Purpose: This paper reports the results of a hospital centered follow-up program on parenting stress, parenting efficacy and coping for mothers with very low birth weight (VLBW) infants. Methods: The follow-up program consisted of home visiting by an expert group and self-help program for 1 year. A non-equivalent control group pre-post quasi-experimental design was used. Participants were 70 mothers with low birth weight infants and were assigned to one of two groups, an experimental groups (n=28), which received the family support program; and a control group (n=27), which received the usual discharge education. Data were analyzed using ${\chi}^2$-test, t-test, and ANCOVA with IBM SPSS statistics 20.0. Results: Mothers' parenting stress (F=5.66, p=.004) was significantly decreased in the experimental group. There were also significant increases in parenting efficacy (F=13.05, p<.001) and coping (F=8.91, p=.002) in the experimental group. Conclusion: The study findings suggest that a follow-up program for mothers with VLBW infants is an effective intervention to decrease mothers' parenting stress and to enhance parenting efficacy and coping.

제안 방법

본 연구에서는 극소저출생체중아 어머니들의 양육스트레스를 감소시키고 양육효능감과 대처를 증가시키는 병원중심의 추후관리프로그램을 개발하고 평가하기 위해 시행되었다. 추후관리프로그램은 1년 동안 병원의 전문가로 구성된 가정방문을 제공하여 극소저출생체중아의 퇴원 후 성장발달을 관리하고 대상자의 요구도에 맞게 양육에 관련된 정보를 제공하며, 매달 1회의 정기적인 자조모임을 통해 전문가 교육과 상담, 회원 간의 상호교류를 지속적으로 진행하도록 유도하였다. 본 연구 결과, 병원중심의 추후관리프로그램은 극소저출생체중아 어머니의 양육스트레스를 감소시키고 양육효능감과 대처 행동을 증가시키는데 효과적인 중재임이 밝혀졌다.

대상 데이터

본 연구 대상자는 2012년 9월부터 2012년 12월까지 서울시 소재 K대학병원에서 출생한 극소저출생체중아와 어머니를 대상으로 하였다. 연구 대상자는 다음의 조건을 충족하는 자로 선정하였다.

이론/모형

극소저출생체중아 추후관리프로그램은 Dunst 등[11]의 가족지원프로그램 이론을 기반에 두고 어머니의 양육 관련 요구도 분석과 일본의 미숙아 재택요양지도 가정방문간호사업[18] 및 미숙아 자조모임인 고베시의 YoYo Club 모형[19]을 근거로 하였다. 구체적인 개발 절차와 내용은 다음과 같다.

성능/효과

둘째, 양육효능감 역시 극소저출생체중아의 추후관리프로그램을 제공받은 실험군의 어머니가 대조군에 비해 유의하게 증가하였다(p <.001). 본 연구에서 실험군 어머니들이 대조군에 비해 양육효능감이 높았던 이유는 퇴원 후 전문가 팀의 가정방문을 통해 어머니가 알고 싶었던 정보를 제공받고 1년간 자조모임에서 비슷한 어머니와 함께 경험을 공유하면서 정서적인 지지가 되었기 때문으로 사료된다.
셋째, 대처수준은 실험군이 대조군에 비해 유의하게 높아 병원중심의 추후관리프로그램을 제공받은 어머니들의 대처 행동 증가에 긍정적인 효과가 있는 것으로 나타났다(p =.002). 이 결과는 추후 관리프로그램에 참여한 실험군이 가정방문과 자조모임을 통해 스트레스 상황을 극복하고 효과적인 대처를 한 것으로 이해한다.
연구 결과를 살펴보면 첫째, 극소저출생체중아의 병원중심의 추후관리프로그램을 제공받은 실험군의 중재 전과 중재 후의 양육스트레스 차이점수와 대조군의 사전과 사후 간에 양육스트레스 차이 점수 간에 유의한 차이가 있어서(p =.004), 추후관리프로그램이 극소저출생체중아 어머니의 양육스트레스 감소에 효과가 있는 것으로 나타났다. 이 결과는 아동의 발달시기에 따라 행동반응이 달라서, 어머니들이 아기를 직접 키우면서 이에 대한 이해 부족과 어려움에 부딪치는데, 1년 동안의 추후관리 프로그램을 통해 양육에 대한 양육에 대한 구체적인 정보지원과 사회적 지지체계를 제공한 것이 양육스트레스를 감소시킨 것으로 이해한다.

후속연구

또한, 극소저출생체중아를 위한 의료측면의 간호중재 뿐만 아니라, 가족의 육아지원, 가족건강증진을 체계적으로 관리할 수 있는 지역사회 내의 전문적인 간호사의 역할확대에 기여할 것으로 본다. 향후 극소저출생체중아의 퇴원 후 관리를 위한 공공의료지원정책을 마련하기 위하여 지역사회 내의 전문가를 활용한 가정방문과 자조모임의 사회적 지지체계를 갖춘 지역사회 중심의 추후관리프로그램의 개발 및 운영이 필요함을 제언한다.
