[논문]특발성 안면마비를 지닌 개에서 봉독 약침을 이용한 치료 1예

6년령의 중성화한 수컷 시츄견이 좌측 안면마비를 주증으로 내원하였다. 환자는 신경검사에서 좌측 안면의 신경반사 소실 및 MRI 검사에서 두개강내 지주막낭종이 확인되었다. 환자는 안면신경마비를 유발할 수 있는 다른 기저질환을 감별하고, 또 스테로이드와 이뇨제를 이용한 치료에 대한 반응은 미약하였기에 특발성 안면신경마비로 잠정진단 하였다. 봉독 약침을 이용한 침술치료를 진행 후, 환축의 임상증상은 단계적으로 호전되었으며 치료 8주 후에는 신경반응이 거의 정상적으로 회복되었다. 본 증례는 특발성 안면마비 환축에서 봉독 약침을 이용한 치료 증례 보고이다.

Neurological examination revealed absence of facial sensation, menance response, and palpebral reflex on the left side. On magnetic resonance imaging (MRI), intracranial intra-arachnoid cyst (IIAC) was noted. The dog was poor response to steroid and dieuretic therapy. Based on characteristic historical and clinical findings, and excluding of other causes of acute facial nerve dysfunction, the dog was tentatively diagnosed as idiopathic facial paralysis. The clinical signs were improved gradually after acupuncture therapy with bee venom. Eight weeks after initial acupuncture with bee venom, the patient recovered sensory and neurological facial signs. This case report demonstrates that bee venom acupuncture for an idiopathic facial paralysis could be useful in a dog.

To evaluate the osseous structures of the inner ear and brain, magnetic resonance (MR) scan was obtained using a 0.2 Tesla magnet unit (E-Scan, ESAOT, Italy). On magnetic resonance imaging (MRI), a quadrigeminal intracranial intra-arachnoid cyst (IIAC) was observed (Fig 1), however the otitis media or interna were not detected, and the tympanic bulla was normal.

However, some dogs might have anaphylaxis against bee venom, and be shocked from injections (8). In conclusion, it is demonstrated that the determination of acupoints, as well as amount and concentration of bee venom should be carefully considered. In the future more studies on the standardization of measurements are required to treat idiopathic facial paralysis in dogs.

The serum biochemical profile revealed that mild azotemia (blood urea nitrogen (BUN), 49.9 mg/dl; reference range, 9.2 to 29.2 mg/dl), increased serum alkaline phosphatase activity (ALP, 668 U/liter; reference range, 47 to 254 U/liter), mild hyperproteinemia (total protein 7.3 g/dl; reference range, 5.0 to 7.2 g/dl), hyperalbuminemia (4.5 g/dl; reference range, 2.6 to 4.0 g/dl), and severe hypokalemia (2.3 mmol/L, reference range 3.8 to 5.0 mmol/L). These imbalances of elecrolyte and dehydration might be induced by premedication with methyl prednisolone (MPDS, 1 mg/kg, SID, per oral (PO)), acetazolamide, furosemide and mannitol at a local veterinary hospital.
