This study was designed to construct a structural model for health promoting behavior of patients with chronic disease. The hypothetical model was developed based on the literature review and Pender's health promotion model. Method: Data was collected by questionnaires from 1748 patients with chronic disease in General Hospital from December 1999 to July 2000 in Seoul. The disease of subject were cardiac disease included hypertension peptic ulcer, pulmonary disease included COPD and asthma, DM, and chronic kidney disease. Data analysis was done with SAS 6.12 for descriptive statistics and PC-LISREL 8.13 Program for Covariance structural analysis. Results: 1. The fit of the hypothetical model to the data was moderate, it was modified by excluding 4 path and including free parameters to it. The modified model with path showed a good fitness to the empirical data (χ2=591.83, p<.0001, GFI=0.97, AGFI= 0.94, NNFI=0.95, RMSR=0.01, RMSEA=0.05). 2. The perceived benefits, perceived barriers, self-efficacy, self- esteem, and the plan for action were found to have significant direct effect on health promoting behavior of chronic disease. 3. The health concept, health perception, emotional state, social support were found to have indirect effects on health promoting behavior of chronic disease. Conclusion: The derived model in this study is considered appropriate in explaining and predicting health promoting behavior of patients with chronic disease. Therefore, it can effectively be used as a reference model for further studies and suggested implication in nursing practice.

따라서 본 연구에서는 만성질환 중 유병율이 높은 위십이지장 궤양을 포함한 소화기 질환, 고혈압 및 심장질환을 포함한 순환기 질환, 천식 및 COPD를 포함한 호흡기 질환, 당뇨병, 신장질환 및 류마치스성 관절염 환자를 중심으로 만성 질환자의 건강증진 행위를 예측하는 구조 모형을 검증, 제시함으로서 만성 질환자의 건강증진 프로그램을 개발하기 위한 기초 자료를 제공하고자 하였다.
본 연구는 만성 질환자의 건강증진 행위를 예측하는 구조 모형을 검증하는 횡단적 구조 모형 검증 연구이다.
제안 방법
1999년 12월부터 2000년 7월까지 만성 질환자를 대상으로 하여 연구에의 참여를 동의한 대상에게 본 연구를 위해 훈련받은 자료 수집 조원이 직접 설문 조사하거나 대상자가 원하는 경우 대상자가 설문에 직접 답하게 하였다. 설문 조사 방법 및 절차의 일관성을 도모하기 위하여 만성질환자를 대상으로 예비조사를 거쳐 정규자료수집 프로토클을 마련하였고, 연구 보조원 6명을 대상으로 자료수집 시작 전 교육을 통하여 설문 조사 방법 및 절차의 시범을 보였으며, 연구 책임자가 직접 환자를 대상으로 자료수집 절차의 시범을 보여 연구 도구와 자료 수집 절차와 관련된 타당도의 문제를 최소화하였다.
본 연구에서는 Pender 의 개정된 건강 증진 모형과 기존 연구를 기초로 만성질환자 개인적 특성과 경험과 관련된 변수로는 흠연, 음주, 운동 등 이전의 일상 생활습관 인구학적, 사회 문화적 특성, 건강 개념, 건강 지각, 자아존중감을 호함하였고, 행위와 관련된 인지와 감정은 행동의 지각된 이익, 행동의 지각된 장애, 지각된 자기효능감, 행동과 관련된 감정, 사회적 지지를 포함하였다. 행위의 결과로는 즉각적인 갈등적 요구와 건강증진 행위에 대한 선호성, 행동 계획 수림을 거쳐 건강증진 행위에 이르는 것으로 제시하였다<Figure 1>.
대상 데이터
대상자 선정 기준은 만성질환으로 진단 받고 치료한지 3개월 이상된 국문 해독이 가능한 20세 이상의 성인으로 하였으며, 질병 유형은 우리나라 만성질환 관리의 주요 질환인 고혈압 및 심장 질환, 위·십이지장 궤양, 당뇨병, 심장병, COPD 및 천식, 간 질환, 신장병, 류마티스성 질환으로 진단받은 환자를 편의 표집하였다.
본 연구의 대상자는 서울시 일 대학 부속 의료원 산하 3개 병원에 입원 증이거나 내과 외래에 방문하여 치료 증인 환자 1800명을 대상으로 설문조사 하였으나 잘못된 설문지와 누락된 응답이 있는 경우를 제의하여 총 1748명을 대상으로 하였다. 대상자 선정 기준은 만성질환으로 진단 받고 치료한지 3개월 이상된 국문 해독이 가능한 20세 이상의 성인으로 하였으며, 질병 유형은 우리나라 만성질환 관리의 주요 질환인 고혈압 및 심장 질환, 위·십이지장 궤양, 당뇨병, 심장병, COPD 및 천식, 간 질환, 신장병, 류마티스성 질환으로 진단받은 환자를 편의 표집하였다.
데이터처리
1) 만성질환자의 인구학적, 일반적 특성 및 각 변수의 정도는 기술 통계를 산츨하였다.
2) LISREL 입력 자료는 공분산 행렬을 이용하며, 가설적 모형에 대한 공변량 자료 분석은 LISREL 8.13 프로그램을 이용하여 분석하였다.
설문지를 이용하여 수집된 자료는 SAS 프로그램과 LISREL프로그램을 통해 분석하였다.
성능/효과
수정 모형에서 건강 증진 행위에 대해 이익성, 자기효능감, 자아존중감, 행동 계획에서 유의한 양의 직접효과를, 장애성에서는 유의한 음의 효과를 나타내었으며 건강 증진 행위가 이들 변수에 의해 설명되는 정도는 50%였다 행동 계획에 대해서는 이익성, 자기효능감, 자아존중감, 감정 상태, 사회적 지지에서 유의한 양의 효과를, 건강 개념, 장애성은 유의한 음의 효과를 나타내었으며 이들 변수가 행동 계획을 22% 설명하는 것으로 나타났다. 선호성에 대해서는 이익성이 유의한 양의 효과를, 장애성이 유의한 음의 효과를 나타내었으며 이들 변수가 선호성을 66% 설명하는 것으로 나타났다.
