에 따른 통증 및 기능 척도의 변화
1) 운동 프로그램에 따른 그룹간 통증 시각 아날로그 척도 변화를 분석한 결과 그룹간의 차이가 있는 것으로 나타났다(P<.001). 자가 교정운동 그룹과 자가 회전근개 근력운동 그룹간의 통증 시각 아날로그 척도는 통계적으로 유의한 수준을 나타냈으며(P<.01), 자가 교정운동 그룹과 통제집단의 통증 척도 차이 또한 유의한 차이를 나타냈다(P<.01). 하지만 자가 회전근개 근력운동 그룹과 통제집단의 통증 척도는 유의한 차이가 없었다(P=.751).
2) 운동 프로그램에 따른 그룹간 기능 시각 아날로그 척도 변화를 분석한 결과 그룹간의 차이가 있는 것으로 나타났다(P<.01). 자가 교정운동 그룹과 자가 회전근개 근력운동 그룹간의 기능 시각 아날로그 척도는 통계적으로 유의한 수준을 나타내지 못하였으며(P=.213) 자가 교정운동 그룹과 통제집단의 기능 척도는 유의한 차이를 나타냈다(P<.01). 하지만 자가 회전근개 근력운동 그룹과 통제집단의 기능 척도는 유의한 차이가 없었다(P=.531).
2. 운동 프로그램에 따른 어깨관절 가동범위의 변화
1) 운동 프로그램에 따른 그룹간 전방거상 범위 변화를 분석한 결과 그룹간의 차이가 없는 것으로 나타났다(P=.234).
2) 운동 프로그램에 따른 그룹간 외회전 범위 변화를 분석한 결과 그룹간의 차이가 있는 것으로 나타났다(P<.05). 운동프로그램에 따른 외회전 범위의 사후 분석결과 각 그룹간의 외회전 범위의 차이는 없는 것으로 나타났다.
3) 운동 프로그램에 따른 그룹간 내회전 범위의 변화를 분석한 결과 그룹간의 차이가 없는 것으로 나타났다(P=.197).
4) 운동 프로그램에 따른 그룹간 어깨관절의 외전 범위의 변화를 분석한 결과 그룹간의 차이가 없는 것으로 나타났다(P=.217).
3. 운동 프로그램에 따른 어깨관절 근력 변화
1) 운동 프로그램에 따른 그룹간 Scaption 근력 비율 변화를 분석한 결과 그룹간의 차이가 있는 것으로 나타났다(P<.01). 자가 교정운동 그룹과 자가 회전근개 근력운동 그룹간의 Scaption 근력 비율는 통계적으로 유의한 수준을 나타냈으며(P<.05), 자가 교정운동 그룹과 통제집단의 Scaption 근력 비율 차이 또한 유의한 차이를 나타냈다(P<.05). 하지만 자가 회전근개 근력운동 그룹과 통제집단의 Scaption 근력 비율은 유의한 차이가 없었다 (P=.954).
2) 운동 프로그램에 따른 그룹간 외회전 근력 비율 변화를 분석한 결과 그룹간의 차이가 있는 것으로 나타났다(P<.01). 자가 교정운동 그룹과 자가 회전근개 근력운동 그룹간의 외회전 근력 비율은 통계적으로 유의한 수준을 나타냈으며(P<.05), 자가 교정운동 그룹과 통제집단의 외회전 근력 비율 차이 또한 유의한 차이를 나타냈다(P<.01). 하지만 자가 회전근개 근력운동 그룹과 통제집단의 외회전 근력 비율은 유의한 차이가 없었다 (P=.830).
3) 운동 프로그램에 따른 그룹간 내회전 근력 비율 변화를 분석한 결과 그룹간의 차이가 있는 것으로 나타났다(P<.001). 자가 교정운동 그룹과 자가 회전근개 근력운동 그룹간의 내회전 근력 비율은 통계적으로 유의한 수준을 나타내지 못하였다(P=.076). 하지만 자가 교정운동 그룹과 통제집단의 내회전 근력 비율 차이는 유의한 차이를 나타냈다(P<.01). 마지막으로 자가 회전근개 근력운동 그룹과 통제집단의 내회전 근력 비율 유의한 차이가 없었다(P=.273).
