B형 간염환자의 질병관리가 효과적으로 이루어지기 위해서 필요한 것은? 만성 B형 간염환자의 질병관리가 효과적으로 이루어지기 위해서는 이들의 건강행위의 근본 바탕이 되는 사회문화적 맥락과 의료소비자의 관점에 대한 이해가 필수적이다. 특히 한국계 미국인의 73%가 한국출생임을 고려할 때 이들의 건강과 질병에 대한 연구는 미국은 물론 한국의 사회문화적 맥락을 고려하지 않을 수 없다.
미주한인환자들이 특별한 자각증상이 없는 한 병원방문이 쉽지 않은 이유는? 참여자들의 의료보험 가입현황을 살펴보면 65세 이상 노인을 위한 메디케어(medicare)의 수혜자 2명을 제외한 10명 중 4명이 의료보험이 없고 연수입이 평균 3-5만불 정도였다. 소자본으로 자영업을 하는 대부분의 미주한인들의 경우 가족에 따라 비용차이는 있으나 1년에 대략 만불 내외의 보험비를 지불하기란 쉽지 않다. 미국에서 의료보험이 있다하더라도 의료시스템 이용 시 본인부담액이 적지 않은 상황에서, 의료보험이 없이 건강관리체계를 이용하려면 고비용의 의료비지불을 감당해야만 한다. 따라서 의료보험이 없는 미주한인환자들의 경우 특별한 자각증상이 없는 한 병원방문이 쉽지 않으며 어느 정도의 증상조차 감수하며 살아가고 있는 실정이다.
