Purpose: The purpose of this study was to identify the effects of nurse's incivility experienced by nursing students and coping against incivility on burnout in clinical practice. Methods: A cross-sectional correlation study design was used. The subjects were 120 nursing student from four universities in Busan and Yangsan, South Korea. A self-report questionnaire was used to collect data on incivility, coping, and burnout. Finally total 117 nursing students' data was analyzed except 3 nursing students who had never experienced nurse's incivility for clinical practice. Data analyzed using descriptive statistics, Pearson correlation coefficient, independent t-test, ANOVA, and multiple regression. Results: 97.5% of subjects (n=120) experienced incivility. Incivility was positively correlated with seeking social support coping, avoiding focused coping and burnout. Incivility and avoiding focused coping had a significant positive effect on burnout. The explained variance for burnout was 10.0% and avoiding focused coping was the most significant factor in burnout. Conclusion: Most of nursing students experience the nurses' incivility. Judging incivility as a difficult problem to solve, students use more avoiding focused coping strategy and burnout is increased. Therefore education is needed to improve the coping strategies at incivility. Additionally colleges and hospitals should establish the formal reporting system to handle the incivility.

이론/모형

본 연구에서는 대처를 간호대학생이 간호사로부터 경험한 무례함으로 초래된 스트레스를 다루기 위해 사용하는 전략으로 보고 이를 측정하기 위해 개발되고[23] 한국어판으로 번안된[24] 한국형 스트레스 대처전략 검사지(Korean Coping Strategy Indicator)를 사용하였다. 지난 6개월 동안 발생했던 스트레스 사건 중 한 가지를 회상하면서 당시 실제로 시행했던 대처 반응을 사용한 정도를 측정하며 하위영역인 사회적 지지추구 대처, 문제해결중심 대처, 회피중심 대처, 각 11문항씩 총 33문항으로 구성되었다.
본 연구에서는 무례함을 임상실습 중에 간호대학생이 경험 하는 상호 존중해야 하는 환경에서 규범에 위반되고 대상을 해치려는 의도가 모호한 낮은 강도의 일탈된 행동으로 보고 이를 측정하기 위해 개발된 The Uncivil Behavior in Clinical Nursing Education (UBCNE)도구[7]를 연구자가 진행한 번역-역번역 과정을 거쳐 한국어판으로 번안한 척도를 사용하였다. H-M/Dismissive (적대적인/비열한/무시하는)와 EXBEV (배제하는)의 2가지 영역으로 구성되었으며 총 12문항이다.
본 연구에서는 소진을 간호대학생이 임상실습 중에 경험하는 정서적 탈진, 냉담, 무능감으로 보고, 이를 측정하기 위해 대학생을 대상으로 개발된 학업소진척도(MBI-SS)[25]를 한국어판으로 번안한 척도[26]를 사용하였다. 하위영역으로는 정서적 탈진(5문항), 냉담(4문항), 무능감(6문항)으로 구성되었으며 무능감 6문항은 역문항이다.

성능/효과

본 연구결과를 통해 임상실습에서 간호대학생은 무례함을 대다수가 경험하고 있는 것을 알 수 있었으며, 이러한 무례함의 경험은 문제해결이 어려운 것으로 판단되어 적극적 대처보다 회피중심 대처를 사용하게 되고, 이로 인해 간호대학생의 소진이 가중되는 것을 확인할 수 있었다. 그러므로 간호대학생이 경험하는 무례함에 대하여 회피중심 대처보다는 보다 적극 적인 대처전략을 사용할 수 있도록 의사소통기술 및 자기주장 훈련이 필요하다.
본 연구는 간호대학생을 대상으로 임상실습에서 경험하는 간호사의 무례함과 이에 대한 대처가 소진에 미치는 영향을 파악하기 위해 시도되었다. 본 연구에서 대상자가 경험한 무례함은 사회적 지지 추구대처, 회피중심 대처, 소진과 양의 상관관계를 보여 무례함 정도가 높을수록 사회적 지지 추구대처와 회피중심 대처를 많이 사용하고, 무례함의 정도가 높을수록 소진도 높았다. 또한 회피중심 대처도 소진과 양의 상관관계를 보여 회피중심 대처를 많이 사용할수록 소진의 정도가 높음을 알 수 있었다.

후속연구

그러나 무례함에 대해서는 지금까지 그리 중요하게 언급되어 오지 않았고, 또 우리나라의 문화에서 무례함은 불쾌감을 느낀다 해도 사회적 규범이나 나이, 권력의 위계관계로 인해 참아야 하는 것으로 여겨져 왔기에 간호조직에서의 무례함의 정도와 사례에 대해서는 좀 더 많은 연구가 필요하다. 또한 이와 관련하여 영향을 미치는 요인들에 대한 파악과 함께 이를 해결하기 위한 환경적, 제도적, 중재적 방안 마련에 학교와 병원의 협력적 노력이 함께 이루어져야 한다.
학생이 임상실습에서 경험하는 무례함을 간과하지 않도록 학교와 병원이 협력하여 공식적 보고 체계를 마련하여야 한다. 그러나 본 연구는 두 지역 4개 대학의 간호대학생을 대상으로 하였기에 연구결과의 일반화에는 신중을 기하여야 하며 추후 반복연구가 필요할 것이다. 또한 임상실습에서 간호대학생이 경험하는 무례함을 정확하게 측정할 수 있는 한국의 문화에 맞는 측정도구의 개발이 필요하다.
일반적 특성에 따른 변수들의 차이를 살펴본 결과, 성별에 따라서는 소진 정도에 유의한 차이를 보여 여학생이 남학생보다 소진 정도가 높았다. 그러나 연구대상자의 91.5%가 여학생이므로 성별에 따른 소진의 차이는 추후 반복 연구가 필요할 것이다. 또한 학년에 따라서는 무례함에 있어 4학년 학생들이 3학년보다 경험 정도가 높았고, 이에 대한 회피중심 대처와 사회적 지지추구 대처도 더 많이 사용한 것으로 나타났다.
