Purpose: This study is a descriptive correlational study to identify the mediating effect of resilience on depression and rehabilitation motivation. Methods: The study was conducted on 206 stroke patients that visited the D University Hospital in B City. Data was collected from July 1 to September 11, 2015. Data were collected for depression, rehabilitation motivation, and resilience. The data were analyzed by the t-test, ANOVA, Pearson's correlation coefficient, and hierarchical multiple regression using the SPSS/WIN 21.0 statistical program. Results: 33.7% of subjects showed beyond mild depression. The level of rehabilitation motivation was higher in male stroke patients, and when the level of education and monthly income was higher. Moreover the levels of rehabilitation motivation were significantly higher when stroke patients were participating in exercise continuously. Resilience was found to partially mediate the relationship between depression and rehabilitation motivation. Conclusion: The finding of this study demonstrates that intervention on resilience, in addition to direct intervention on depression, can improve rehabilitation motivation in stroke patients.

본 연구를 위해 D 대학교 의료원 생명윤리심의위원회의 승인을 받은 후 2015년 7월 1일부터 9월 11일까지 외래에 내원한 연구대상자에게 연구목적, 자료수집방법 등을 설명하고, 자료조사에 대한 동의를 얻었다. 외래 상담실에서 MMSE-K과 MRS를 측정하여 선정기준에 적합하면 설문조사를 시행하였다. 설문지는 자가 보고식으로 응답을 기입하도록 하되, 대상자가 편마비로 인해 설문지 작성이 어렵거나, 글을 모르는 경우, 시력 등의 문제로 직접 응답하기 어려운 경우에는 연구자와 훈련된 연구 보조자 1명이 질문지를 읽어 주어 답변한 내용을 적는 방식으로 하였다.

대상 데이터

극복력, 재활동기, 연령, 성별, 학력, 결혼상태, 치료비용, 주 돌봄자, 월 소득, 뇌졸중 재발여부, 기저질환, 재활치료)로 했을 때 189명이었다. 20%의 탈락률을 고려하여 총 227부의 설문지를 배부하였고, 응답이 불충분한 21부의 설문지를 제외한 총 206부를 분석하였다.
본 연구의 대상자는 뇌졸중으로 진단받은 자로 B광역시에 있는 D대학교 의료원 심뇌혈관센터 외래에 내원한 환자를 대상으로 하였다. 본 연구의 대상자 선정기준은 다음과 같다.

데이터처리

• 우울, 극복력, 재활동기 간의 상관관계를 파악하기 위하여 Pearson’s correlation coefficient를 실시하였다.
• 우울이 재활동기에 미치는 영향에 대한 극복력의 매개효과를 검증하기 위하여 위계적 회귀분석을 실시하고, 매개효과의 통계적 유의성 검정을 위해 Sobel test를 시행하였다.
• 일반적 특성과 질병 관련 특성에 따른 재활동기의 차이는 t-test 및 ANOVA로 검정하였다. 사후 검정으로 Scheffétest를 실시하였다.

이론/모형

극복력을 측정하기 위해 Wagnild (2009)가 18세 이상의 성인을 대상으로 개발한 극복력(14-item Resilience Scale, RS14)측정도구를 사용하였다. 본 연구에 사용하기 위하여 도구의 저작권을 가지고 있는 Resilience center (www.
우울정도를 측정하기 위해 Beck, Steer와 Brown (1996)이 개발한 21문항의 Beck Depression Inventory-II (BDI-II)를 사용하였다. 본 도구를 사용하기 위하여 도구의 저작권을 가지고 있는 한국심리주식회사(www.
재활동기를 측정하기 위해 Han과 Lim (2002)이 지체장애와 뇌병변장애인의 재활동기를 측정하기 위해 개발한 도구를 사용하였다. 이 도구는 총 27개 문항의 4점 척도로 구성되어 있다.

성능/효과

결론적으로 본 연구결과는 통원 뇌졸중 환자의 우울을 경감시키고 극복력을 향상시킨다면 뇌졸중 환자의 재활동기를 증진 시킬 수 있음을 제시해주었다.
본 연구에서 극복력은 우울이 재활동기에 미치는 영향에 대해 부분적인 매개효과가 있는 것으로 나타나 우울에 대한 직접적인 중재가 아니더라도 극복력에 대한 중재를 통해 재활동기에 영향을 미칠 수 있음을 보여주었다. 불임여성을 대상으로 삶의 질을 연구한 Kim와 Shin (2013)의 연구에서도 우울이 극복력을 매개로 삶의 질에 영향을 주는 것으로 나타나 실무에서 질환증상이나 우울을 직접적으로 중재하기 어려운 경우 극복력이라는 매개변수의 중재를 통해 건강상태나 삶의 질 등에 영향을 미칠 수 있음을 보여주는 결과라 하겠다.
본 연구에서 배우자가 있는 경우 재활동기가 높았고, 교육수준과 월 소득이 높을수록 재활동기가 높았는데, 급성기 및 아급성기 출혈성 뇌졸중 환자를 대상으로 조사한 Moon (2010) 연구에서도 가족과 함께 살 경우, 그리고 경제수준이 높은 경우 재활동기가 높게 나타났으며, Lee와 Kim (2014)의 연구에서도 뇌졸중 환자들이 교육수준이 높을수록, 그리고 배우자가 있는 경우에 재활동기가 높은 것으로 나타나 본 연구결과와 일치하였다. 월 소득이 높은 경우에 재활치료방법 선택에 있어 제약을 적게 받아 재활동기 정도에 영향을 미쳤다고 생각된다.

후속연구

둘째, 입원 뇌졸중 환자를 대상으로 극복력 증진 프로그램을 적용하여 재활동기에 미치는 효과를 검정하는 연구를 제언한다.
