Purpose: The purpose of this study was to identify factors affecting problem drinking in male nursing students. Methods: This study was conducted using a cross-sectional survey. A total 126male nursing students completed a self-report questionnaire to assess alcohol consumption, motives for drinking, and self-efficacy in drinking refusal self-efficacy. The participants were divided into 2 groups, moderate and problem drinkers. Multivariate analysis was used to identify factors affecting problem drinking in these nursing students. Results: Problem drinkers was found to be related to young age (adjusted OR=0.83, 95% CI [0.71, 0.96], coping motives (adjusted OR=1.19, 95%CI [1.00, 1.42], and drinking refusal self-efficacy (OR=0.96, 95% CI [0.93, 0.99]). Conclusion: The findings highlight the important role that coping motives and drinking refusal self-efficacy play in the problems in the drinking behavior of male nursing students.

데이터처리

대상자의 음주유형에 따른 음주동기와 음주거절효능감의 차이는 t-test로 분석하였다.
대상자의 음주유형에 따른 일반적 특성의 차이는 -test와 t-test로 분석하였다.
문제음주에 영향을 미치는 요인은 다중 로지스틱 회귀분석(multiple logistic regression)으로 분석하였으며, 승산비(odds ratio)와 95% 신뢰구간을 제시하였다.

이론/모형

본 연구에서 음주유형은 문제음주와 정상음주로 구분하였다. 문제음주란 음주로 인한 문제들을 광범위하게 포함하는 개념으로 신체적 문제음주군, 알코올 남용군, 알코올 의존군을 모두 포함하는 개념으로[21] 두 개의 음주유형은 Alcohol Use Disorders Identification Test-Consumption Questions(AUDIT-C)를 사용하여 분류하였다. AUDIT-C는 음주빈도와 평소 음주량, 그리고 한 번의 술자리에서 폭음하는 빈도를 묻는 3개의 문항으로 구성된 도구이다.
음주거절효능감은 음주를 할 기회가 주어졌을 때, 음주를 하도록 권하는 사회적 압력 상황에 있을 때, 정서적 해소를 하고자 하는 개인의 동기가 있을 때 음주에 저항할 수 있는 능력에 대한 개인의 신념을 의미한다[25]. 본 연구에서 음주거절효능감은 Oei, Hasking과 Young [25]이 2005년 성인을 대상으로 개발한 단축형 음주거절효능감(Drinking Refusal Self-Efficacy Questionnaire- Revised, DRSEQ-R) 도구를 국내 대학생을 대상으로 신뢰도와 타당도를 검증한 도구[20]로 측정하였다. 본 도구는 사회적 압력 5문항, 정서적 해소 7문항, 음주기회 상황에서의 음주거절효능감 7문항의 총 19문항으로 구성되어 있으며 각 문항은 ‘전혀 참을 수 없다’ 1점에서 ‘확실히 참을 수 있다’ 6점의 6점 Likert Scale로 측정된다.
음주동기는 사람들이 술을 마시는 이유를 의미하며[10] 본 연구에서는 음주동기를 수량적으로 측정하기 위해 남녀 대학생을 대상으로 Shin과 Han이 1999년 개발한 음주동기 측정도구로 측정하였다[10]. 본 도구는 고양동기, 대처동기, 동조동기, 사교동기의 4요인, 16문항으로 구성되며 각 문항은 음주 이유에 대한 내용을 포함하고 있다.

성능/효과

문제음주에 영향을 미치는 요인을 파악하기 위해 연령과 음주동기 하부요인 음주거절효능감을 투입하여 로지스틱 회귀분석으로 분석한 결과 연령, 대처동기, 음주거절효능감이 문제음주 영향요인으로 나타났다(Table 4). 문제음주 승산비는 연령이 증가할수록 0.83배(95% CI: 0.71~0.96), 대처동기가 1점 상승하면 1.19배(95% CI: 1.00~1.42), 음주거절효능감이 1점 상승하면 0.96배(95% CI: 0.93~0.99) 증가하는 것으로 나타났다(Table 4).
본 연구는 간호학과 남학생의 문제음주 실태를 확인하고 문제음주 관련 요인을 파악하고자 실시된 횡단적, 서술적 조사연구이다. 본 연구 대상자의 57.1%가 문제음주로 확인되었으며 연령이 낮을수록, 대처동기가 높을수록, 음주거절효능감 낮을수록 문제음주 승산비가 높아지는 것으로 나타났다. 따라서 간호학과 남학생의 문제음주 예방을 위해서는 연령이 낮은 학생을 우선 중재 집단으로 선정하고, 그들이 대학생활에서 경험하는 어려움을 파악한 후 부정적 정서를 해결할 수 있는 대처 전략 강화와 함께 음주거절효능감을 증진시킬 수 있는 방안이 모색되어야 할 것이다.

후속연구

1%가 문제음주로 확인되었으며 연령이 낮을수록, 대처동기가 높을수록, 음주거절효능감 낮을수록 문제음주 승산비가 높아지는 것으로 나타났다. 따라서 간호학과 남학생의 문제음주 예방을 위해서는 연령이 낮은 학생을 우선 중재 집단으로 선정하고, 그들이 대학생활에서 경험하는 어려움을 파악한 후 부정적 정서를 해결할 수 있는 대처 전략 강화와 함께 음주거절효능감을 증진시킬 수 있는 방안이 모색되어야 할 것이다.
간호학과 남학생은 남성 유망 직종으로 분류되기도 하지만 여전히 여성 중심의 직종이라는 사회적 이미지가 공존하는 상황에서[30] 간호학을 전공하고 있어 대학생활 중 타 학과와는 다른 어려움을 경험할 수 있다. 향후 간호학과 남학생을 대상으로 대학생활 관련 스트레스를 찾고 문제음주와의 관계에서 대처동기의 매개효과를 파악하는 연구와 간호학과 남학생의 문제음주 실태를 파악하기 위한 반복 연구를 제안하며 다양한 측면에서 문제음주 관련 요인을 확인하고 관련 변수들의 관계를 종합적으로 파악하는 후속 연구를 제안한다.
