Purpose: The purpose of this study was to identify the factors influencing health-related quality of life (HRQOL) of patients with chronic obstructive pulmonary disease (COPD). Methods: A sample of 108 Patients diagnosed with COPD was recruited from outpatients, at a university hospital. Data were collected from February to April in 2016 using the St. George's Respiratory Questionnaire, the Hospital Anxiety-Depression Scale, and the COPD Self-efficacy scale. Results: The average score of HRQOL was 47.56 in patients with COPD. Results of the regression analysis showed that dyspnea phase (${\beta}$=.34, p<.001) and presence or absence of oxygen (${\beta}$=.20, p=.009), anxiety (${\beta}$=.29, p<.001), $FEV_1$ % predicted (${\beta}$=-.15, p=.014) were statistically significant in patients' HRQOL. Conclusion: The research findings show that dyspnea phase and anxiety among psychological factors were the most powerful factors that explain the HRQOL of COPD patients when the demographic and disease characteristics were controlled. Identification of quality of life-related factors should be used to inform targeted interventions, in order to improve HRQOL and help patients' cope.

이론/모형

건강 관련 삶의 질은 Jones, Quirk와 Baveystock (1991)이 개발한 세인트조지 호흡기설문(St. George's Respiratory Questionnaire, SGRQ)를 Byun (2006)이 번역 · 수정한 한글판을 원저자의 승인을 받은 후 사용하였다. 본 도구는 총 50문항으로 이루어져 있으며, 최근 3개월간의 기침 빈도, 객담, 호흡곤란 등에 관련된 ‘증상’ 영역 8문항, 호흡곤란을 유발하거나 호흡곤란으로 인해 영향 받는 활동을 묻는 ‘활동’ 영역 16문항, 사회적, 정서적 기능 상태와 건강에 대한 기대 등 다양한 측면을 묻는 ‘영향’ 영역 26문항으로 이루어져 있다.
불안과 우울은 Zigmond와 Snaith (1983)가 개발한 도구를 Oh, Min과 Park (1999)이 번안한 병원불안-우울척도(Hospital Anxiety and Depression Scale, HADS)를 원저자의 승인을 받은 후 사용하였다. 불안 하부척도는 총 7개 문항이며, 4점 (0~3점) Likert 척도로 구성되어 있다.
자기효능감은 Wigal., Creer와 Kotses (1991)가 개발한 만성폐쇄성폐질환자를 위한 자기효능감 측정도구를 Jang & Kim (2006)이 번안한 도구를 승인을 받은 후 사용하였다. 본 도구는 총 34문항 5점 Likert 척도로 구성되어 있으며, 점수가 높을수록 자기효능감이 높음을 의미한다.

성능/효과

본 연구는 만성폐쇄성폐질환자 108명을 대상으로 건강 관련 삶의 질 정도와 건강 관련 삶의 질에 영향을 미치는 요인을 파악한 횡단적 조사연구이다. 본 연구대상자의 건강 관련 삶의 질 점수는 평균 47.56점으로 나타났으며, 위계적 회귀분석을 통한 건강 관련 삶의 질에 영향을 미치는 요인을 파악한 결과 mMRC 호흡곤란점수, 산소 유무, 불안, 1초간 노력성 호기량의 예측치가 영향요인으로 제시되었다. 본 연구결과를 기반으로 만성폐쇄성폐질환자의 건강 관련 삶의 질을 증진시키기 위해서는 호흡곤란 증상 및 산소 사용 유무와 같은 질병 중등도에 따른 증상 관리가 요구되며 이와 함께 불안과 같은 심리적 스트레스를 감소시킬 수 있는 중재 프로그램 개발이 요구된다.

후속연구

그러나 국외에 비해 국내에서 우울과 삶의 질의 관련성을 파악한 연구가 아직 소수에 지나지 않기 때문에 우울과 삶의 질의 관련성을 파악하는 연구가 계속적으로 요구된다. 또한 본 연구에 사용된 우울 측정도구는 7문항으로 이루어져 있어 우울의 스크리닝을 단시간에 쉽게 할 수 있는 장점이 있으나 한 개의 문항으로 경증 우울과 중등도 이상의 우울의 진단이 달라질 수 있기 때문에, 만성질환자의 우울을 좀 더 구체적으로 측정할 수 있는 도구를 이용한 추후 연구가 요구된다.
