Purpose: The purpose of this study was to examine the relationship among family support, self-care and quality of life(QOL) in Patients with hepatocellular carcinoma receiving transarterial chemoembolization, including the effects of these variables on QOL. Methods: The research was a cross-sectional, descriptive design. Participants were 103 patients with hepatocellular carcinoma who were receiving transarterial chemoembolization. Data were analyzed using descriptive statistics, t-test, one way ANOVA, and hierachial multiple regression analysis with SPSS 18.0 program. Results: QOL had a significant correlation with family support(r=.60, p<.001) and self-care(r=.38, p<.001). Family support had a significant correlation with self-care(r=.41, p<.001). Hierarchial multiple regression analysis for QOL revealed that the most powerful predictor was family support followed by self-care. Family support, self-care, gender, and occupation explained 50% of the variance in quality of life. Conclusion: These results indicate that various factors are related to the quality of life of these patients. Further, nursing strategies to improve the family support and self-care of patients with hepatocellular carcinoma are needed.

대상 데이터

본 연구의 대상자는 B광역시 소재 I 대학병원 소화기내과에서 간세포암으로 간동맥 화학색전술을 1회 이상 받은 경험이 있는 환자이다. 적절한 대상자수를 산출하기 위해 검정력 분석 프로그램인 G*Power 3.
또한 대상자가 원치 않을 경우 언제든지 연구 참여를 철회할 수 있도록 하였다. 설문지는 110부를 배포하였고 응답이 불완전한 7부를 제외하고, 총 103부를 본 연구 자료로 활용하였다.

데이터처리

● 대상자의 가족지지, 자가간호 및 삶의 질 간의 상관관계는Pearson’s Correlation Coefficient로 분석하였다.
● 대상자의 삶의 질에 미치는 영향요인을 파악하기 위하여 위계적 다중회귀분석(Hierachial multiple regression analysis) 을 하였다.
● 대상자의 인구사회학적 특성과 질병 관련 특성은 기술통계를 이용하여 실수와 백분율, 평균과 표준편차로 분석하였으며, 이에 따른 삶의 질의 차이 정도는 t-test와 NOVA로 분석하였다.

이론/모형

Kang [13]이 개발한 가족지지 측정도구를 사용하였다. 긍정 형과 부정형의 11문항으로 구성된 5점 평정척도로 최소 11점에서 최대 55점으로 점수가 높을수록 가족지지 정도가 높은 것을 의미한다.
Kim [22]이 개발하고 Park [23]이 수정 ․ 보완한 총 20개의 문항으로 구성된 자가간호 측정도구를 사용하였다. 5개의 하위영역인 식이요법 10문항, 안정 및 활동 3문항, 합병증 예방 및 관찰 3문항, 추후 병원방문 2문항, 약물 요법 2문항으로 구성된 5점 평정척도로 최소 20점에서 최대 100점으로 점수가 높을수록 자가간호 정도가 높음을 의미한다.
국내 암환자를 대상으로 타당성이 입증된 한국 암 특이형 삶의 질 측정도구[24]를 사용하였다. 총 21문항으로 5개의 하위영역인 신체상태 6문항, 정서상태 6문항, 사회상태 3문항, 걱정상태 2문항 및 대처기능 4문항으로 구성된 5점 평정척도로 최소 0점에서 최대 84점으로 점수가 높을수록 삶의 질이 높음을 의미한다.

성능/효과

연구대상자의 가족지지, 자가간호 및 삶의 질은 각각 유의한 양의 상관관계가 있었으며, 가족지지, 자가간호 및 인구사 회학적 특성 중 성별과 직업유무가 삶의 질에 영향을 미치는 요인으로 나타났다.

후속연구

둘째, 본 연구의 대상자인 간세포암 환자를 포함한 만성 간질환 환자의 주돌봄 가족구성원에 따른 지지 정도를 측정할 수 있는 도구개발이 요구된다.
셋째, 간동맥 화학색전술을 받은 간세포암 환자의 가족지지와 자가간호를 향상시키기 위한 자조모임 구성과 신체적, 심리적, 사회적 측면에서 접근할 수 있는 개별화된 간호중재 및 교육 프로그램 제공을 할 필요가 있다.
첫째, 연구 기간의 연장과 더 많은 대상자 수를 확보하여 시술 직후, 시술 후 6개월, 1년 후까지 장기간에 걸쳐 가족지, 자가간호 및 삶의 질을 측정하여 변화 정도를 확인할 수 있는 추후 연구가 필요하다.

	간세포암 치료 방법은?	이러한 간세포암의 치료에서 가장 우선적으로 고려되는 최적의 치료법은 간이식과 간절제술 같은 수술적 요법이지만 이는 간경화가 없는 일부의 환자에게 국한된 치료법으로 대부분의 환자들은 진행성 간세포암의 형태로 발견되기 때문에, 암세포의 침범범위가 넓고 간기능이 저하되어 있는 경우가 많아 수술적 치료는 적용이 불가능하다[3]. 수술적 요법이나 고주파 열치료와 경피적 에탄올 주입술과 같은 근치적 치료가 불가능한 간세포암 환자에게 고식적 치료법인 간동맥 화학색전술의 적용은 암 진단 후의 장기 생존율을 유의하게 증가시키나 암세포의 재발 위험을 막기 위해서는 주기적인 복부단층 촬영과 필요시에는 반복적인 간동맥 화학색전술 치료가 적용된다[4].
	간동맥 화학색전술을 받은 간세포암 환자에게 가족의 역할은 어떠한 의미를 갖는가?	지지체계로서 가족은 스트레스 혹은 위기 상황에서 가족 구성원이 적응에 실패하게 되는 주요원인인 불안, 우울, 무력감과 같은 부정적인 정서를 경감시켜주는 역할을 하는 가장 중요한 직접적이고 일차적인 사회체계이다[13]. 간세포암을 포함한 만성 간질환 대상자는 앞서 언급한 피로, 허약감 및 복부 통증 등 질병치료과정에서 나타나는 여러 신체적 증상과 불안과 우울 등 정신적 장애로 인해 자신에게 주어진 역할을 제대로 수행하지 못하기 때문에 가족의 역할에 현저한 변화를 초래하게 되고 이로 인한 스트레스도 가중된다[14].