4. 운동 프로그램에 따른 어깨관절 기능점수의 변화
1) 운동 프로그램에 따른 그룹간 ASES 어깨기능점수 변화를 분석한 결과 그룹간의 차이가 있는 것으로 나타났다(P<.001). 자가 교정운동 그룹과 자가 회전근개 근력운동 그룹간의 ASES 어깨기능점수는 통계적으로 유의 수준을 나타냈다(P<.001). 그리고 자가 교정운동 그룹과 통제집단의 ASES 어깨기능점수 차이도 유의한 차이를 나타냈다(P<.01). 그러나 자가 회전근개 근력운동 그룹과 통제집단의 ASES 어깨기능점수는 유의한 차이가 없었다(P=.996).
2) 운동 프로그램에 따른 그룹간 Constant 어깨기능점수 변화를 분석한 결과 그룹간의 차이가 있는 것으로 나타났다(P<.01). 자가 교정운동 그룹과 자가 회전근개 근력운동 그룹간의 Constant 어깨기능점수는 통계적으로 유의 수준을 나타냈다(P<.05). 그리고 자가 교정운동 그룹과 통제집단의 Constant 어깨기능점수 차이도 유의한 차이를 나타냈다(P<.01). 그러나 자가 회전근개 근력운동 그룹과 통제집단의 Constant 어깨기능점수는 유의한 차이가 없었다(P=.975).
3) 운동 프로그램에 따른 그룹간 KSS 어깨기능점수 변화를 분석한 결과 그룹간의 차이가 있는 것으로 나타났다(P<.01). 자가 교정운동 그룹과 자가 회전근개 근력운동 그룹간의 KSS 어깨기능점수는 통계적으로 유의 수준을 나타냈다(P<.05). 그리고 자가 교정운동 그룹과 통제집단의 KSS 어깨기능점수 차이도 유의한 차이를 나타냈다(P<.01). 그러나 자가 회전근개 근력운동 그룹과 통제집단의 KSS 어깨기능점수는 유의한 차이가 없었다 (P=.915).
5. 운동 프로그램에 따른 회전근개 근단면적 변화
1) 운동 프로그램에 따라 극상근의 근단면적 변화에 유의한 차이가 있는 것으로 나타났다 (P<.01). 자가 교정운동 그룹과 자가 회전근개 근력운동 그룹간의 극상근 단면적의 변화는 통계적으로 유의 수준을 나타냈다(P<.05). 그리고 자가 교정운동 그룹과 통제집단의 극상근 단면적의 변화도 유의한 차이를 나타냈다(P<.01). 그러나 자가 회전근개 근력운동 그룹과 통제집단의 극상근 단면적은 유의한 차이가 없었다 (P=.810).
2) 운동 프로그램에 따라 극하근의 근단면적 변화에 유의한 차이가 있는 것으로 나타났다 (P<.001). 자가 교정운동 그룹과 자가 회전근개 근력운동 그룹간의 극하근 단면적의 변화는 통계적으로 유의 수준을 나타냈다(P<.05). 그리고 자가 교정운동 그룹과 통제집단의 극하근 단면적의 변화도 유의한 차이를 나타냈다(P<.01). 그러나 자가 회전근개 근력운동 그룹과 통제집단의 극하근 단면적은 유의한 차이가 없었다 (P=.352).
3) 운동 프로그램에 따라 견갑하근의 근단면적 변화에 유의한 차이가 있는 것으로 나타났다 (P<.01). 자가 교정운동 그룹과 자가 회전근개 근력운동 그룹간의 견갑하근 단면적의 변화는 유의한 차이가 없었다 (P=.265). 그리고 자가 교정운동 그룹과 통제집단의 견갑하근 단면적의 변화는 유의한 차이를 나타냈다(P<.01). 그러나 자가 회전근개 근력운동 그룹과 통제집단의 극하근 단면적은 유의한 차이가 없었다 (P=.224).
본 연구 결과를 종합해보면 전방 어깨 자세를 가진 회전근개 봉합술 환자에게 통증의 감소, 어깨의 근력 및 기능적 향상에 있어 자가 교정운동 프로그램의 적용이 효과적인 중재라는 것을 알 수 있다. 회전근개 봉합술을 받은 환자에게 자세적 특성을 고려한 선택적인 운동 프로그램의 적용이 반드시 필요하고 본 연구의 결과는 이를 뒷받침하는 객관적인 자료를 제시해 줄 수 있을 것이라 생각된다.

