Results : United States has a two-year associate degree(AS) and a four-year bachelor degree(BS). Canada has two to three years degree and a two-year course is accepted in england and Australia. In the meanwhile, Korea and Japan have two-year and four-year educational courses. The duty of dental hygienists includes the prevention education for dental health and continuing dental health care. Most of the dental hygienists in Korea and Japan play the assistant roles for the dental surgeons. United States has national board examinations including written examinations, practical examinations, and computer assisted examination. Written and practical examinations are also conducted in Korea. England and Australia have the recognized educational organizations for qualification. Conclusions : Problem based and problem solving skills are the most important in dental hygienist education in Korea. The training of highly competent dental hygienists must be done for the improvement of dental hygiene in Korea in the near future.

제안 방법
미국치과위생사 면허시험은 필기시험과 실기시험으로 나누어져 있으며, 시험방법은 미국 전체 주가 인정하는 필기시험으로 약 350개의 4-5지 선다형의 문항으로 구성되어 있고, 문항형식은 치과위생과 학과목 내용에 근거한 기초치과위생과정, 포괄적인 치과위생과정, 지역사회 치과위생 업무영역으로 구성된 문항과 치과환자의 12-15개의 증례보고를 통한 문항의 2가지로 구성하여 실시하고 있으며 시험시행은 1년에 봄, 여름, 겨울에 3회 실시하고 있다. 실기시험(Clinical examination)은 필기시험인 National Board Dental Hygiene Examination에 합격한 자로 반드시 CPR(심폐소생술)인증서를 제출해야 한다.
본 연구에서는 우리나라 치과위생사의 교육제도, 개인 인증제도 및 업무범위를 비교분석하기 위하여 자료접근이 비교적 용이하며 선진국에 해당하는 미국, 캐나다, 영국, 호주 그리고 가까운 일본 5개국을 임으로 선정하였으며, 조사항목으로는 직업의 정의, 주요 업무, 유사직업, 교육제도, 직무내용, 개인인증제도, 합격기준, 문항관리, 인증취득현황, 업무범위 등을 포함하였다.
캐나다 치과위생사의 자격기준은 각 주마다 약간씩 다르지만 반드시 캐나다치과위생사 자격인증위원회(NDHCB)에서 주관하여 시행하는 캐나다 치과위생사면허시험(NDHCB)인 필기시험에 합격해야 한다. 캐나다 치과위생사 면허시험위원회(NDHCE; National Dental Hygiene Certification Board)에서 문항개발과 시험시행을 담당하고 있으며, 미국과 마찬가지로 필기시험과 실기시험으로 구성되며 필기시험은 미국과 거의 동일한 형식으로 총 225-250개의 문항으로 구성되어 있고, 실기시험은 OSCE 방식으로 환자에 대한 자료를 제시하고 답을 구하는 방식으로 오전과 오후에 나누어서 실시된다. 시험시행은 1년에 3월과 9월에 2회 실시되며 합격기준은 각 주 마다 다르다.
대상 데이터
자료 수집은 선행연구, 각종 통계자료, 정부간행물 등의 참고문헌과 American Association of Dental Examiners(AADE), National Dental Hygiene Certification Board(NDCB), ADHA (Australian Dental Hygienist Association) 등 해당기관의 web site를 통한 인터넷 검색을 활용하였으며, 보다 구체적인 내용은 관련자와의 e-mail을 통해 수집하였다.
성능/효과
우리나라도 치과위생사의 발전을 위해서 현행 국가시험의 질과 국가시험제도 개선을 위해 직무기술서와 직무요건서 개발 및 국가시험 교과목의 타당성과 개인 업무 수행능력에 초점을 맞춘 문제해결방식의 포괄치과위생능력을 평가하는 실기시험 개선방안이 개발되어야 할 것이다3). 또한 영국이나 호주처럼 대학인증평가를 통한 치과위생사 교육프로그램의 표준화를 통해 교육의 질적 저하를 막고 수준 높은 치과위생사를 양성하기 위한 장치로 대학교육인증제도 또한 우리나라에서 고려해볼 만한 것으로 사료되었다.
선진국과 우리나라의 치과위생사 교육제도, 자격인증제도 및 업무범위를 통해 개선할 점과 보완할 점이 있는지를 알아 보기 위해 미국, 캐나다, 영국, 호주, 일본 등 5개국의 교육제도, 자격인증제도 및 업무범위를 조사하고 비교 분석한 결과, 각 나라별 교육제도나 개인자격인증제도, 그리고 업무범위에 상당한 차이가 있었다. 교육제도와 주요업무는 미국의 경우 준학사과정(AS)과 학사과정(BS)으로 2년제 전문대학 과정과 4년의 학사학위 과정으로 이루어져 있었고, 캐나다는 교육인 준위원회(CDA)에서 인정한 대학 및 전문대학에서 최소한 2-3 년의 이수과정으로 이루어져 있었으며, 영국과 호주의 경우는 2년 과정으로 4개국의 치과위생사 주요업무로는 예방업무, 구강보건교육 및 계속관리업무를 담당하고 있었다.

각국의 시험제도는 크게 두 가지로 나눌 수 있다. 첫째, 미국, 캐나다, 한국, 일본의 치위생학을 전공한 대학 또는 전문대학을 졸업한 자에게 주어지는 국가시험제도와 둘째, 영국, 호주의 치과위생사의 자격인증취득을 위해 철저한 교육 과정 및 임상실습과정을 이수한 후에 졸업시험에 합격한 사람을 인증하고 있는 인증제도이다.
후속연구
본 연구의 제한점으로는 일부 선진국들과 우리나라의 현실적인 상황이 다르기 때문에 교육제도, 자격인증제도 및 업무 범위를 비교하여 우리나라에 적용시키기는 무리가 있을 수 있다. 그럼에도 불구하고 치과위생사들의 질적 수준을 높이기 위해서는 우리나라의 국가시험능력 평가가 변별력을 갖지 못하고 시간적, 경제적 낭비를 초래하고 있는지와 치과위생사가 환자를 대상으로 문제해결방식의 포괄치과위생능력 평가시험을 보고 있는지 등을 파악하여 추후 연구에서는 전반적인 치과위생사 인증제도에 대한 재정비가 필요한 것으로 사료된다.

그러나 내용면에서 치과위생사의 업무가 예방지향적 포괄구강건강관리 부분이 부각되지 않고 아직까지는 진료협조, 즉 진료보조라는 특수한 형태로 자리 매김된 형태에서 향후 치과위생사라는 직업탄생의 목적인 예방 및 교육의 전문직으로 성장되지 않으면 그 미래를 긍정적으로만 해석할 수 없다. 이에 우리나라의 치과위생사도 향후 업무는 물론 사회적, 경제적 레벨 업(level-up)을 하기 위해서는 치과 위생사 업무의 전문화, 치과위생교육의 전문화, 치과위생연구의 전문화, 치과위생지도력에 있어서의 완전한 전문화가 이루어져야만 치과위생사로서의 정체성을 확고히 할 수 있고 이러한 노력이 자율성의 확립이라는 높은 가치로 자리매김할 수 있도록 해야 할 것이다9) .
