Purpose: The purpose of this study was to investigate the level of cultural competence, intercultural communicative competence, and multi-cultural job stress among healthcare workers and to explore factors that are related to their cultural competence. Methods: The study subjects were 142 healthcare workers at a general hospital. Data were collected using a questionnaire on cultural competence, intercultural communicative competence, and multi-cultural job stress. A t-test, ANOVA Pearson's correlation coefficient, and multiple regression analysis were conducted using SPSS. Results: Cultural competence was significantly related to the necessity of multi-cultural education, and intercultural communicative competence was significantly related to age, a vocational career, communication in foreign languages, and having multi-cultural neighbors. Moreover, multi-cultural job stress was significantly related to religion. In multiple regression results, cultural competence was found to be related to intercultural communicative competence and multi-cultural job stress. Conclusion: Healthcare workers who are set to care for multi-cultural patients should improve intercultural communicative competence and reduce multi-cultural job stress.

본 연구는 연구 시작 전에 대상자에게 연구목적, 방법, 참여의 자발성, 철회의 자율성 및 정보의 비밀유지 등을 알리고 연구 참여에 동의하는 대상자에게 서면 동의를 받은 후 진행하였고, 설문지는 일주일 후에 회수하였다. 총 160부를 배부하였고, 회수된 152부(탈락률 5%)의 설문지 중 작성내용이 불충분한 설문지 10부를 제외한 142부를 최종 분석하였다.

데이터처리

• 대상자의 일반적 특성에 따른 문화적 역량, 문화간 의사소통 능력 및 다문화 직무스트레스 정도는 t-test, ANOVA로 분석하였으며, 사후분석은 Scheffé test를 시행하였다.
• 문화적 역량, 문화간 의사소통 능력 및 다문화 직무스트레스의 상관관계는 Pearson Correlation Coefficient로 분석하였다.

이론/모형

• 다문화 직무스트레스

다문화 직무스트레스 도구는 Jung(2016)이 개발한 도구로 문화역량 스트레스 5문항, 인종편견 스트레스 9문항으로 총 14문항으로 구성되었다. 각 문항들은 5점 리커트 척도로 1점 ‘전혀 그렇지 않다’, 2점 ‘대체로 그렇지 않다’, 3점 ‘보통이다’, 4점 ‘대체로 그렇다’, 5점‘매우 그렇다’로 구성되었고 점수가 높을수록 다문화 스트레스가 높은 것을 의미한다.
• 문화간 의사소통 능력

문화간 의사소통 능력 도구는 Lee(2011)가 개발한 도구를 Lee와 Kim(2015)이 수정ㆍ보완한 도구로 문화간 의사소통 기술 10문항, 문화간 의사소통 지식 10문항, 문화간 의사소통 인식 10문항, 문화간 의사소통 태도 10문항으로 총 40문항으로 구성되었다. 각 문항들은 리커트 5점 척도로 1점‘전혀 아니다’, 2점 ‘약간 그렇다’, 3점 ‘보통이다’, 4점 ‘자주 그렇다’, 5점‘항상 그렇다’로 구성되었고 점수가 높을수록 문화간 의사 소통능력이 높은 것을 의미한다.
• 문화적 역량

문화적 역량 도구는 Caffrey 등(2005)이 문화적 수용성, 문화적 인식, 문화적 지식을 측정하기 위해 개발한 ‘The Caffrey Cultural Competence in Healthcare Scale(CCCHS)’ 28문항에 Schim, Doorenbos, Miller와 Benkert(2003)이 개발한 ‘Cultural Competence Assessment(CCA)’중 문화적 간호행위를 측정하기 위한 14문항을 보완하여 Park(2011)이 번역·역번역 및 수정·보완하여 신뢰도와 타당도를 검증한 도구를 사용하였다. 본 도구는 문화적 수용성 12문항, 문화적 지식 10문항, 문화적 인식 6문항, 문화적 간호행위 14문항으로 총 42문항으로 구성되었다.

성능/효과

의료종사자들의 문화적 역량이나 문화간 의사소통 능력은 중간점수 이하로 역량이 낮았으며, 다문화 직무스트레스도 중간 점수 이상으로 스트레스 점수가 높은 것으로 나타났다. 문화적 역량은 다문화 관련 교육경험과 다문화 관련 교육 필요 여부에 따라, 문화간 의사소통 능력은 연령, 근무경력, 외국어 실력, 다문화 친구 유무에 따라, 다문화 직무스트레스는 종교에 따라 유의한 차이를 보였다. 또한 의료종사자들의 문화적 역량은 문화간 의사소통 능력과 다문화 직무스트레스와 관련성이 있는 것으로 나타났다.
본 연구는 의료종사자들을 대상으로 문화적 역량, 문화간 의사소통 능력 및 다문화 직무스트레스를 확인하고자 하였다. 의료종사자들의 문화적 역량이나 문화간 의사소통 능력은 중간점수 이하로 역량이 낮았으며, 다문화 직무스트레스도 중간 점수 이상으로 스트레스 점수가 높은 것으로 나타났다. 문화적 역량은 다문화 관련 교육경험과 다문화 관련 교육 필요 여부에 따라, 문화간 의사소통 능력은 연령, 근무경력, 외국어 실력, 다문화 친구 유무에 따라, 다문화 직무스트레스는 종교에 따라 유의한 차이를 보였다.

후속연구

첫째, 의료종사자들이 다문화 대상자에게 보건의료서비스를 제공하면서 가진 경험들에 대한 현상학적 질적 연구를 시행하여 문화적 역량을 파악하는 것도 필요하다. 둘째, 의료 종사자들의 문화적 역량을 높이기 위한 다문화 교육 프로그램의 개발 및 효과평가가 필요하다. 셋째, 다문화 대상자들이 많이 방문하는 보건진료소, 보건소, 의원 등의 일차의료기관 및 방문이나 가정간호를 담당하는 의료종사자들을 대상으로 한 반복연구를 제언한다.
둘째, 의료 종사자들의 문화적 역량을 높이기 위한 다문화 교육 프로그램의 개발 및 효과평가가 필요하다. 셋째, 다문화 대상자들이 많이 방문하는 보건진료소, 보건소, 의원 등의 일차의료기관 및 방문이나 가정간호를 담당하는 의료종사자들을 대상으로 한 반복연구를 제언한다.
이러한 결과를 토대로 다음과 같은 제언을 하고자 한다. 첫째, 의료종사자들이 다문화 대상자에게 보건의료서비스를 제공하면서 가진 경험들에 대한 현상학적 질적 연구를 시행하여 문화적 역량을 파악하는 것도 필요하다. 둘째, 의료 종사자들의 문화적 역량을 높이기 위한 다문화 교육 프로그램의 개발 및 효과평가가 필요하다.

	다문화 직무스트레스란 무엇인가?	, 2016). 다문화 직무스트레스는 일반적인 직무스트레스와는 다르게 다양한 문화 적응에 필요한 전문지식 부족, 다문화 교육 부족, 다양한 문화적 행동 차이에 대한 이해부족, 개인적인 인종 태도에 따른 수용과 관련되어 다문화 관련 업무를 수행함에 있어 발생하는 스트레스를 말한다(Im & Lee, 2014; Jung, 2016). 스트레스에 대한 노출은 개인의 신체적ㆍ정신적 건강상태에 심각한 영향을 미칠 뿐만 아니라 보건의료서비스를 제공하는데 있어서도 많은 영향을 미칠 수 있다.