The purpose of this study was to evaluate pain in shoulder joint, range of motion, functional recovery, muscle strength, and change in cross-sectional area in the rotator cuff muscles following an exercise program in male patients who underwent arthroscopic rotator cuff repair. Another aim of this study is to confirm the effectiveness of the self-correcting exercise program and self-rotator cuff exercise program by comparative analysis and to help patients with rotator cuff repair with forward shoulder posture to recover quickly after surgery. In this study, a total of 84 male patients who underwent arthroscopic rotator cuff repair with forward shoulder posture were enrolled and they was allocated into 3 groups: self-corrective exercise group(n=29), self-rotator muscle strength exercise group (n=27), and control group (n=28) who received only rotator cuff repair., Exercise education was performed once for 50 minutes at the postoperative 10 week. The muscle strength, range of motion, and function in the shoulder joint were evaluated at 6 and 12 months after rotator cuff repair, and MRI was evaluated before and 6 months after surgery to measure cross-sectional area of the shoulder muscles. After the exercise education, a video of the exercise program was distributed and telerehabilitation was conducted for increasing exercise compliance to home-based exercise program.
The result of this study can be summarized as below.
1. Change in the pain using visual analog scale and functional visual analog scale according to the exercise program
1) There was a significant difference between the groups (P<.001). The visual analog scale between the self-corrective exercise group and the self-rotator muscle strength exercise group showed a statistically significant level (P<.01), and the difference in the pain scale between the self-corrective exercise group and the control group also showed a significant difference (P<.01). However, there was no significant difference in pain scale between the self-rotator muscle strength exercise group and the control group (P=.751).
2) There was a difference between groups (P<.01). The functional visual analog scale between the self-corrective exercise group and the self-rotator muscle strength exercise group did not show a statistically significant level (P=.213), and the functional scale of the self-corrective exercise group and the control group showed a significant difference (P <.01). However, there was no significant difference in functional measures between the self-rotator muscle strength exercise group and the control group (P=.531).
2. Change in the shoulder range of motion according to the exercise program
1) In analysis of the change in the forward elevation between groups according to the exercise program, there was no difference between groups (P=.234).
2) There was a significant difference between the groups (P<.05). In the post-hoc analysis of the external rotation range according to the exercise program, there was no difference in the external rotation range between each group.
3) In analysis of the change in the internal rotation between groups according to the exercise program, there was no difference between groups (P=.197).
4) In analysis of the change in the shoulder abduction between groups according to the exercise program, there was no difference between groups (P=.217).
3. Change in the shoulder strength ratio according to the exercise program
1) There was a significant difference between the groups (P<.01). The ratio of scaption strength between the self-corrective exercise group and the self-rotator muscle strength exercise group was statistically significant (P<.05), and the difference in the ratio of scaption strength between the self-correction exercise group and the control group also showed a significant difference (P <.05). However, there was no significant difference in the ratio of scaption strength between the self-rotator muscle strength exercise group and the control group (P=.954).
2) There was a significant difference between groups (P<.01). The purpose of this study was to evaluate pain in shoulder joint, range of motion, functional recovery, muscle strength, and change in cross-sectional area in the rotator cuff muscles following an exercise program in male patients who underwent arthroscopic rotator cuff repair. Another aim of this study is to confirm the effectiveness of the self-correcting exercise program and self-rotator cuff exercise program by comparative analysis and to help patients with rotator cuff repair with forward shoulder posture to recover quickly after surgery. In this study, a total of 84 male patients who underwent arthroscopic rotator cuff repair with forward shoulder posture were enrolled and they was allocated into 3 groups: self-corrective exercise group(n=29), self-rotator muscle strength exercise group (n=27), and control group (n=28) who received only rotator cuff repair., Exercise education was performed once for 50 minutes at the postoperative 10 week. The muscle strength, range of motion, and function in the shoulder joint were evaluated at 6 and 12 months after rotator cuff repair, and MRI was evaluated before and 6 months after surgery to measure cross-sectional area of the shoulder muscles. After the exercise education, a video of the exercise program was distributed and telerehabilitation was conducted for increasing exercise compliance to home-based exercise program.
The result of this study can be summarized as below.
