Purpose: Purpose was to explore associations between sleep duration and metabolic syndrome (MS) risks, and to determine factors associated with self-rated health (SRH) of adults with MS compared to other adults. Methods: This is a secondary data analysis based on the Fifth Korea National Health and Nutrition Examination Survey KNHANES V (N=12662). Study instruments included sleep duration, MS risk factors, SRH and health-related quality of life (HRQoL). Results: Mean age of participants was $43.68{\pm}12.26years$. Fifty-eight percent were women, and 18.3% were identified as having MS. Systolic blood pressure (SBP), diastolic blood pressure (DBP), and SRH were significantly different according to sleep duration (p <.05) among all participants. In the non MS group, male gender, younger age (19~30 and 41~50 age brackets) upper income level, sufficient sleep duration, and high density lipoprotein (HDL) were positively associated with SRH, whereas, lower education levels (${\leq}$ middle school), glucose level, and waist circumference were negatively associated with SRH (p <.05). In the MS group, lower income, lower education levels (${\leq}$ middle school), glucose level, and waist circumference were negatively associated with SRH, whereas, having an occupation was positively associated with SRH (p <.05). Conclusion: Results suggest that tailored approaches are required for prevention and control of MS and sleep duration of each individual should be considered rather than applying standardized guidelines. However, as sleep quality was not included in the analysis, further investigations regarding influence of sleep quality on MS and SRH and controlling for other lifestyle and health behavior factors are required.

따라서, 수면시간과 대사증후군 위험요인 분포 및 주관적 건강상태와의 관련성을 파악하고, 수면시간이 주관적 건강상태에 미치는 영향이 정상집단과 대사증후군집단 간에 차이가 있는지를 비교분석할 필요가 있다. 이에 본 연구에서는 정상집단과 대사증후군집단의 주관적 건강상태를 파악하고, 각 집단의 주관적 건강상태에 영향을 미치는 요인으로써 사회경제적 요인, 수면시간 및 대사증후군 위험요인들을 종합적으로 검토하고자 하였으며, 특히 수면시간에 따른 대사증후군 위험요인 분포를 파악하여, 이를 대사증후군의 예방 및 관리를 위한 중재 전략에 활용하고자 하였다.
제안 방법
수면시간은 하루 평균 수면시간 조사를 이용하였다. 정상 성인에게서 권장되는 수면시간이 7~8시간임을 고려하여[13], 본 연구에서는 6시간미만을 과소, 6시간 이상 9시간미만을 적정, 9시간 이상을 과다로 나누었다.
일반적 특성으로써 성별, 연령, 소득수준, 교육수준, 경제활동 상태를 포함하였다. 연령은 10세 단위로 나누고, 소득수준은 통계청에서 작성하는 월평균가구균등화소득(가구 월 소득/#가구원수)을 성·연령별로 4개 군으로 등분한 소득4분위 수(개인) 자료를 이용하였다.
대상 데이터
참여율은 약 80% 수준이었으며[17], 참여자 중 만 19세 이상 65세 미만의 전체 성인은 14,857명이었다. 이 중 본 연구의 주요변수를 측정하지 않은 대상자를 제외한 12,662(85.2%)명의 자료를 최종 연구 분석에 활용하였으며, 미국의 콜레스테롤 교육 프로그램 3차 관리지침(National Cholesterol Education Program Adult Treatment Panel III [NCEP ATP III]) 및 아시아-태평양비만진료지침에 의거하여 혈압, 공복혈당, 중성지방, HDL(고밀도)-콜레스테롤, 허리둘레 중 3가지 기준에 부합되어 대사증후군으로 진단한 대상자는 이중 2,322명(18.3%)이었다.
데이터처리
둘째, 정상집단과 대사증후군집단의 주관적 건강상태에 미치는 영향 요인을 분석하기 위하여 ordinal logistic regression을 실시하였다.
셋째, 수면시간에 따른 대사증후군 위험요인 및 주관적 건강상태차이는 연령, 성별을 공변수로 하여 공분산분석을 실시하였으며, 사후 검정은 Bonferroni 다중비교법을 이용하였다.

