This study aimed to investigate the effects of job satisfaction and self-esteem on psychological burnout in dietitians at a geriatric hospital in Busan. The survey was conducted from July 8 to August 31, 2014, and the data were analyzed using the SPSS program. The age group of over 36 years showed higher job satisfaction than the 'Under 25' group. Subjects that worked less than 2 years at their present jobs showed lower job satisfaction in job-itself than the 'Over 4 years' group did, and those who worked less than 1 year showed lower job satisfaction in terms of communication, appraisal compensation, and co-workers than did the other groups. Those who worked in '100~149 bed' hospitals showed higher job satisfaction than those in the 'Under 100 bed' group did. 'Over 200 bed' hospital dietitians showed higher satisfaction in supervision of superiors than did the other groups. Their experience of psychological burnout turned out to be 2.39 out of 5. Psychological burnout had a negative relationship with job satisfaction and self-esteem. Higher emotional exhaustion correlated with lower satisfaction in job-itself, less communication, and less supervision of superiors. Dehumanization correlated with lower satisfaction in job-itself, lower self-esteem, and worse communication, and while decreased personal achievement correlated with lower self-esteem, worse co-workers, lower satisfaction in job-itself, worse appraisal compensation, and worse communication. These results suggest that it would be effective for prevention and management of psychological burnout to mediate factors of job-itself, communication, supervision of superiors, co-worker, and appraisal compensation of dietitians in geriatric hospitals.

본 연구에서는 요양병원에 근무하는 영양사들의 직무만족도, 자아존중감, 심리적 소진 경험 정도를 파악하고 상관관계를 규명하여 급식 및 영양 서비스 제공과 급식 업무의 효율성을 높일 뿐만 아니라, 요양병원 영양사 개인의 삶의 질 향상을 도모하기 위한 기초자료를 제시하고자 하였다.
제안 방법
부정적 의미를 내포하는 문장은 역환산 처리하였다. 심리적 소진은 Maslach & Jackson(1981)이 개발한 척도를 선행연구(Yun 등 2009; Kim & Lee 2013; Hwang 등 2014; Kang 2014; Song 2014)의 내용을 참고로 하여 본 요양병원 영양사의 심리적 소진에 맞게 수정ㆍ보완하여 사용하였다. 정서적 탈진 9문항, 대상의 비인격화 5문항, 개인적 성취감 감소 6문항으로 구성하였고, ‘전혀 그렇지 않다’ 1점, ‘대체로 그렇지 않다’ 2점, ‘보통이다’ 3점, ‘대체로 그렇다’4점, ‘매우 그렇다’ 5점으로 한 5점 척도를 이용하였다.
‘매우 그렇지 않다’ 1점, ‘대체로 그렇지 않다’ 2점, ‘보통이다’ 3점, ‘대체로 그렇다’ 4점, ‘매우 그렇다’를 5점으로 한 5점 척도를 이용 하였다. 자아존중감은 선행연구(Yun 등 2009; Lee 2013) 의 내용을 기초로 하여 본 연구의 목적에 맞게 수 정ㆍ보완하여 총 10개 문항으로 구성하였다(Cronbach’s α=0.793). ‘전혀 그렇지 않다’ 1점, ‘대체로 그렇지 않다’ 2점, ‘보통이다’ 3점, ‘대체로 그렇다’ 4점, ‘매우 그렇다’를 5점으로 한 5점 척도를 이용하였다.
일반적 사항은 성별, 연령, 근무경력, 현 직장 근무기간, 직위, 결혼여부, 교육수준, 고용형태, 근무시간, 급여 등의 문항으로 구성하였고, 근무 환경은 병원의 병상수, 입원환자수, 영양사의 수, 소속부서, 급식 운영형태, 조리종사원의 수, 치료식 여부, 평균 급식수 등의 문항으로 구성하였다. 직무만족도 측정 시 선행연구(Park 2008; Hong 2009; Kim 등 2012)의 내용을 기초로 하여 본 연구의 목적에 맞게 수정ㆍ보완하여 총 39개 문항으로 구성하였다 (신뢰도계수=0.875). 영역별로는 직무자체 5문항, 의사소통 5문항, 평가와 보상 5문항, 전문성 신장 5문항, 근무환경 9문항, 상사의 감독 5문항, 동료 5문항 등으로 구성하였다.
대상 데이터

성능/효과
조사대상자의 직무만족도, 자아존중감, 심리적 소진의 상관관계를 파악한 결과는 Table 8에 나타내었다. 조사대상자의 직무만족도와 자아존중감은 양의 상관(P＜0.01)을 보였으나 심리적 소진은 직무만족도와 자아존중감과 음의 상관(P＜0.01)을 보여, 자아존중감이 높을수록 직무만족도가 높고, 직무만족도와 자아존중감이 높을수록 심리적 소진이 낮은 것으로 나타났다.
직무자체가 심리적 소진의 세부영역인 정서적 탈진, 대상의 비인격화, 개인적 성취감 감소와 음의 상관을 보여(P＜0.001, P＜0.001, P＜0.01) 본인이 수행해야할 역할과 업무량이 적절하고 직무에 흥미를 느낄수록 정서적 소진을 덜 느끼며 대상에 대한 비인격화나 개인적 성취감 감소가 적어짐을 알 수 있 었다. Song(2014)은 우정사업본부 공무원을 대상으로한 연구에서 역할과다가 정서적 소진에 영향을 미친다는 보고를 하면서 자신이 맡은 역할의 뚜렷한 구분과 업무의 적절한 배분이 이루어질 경우 성취감을 향상시킬 수 있음을 주장하였다.
후속연구
이상의 결과로 볼 때, 심리적 소진에 높은 영향을 미치는 것으로 나타난 직무자체, 의사소통, 상사의 감독, 동료, 평가와 보상 요인에 대하여 우선적으로 중재하는 것이 요양병원 영양사의 직무만족을 효과 적으로 향상시키고, 소진을 예방하고 관리하는 방안이 될 것으로 사료된다. 요양병원 영양사들의 직무 만족 수준을 높이고 심리적 소진을 낮추는 데 효과 적인 맞춤형 프로그램이 개발되고 실시되어야 할것으로 생각된다.
이상의 결과를 볼 때 심리적 소진에 높은 영향을 미치는 것으로 나타난 직무, 의사소통, 상사의 감독, 동료, 평가와 보상 요인에 대하여 우선적으로 중재 하는 것이 요양병원 영양사의 직무만족을 효과적으로 향상시키고, 소진을 예방하고 관리하는 대응방안인 것으로 사료된다. 요양병원 영양사들의 직무만족 수준을 높이고 심리적 소진을 낮추는 데 효과적인 맞춤형 프로그램이 개발되고 실시되어야 할 것으로 생각된다.
자아존중감은 대인관계에서의 일탈과도 부(–)의 관계를 가지므로 높은 자아존중감은 대상의 비인격화를 낮추어줄 수 있을 것이다(Kanov 등 2004). 전문지식과 실무능력 기술의 습득은 전문직 종사자의 보람과 긍지로 필수적이므로 재교육과 훈련과정을 통하여 자아존중감을 높임으로써 스스로 소진을 예방ㆍ관리할 수 있는 자질을 향상시킬 수 있는 지원체계를 확립해야 할 것으로 본다.
