In this study, we investigated about the patients visiting emergency room during recent 5 years(1999-2003), and compared with retrospective study during former 5 years(1992-1996) in Dept. OMS Pusan Univ-Hospital. We are assessed the cause, type, demographic ect. And analyzed alterations of facial bone fractures during past 2 periods. the results were as follows. The total number of patients was increased slightly. 429 patients treated for maxillofacial fractures between January 1992 and December 1996 and 466 patients treated between January 1992 and December 1996. The male-to-female ratio reduced in the second period by a factor of 0.7. Patients in the age groups of 10-19 years and 20-29 years increased by a factor of 4.2 and 7.9 in the second period. Assaults and falls in the second period decreased by a factor of 15.4 and 7.9. But, traffic accidents and slip downs in the second period increased by a factor of 6 and 6.6. The mandibular fractures(70.6%) showed the highest incidence, followed by zygomatic bone and arch fractures(7.5%), maxillary bone fractures(4.0%), and nasal bone fractures(4.0%). In the second period, the mid-face fracture was increased slightly. So, the mandibular fractures(69.0%) followed by maxillary bone fractures(12.9%), zygomatic bone and arch fractures(8.0%), nasal bone fractures(7.0%). Ramus fractures and body fractures of mandible was increased slightly in the second period.

제안 방법

국내에 보고된 결과는 김 등14)은 하악골, 비골, 관골 순으로, 고 등21) 은 비골, 하악골, 관골 순으로 서6)는 하악골, 비골 상악골순으로 심 등19)은 하악골 비골 상악골 순으로 보고 하였다. 본 연구에서는 1992년에서 1996년까지의 기간에서는 하악골, 관골 및 관골궁, 비골 상악골 순으로 나타났지만 1999년에서 2003년까지 기간에서는 하악골, 상악골, 관골 및 관골궁, 비골의 순으로 관찰 되었다. 이러한 차이는 타 지역에 위치한 병원의 경우 현 연구가 시행된 병원과 지역적 및 사회적 여건의 차이에 기인한 것으로 보여 지고 또한 외래 환자 및 인접 타과 환자는 제외된 연구이기에 차이가 있는 것으로 생각된다.

대상 데이터
1992년 1월부터 1996년 12월까지 만 5년간과 1999년 1월부터 2003년 12월까지 만 5년간에 부산대학교병원 응급실을 내원한 환자 중 안면골 골절이 있는 환자로써 구강악안면외과에서 입원 및 치료를 실시한 악안면 골절 환자를 대상으로 하였다.

성능/효과
은 과두부위를 보고한 바 있다. 본 연구에서 하악골의 골절 부위별 빈도는큰 차이를 볼 수는 없었지만 1992년에서 1996년까지의 기간보다 1999년에서 2003년 사이 기간에서 하악지와 하악골체부에서 다소 높아진 빈도를 볼 수 있었다. 비교적 발생빈도가 낮은 것으로 보고된 하악지 부위와 하악 골체부에서의 골절은 보다 강력한 외력이 작용 했을 것으로 생각되어이 결과도 교통사고의 증가와 떼어놓을 수 없을 것으로 보인다.
조사된 두 기간에서 모두 하악 골절이 가장 빈번한 것으로 관찰되었다. 전체 환자의 70%가량을 차지하였다. 그러나 상악을 비롯한 중안모 골절에서는 차이가 관찰되었다.
