뇌성마비 아동의 균형 자세를 위해 체간과 골반의 대칭성을 개선하는게 중요한 이유는 무엇인가? | 안정적인 자세를 유지하기 위해서는 기능적인 활동 동안, 감각 시스템과 중추신경계, 근골격계 사이의 복잡한 상호작용이 필요하다.1,4,7 뇌성마비 아동은 체간의 근 긴장도가 낮아 안정적인 자세조절과 운동이 어려워지고 특정방향으로의 운동성이 떨어져 균형능력이 부족해진다.8,9 또한 고관절 굴곡근의 과긴장으로 길항근인 복부근과 대둔근에서 저긴장이 나타나고, 장요근의 단축은 전방경사를 유발한다.10 그 결과 아동들이 대칭성의 결여와 정중위로 모아지는 경험의 부족으로 신체분절에서 균형감각을 보상하는 움직임으로 나타난다.11 따라서 아동의 가능한 정상적인 균형 자세를 위하여 아동의 근위부 즉 체간과 골반의 대칭성을 개선하는데 최우선 순위로 두어야 할 것이다. |