건강 관련 삶의 질에는 주관적 건강상태, Euro QoL, EQ-VAS가포함되었다.
대사증후군을 정의하기 위해 미국의 국립심폐혈액연구소(National Heart, Lung, and Blood Institute [NHLBI])에서 임상적으로 대사증후군을 진단하기 용이하도록 만든 NCEP ATP III 기준 지침을 사용하였으며[18], 복부둘레는 아시아-태평양비만진료지침을 토대로 한국인의 심혈관계 위험도를 반영하여 수정된 지침을 이용하였다[19]. 총 5개 항목 중 3개 항목 이상에 해당될 경우 대사증후군으로 판정하였으며, 각각의 해당 기준은 혈압은 수축기 혈압이 130mmHg 이상 또는 이완기 혈압이 85 mmHg 이상이거나 혈압조절제복용중인 경우, 공복 혈당은 110 mg/dL 이상 혹은 당뇨병 약을 복용중이거나 인슐린주사를 맞는 경우, 중성지방은 150 mg/dL 이상이거나 이상 지혈증 약 복용중인 경우, HDL(고밀도)-콜레스테롤은 남자 40 mg/dL 이하, 여자 50 mg/dL 이하인 경우, 허리둘레는 남자90 cm 이상, 여자 85 cm 이상인 경우이다.
성능/효과
본 연구는 제5기 국민건강영양조사를 바탕으로 정상집단과 대사증후군집단의 일반적 특성, 수면시간, 주관적 건강상태, 건강 관련 삶의 질 차이를 파악하고, 정상집단과 대사증후군집단의 주관적 건강상태에 미치는 영향 요인을 분석하는 한편, 수면시간에 따른 대사증후군 위험요인과 주관적 건강상태의 분포를 파악하기 위해 시도되었다. 대사증후군집단과 정상집단을 비교한 결과 건강 관련 삶의 질변수인 주관적 건강상태, EQ-5D, EQ-VAS 모두 대사증후군집단에서 낮게 나타났으며, 수면시간은 대사증후군집단에서 과소수면이 더 많았다. 두 집단 간 주관적 건강상태에 영향을 미치는 요인을 비교 분석한 결과, 대사증후군집단은 소득수준, 교육수준, 취업유무, 혈당, 고밀도콜레스테롤, 허리둘레가 주관적 건강상태에 영향을 미치는 요인으로 분석되었다.

따라서, 추후 연구에서는 수면의 질을 정확히 평가하기 위해서는 낮 동안 수면, 복용 약물 등을 포함한 다양한 수면의 질 관련 요인들이 함께 평가되어야 할 것으로 여겨진다. 이러한 점에서 본 연구는 국민건강영양조사 자료를 분석한 이차 자료 분석 연구이므로 다양한 수면의 질 관련 변수들을 분석에 포함하지 못하였다는 제한점이 있다. 그러나 본 연구는 대사증후군 발생에 영향을 미치는 성별과 연령을 통제한 상태에서 수면시간에 따른 대사증후군 위험요인의 분포를 분석하였다는 점에서 강점을 지닌다.
마지막으로 본 연구는 몇 가지 제한점을 지닌다. 첫 번째로 본 연구는 제5기 국민건강영양조사를 바탕으로 한 조사연구로써 수면시간은 대상자의 기억에 의존한 변수로써 실제 수면시간과 다를 수 있으며 수면시간 외에 다양한 수면의 질 관련 변수들을 측정할 수 없는 한계를 지닌다. 두 번째로 본 연구에서는 정상집단과 대사증후군집단의 수면시간과 대사증후군 위험요인을 주요 변수로 하여 이들 변수가 두 집단의 주관적 건강상태에 미치는 영향을 알아보고자 하였으므로 건강관련 기타 다른 변수들은 연구에 포함되지 않았다.
