본 연구는 간호대학생의 자아존중감, 공감능력 및 의사소통능력 간의 관계를 파악하고 임상수행능력에 영향을 미치는 요인을 파악하고자 시도되었다. The purpose of this study was to investigate self-esteem, empathy, communication skill and clinical competency, and to identify the influencing factors on clinical competency of nursing students. The data was collected using questionnaires from 212 nursing students who were 3rd and 4th years in 2 nursing universities in Daejeon, September 2014. Data was analyzed using Pearson correlation and stepwise multiple regression with IBM SPSS 21. Significant relationships were found among self-esteem, empathy, communication skill and clinical competency in nursing students. The result of stepwise multiple regression indicates that communication skill and self-esteem (F=37.010, p<.001) predicts 26.2% in clinical competency. The most powerful predictor was communication skill(${\beta}$=.384, p<.001), next one was self-esteem(${\beta}$=.283, p<.001). In conclusion, to enhance clinical competency for nursing students, it is necessary to develop and utilize a program that enhances communication skill and self-esteem.

이론/모형

공감이란 다른 사람의 내적 경험인 주관적 감정이나 심리상태를 ‘마치 나의 것처럼’ 이해하고 느끼며 의사소통 하는 것을 포괄하는 복합적인 개념이다[12]. 본 연구에서 공감능력은 Davis [28]가 제작하고 Jeon [29]이 대학원 학생들을 대상으로 타당성 검토를 마친 도구를 사용하였다. 총 30문항으로 긍정적 24개 문항과 부정적 6개 문항으로 구성되어 Likert 5점 척도로 측정하였다.
의사소통능력은 의사소통과정에서 자신의 생각과 감정을 정확히 전달하는 능력으로[6], 본 연구에서는 Lee 등[31]이 대학생용으로 개발한 의사소통능력 진단 도구로 해석능력, 역할수행능력, 자기제시능력, 목표설정능력, 메시지 전환능력의 총 5개 능력요소로 구성되고, 이는 다시 7개 하위요소, 해석능력-정보수집, 경청, 역할수행능력-고정 관념적 사고극복, 창의적/개방적 의사소통,자기제시 능력-자기 드러내기, 목표설정 능력-주도적 의사소통, 메시지 전환능력-타인관점 이해로 각각 7문항씩 총 49문항으로 구성되어 있다. 이 중 13개 문항은 응답의 편중을 감소시키기 위해 부정문항으로 이루어져 있다.
자아존중감이란 개인이 스스로를 능력이 있고, 중요하며, 성공적이고, 가치 있다고 믿는 정도와 이를 어느 정도 인정하느냐 하는 태도를 말한다[8]. 자아존중감을 측정하기 위한 도구는 Rosenberg[26]가 개발하고 Jon[27]이 번안한 것을 사용하였다. 이 도구는 자기에 대한 긍정적 태도 5문항, 부정적 태도 5문항, 총 10문항으로 이루어져 있다.

성능/효과

따라서 의사소통능력과 관련하여 이를 향상시키거나 계발하는 교육과정을 포함한 정규 교과목으로 편성할 필요가 있으며, 구체적 평가 방법과 피드백 등 어떻게 의사소통능력을 향상시켜나갈 수 있는 지에 대한 검토가 필요해 보인다. 본 연구에서 공감능력은 임상수행능력에 영향을 미치지 않은 것으로 나타났다. 최근 공감능력은 의사소통능력에 영향을 미치고 대인관계를 개선시키는데 주요한 요인으로서 공감을 잘할수록 대인관계를 잘하고 사회적 상호작용을 촉진하는 주요한 요인으로 긍정적 대인관계를 형성한다는 Chung[14]의 연구의 맥락에서 볼 때 임상수행능력에서도 주요한 긍정적 영향을 미칠 것으로 예상하였으나 유의하게 나타나지는 않았다.
이와 유사한 개념으로 본 연구에서는 공감능력을 사용하였는데, 이는 내적 경험인 주관적 감정이나 심리상태를 마치 나의 것처럼 이해하고 느끼며 의사소통하는 것으로 본 연구결과 임상수행능력에 영향을 미치는 유의한 변인으로 나타나지는 않아 반복 연구가 필요하다고 판단된다. 본연구에서 간호대학생의 임상수행능력에 영향을 미치는 두 번째 요인으로는 자아존중감으로 나타났다. 이는 자아존중감이 임상수행능력에 영향을 미치는 요인으로 나타난 Seo [39]의 연구와 유사한 결과이다.
