2019년 발병된 신종 SARS-CoV-2 바이러스는 현재까지도 많은 지역에서 광범위한 지역사회 전파가 발생하고 있으며 장기화로 SARS-CoV-2 mRNA 백신은 전파와 확산을 예방할 수 있는 중요한 수단이 되고 있다. 이에 본 연구는 SARS-CoV-2 mRNA 백신의 유효성 및 안전성을 전략적 연구를 통해 분석하고자 하며 SARS-CoV-2 mRNA 백신 접종의 필요성에 대한 근거를 평가하고자 한다. 총 3개의 데이터베이스(Pubmed, Embase, Cochrane Library)를 활용하여 2022년 6월 22을 기준으로 검색하였고 검색 전략 및 PRISMA flow에 따라 총 6개의 문헌을 최종적으로 선정하였다. 선정된 문헌에서 SARS-CoV-2 바이러스 감염 이력이 없는 사람들을 대상으로 SARS-CoV-2 mRNA 백신 접종을 한 대상군과 위약 접종을 한 대상군의 유효성과 안전성 결과에 대해 체계적인 문헌 고찰 및 메타분석을 수행하였다. 유효성 평가의 지표로는 SARS-CoV-2 mRNA 백신 및 위약에 대해 2차 접종을 완료한 사람을 대상으로 SARS-CoV-2 바이러스 감염률과 면역원성을 분석하였는데, 면역원성의 경우 SARS-CoV-2 중화항체의 기하평균 역가(Geometric Mean Titer, GMT), 기하평균 증가 비율(Geometric Mean Fold Rise, GMFR)로 평가하였다. 안전성 평가의 경우 SARS-CoV-2 mRNA 백신 및 위약 접종 1차 이상을 한 대상자에게서 발생한 모든 이상 사례에 대해 평가하였다. SARS-CoV-2 mRNA 백신에 초점을 두어 현재 시판 중인 2개의 백신 (mRNA-1273, BNT162b2)의 유효성과 안전성에 대해 선정된 6개의 문헌으로 전략적 연구를 진행한 결과, 위약 접종군 대비 백신 접종군에서의 SARS-CoV-2 바이러스에 대해 상대위험도는 0.05로 95%의 높은 유효성을 보였다. 또한, SARS-CoV-2 중화항체의 기하평균 역가, 기하평균 증가 비율의 수치는 위약 접종군 대비 백신 접종군에서 높은 수치를 보였으며 기하평균 역가의 경우 모더나 백신을 접종한 사람이 더 높게 나타났으며 기하평균 증가 비율의 경우 화이자 백신을 접종한 사람이 더 높게 나타났다. 안전성 측면에서 보자면, 1차 접종과 2차 접종 후 각각 보고된 전체 예측되는 이상 사례에 대해 백신 접종군에서의 상대위험도는 각각 1.75, 2.10로 위약 접종군 대비 크게 높지 않은 발생률을 보였다. 예측되는 이상 사례 중 보고된 국소 이상 사례에 대한 1차 접종과 2차 접종 후 상대위험도는 각각 3.96, 4.95로 위약 접종군 대비 다소 높은 발생률을 보였으며 전신 이상 사례의 경우 1.28, 2.11로 위약 접종군 대비 크게 높지 않은 발생률을 보였다. 예측되지 않는 이상 사례와 중대한 이상 사례에 대해서는 각각 1.09, 1.08로 위약 접종군 대비 크게 높지 않은 발생률을 보였지만 통계적으로 유의미한 결과를 나타내지 못하였기 때문에 양호한 안전성을 보였다고 보기 어려웠다. 본 연구를 통해 SARS-CoV-2 mRNA 백신의 예방 효과는 높은 유효성을 보이며 안전성 측면에서 백신 접종군의 위험도는 위약 접종군과 비교하였을 때 높지 않은 위험률을 나타내는 것을 확인하였다. mRNA 백신은 개발 단계부터 시판하기까지 짧은 시간이 걸렸음에도 불구하고 SARS-CoV-2 mRNA 백신의 효과를 보여줄 수 있었고 mRNA 백신 기술의 혁신적인 변화를 볼 수 있었다. 그러나 빠른 개발과 생산으로 다른 백신들에 비해 임상 결과자료가 충분하지 않아 장기간의 연구가 진행되지 못하였기 때문에 체계적인 유효성 및 안전성 평가를 위해 지속적인 모니터링이 필요하다.

The SARS-CoV-2 virus that emerged in 2019 still has widespread community transmission occurred and prolonged, the SARS-CoV-2 mRNA vaccines have become an important means to prevent transmission and spread. This study aims to evaluate the evidence of the necessity for SARS-CoV-2 mRNA vaccines through the strategic study for efficacy and safety of SARS-CoV-2 mRNA vaccines. 3 databases (Pubmed, Embase, Cochrane Library) were used to search as of 06 June, 2022 and a total of 6 studies were selected in the analysis according to the search strategy and PRISMA flow. A systemic literature review and meta-analysis of the efficacy and safety results of the participants who is vaccinated SARS-CoV-2 vaccines/Placebo without a history of SARS-CoV-2 virus was conducted. Focusing on the SARS-CoV-2 mRNA vaccines, the strategic study was performed with six selected studies on the efficacy and safety of two mRNA vaccines (mRNA-1273, BNT162b2). As a result of, the relative risk for the SARS-CoV-2 virus in the vaccination group compared to the placebo group was 0.05, displaying a high efficacy of 95%. In addition to this, the Geometric Mean Titer (GMT) and Geometric Mean Fold Rise (GMFR) of SARS-CoV-2 neutralizing antibody showed higher values in the vaccinated group than the placebo group. The GMT of the participants who was vaccinated for mRNA-1273 was higher than the participants who was vaccinated for BNT162b2, the GMFR of the participants who received the BNT162b2 appeared higher than the participants who received the mRNA-1273. In terms of safety, the relative risk in the vaccinated group were 1.75 and 2.10 respectively for all reported solicited adverse events after first vaccination and second vaccination showing the incidence rate that was not significantly higher than the placebo group. Among solicited adverse events, the relative risk after first vaccination and second vaccination for reported local adverse events were 3.96 and 4.95 respectively showing a slightly higher incidence rate compared to the placebo group, and for systemic adverse events, the relative risk after first vaccination and second vaccination was 1.28 and 2.11 showing the incidence rate that was not significantly higher than the placebo group. For unsolicited adverse events and serious adverse events, the relative risk was 1.09 and 1.08 respectively showing the incidence rate that was not significantly higher than the placebo group. However, it was difficult to say that good safety was displayed because the statistically significant results were not shown. In conclusion, this study demonstrated that SARS-CoV-2 mRNA vaccines showed high efficacy and risk of vaccination group was not high compared to the placebo group. Since there are not enough long-term studies for SARS-CoV-2 mRNA vaccines, consequently continuous monitoring is necessary for the comprehensive evaluation of the efficacy and safety.
