노년기의 인지-언어적 변화는 일상생활의 불편함을 초래할 뿐 아니라 신경학적 질환의 주요 지표가 될 수 있다. 본 연구에서는 정상 노년층의 인지-언어 능력에 대한 주관적 및 객관적 평가 간의 상관성을 분석하고 인지-언어 능력의 예측 변인을 알아보았다. 이를 위해 2015년 7월부터 2016년 2월까지 만 65세 이상 정상 노인 63명과 이들의 정보제공자 63명을 대상으로 인지-언어 능력에 대한 자기 보고형 및 정보제공자 보고형 주관적 평가(ISCOLE)와 객관적 평가(CAPTBI)를 시행하였다. 그 결과, 첫째, 노인 및 정보제공자 집단 간에 주관적 평가의 차이는 유의하지 않았다. 둘째, 노인 집단은 8개, 정보제공자 집단은 15개 영역에서 주-객관적 평가 간에 유의한 상관성을 보였다. 셋째, 문제해결력 등은 정보제공자 보고형의 언어능력이, 그리고 집행기능과 언어능력은 정보제공자 보고형의 기억력이 주요 예측 변인이었다. 본 연구를 통해 인지-언어 능력에 대한 주-객관적 평가 간 상관성에 있어 노인 및 정보제공자 집단이 하위 영역별로 차이를 보이며, 정보제공자 집단의 주관적 평가가 더 많은 영역에서 객관적 평가와 상관성이 있음을 알 수 있었다. 이는 정상 노년층을 대상으로 한 인지-언어 능력의 평가 및 예방적 중재 시 적극 활용될 수 있을 것이다.

Cognitive-linguistic changes that normally accompany aging are often simply an annoyance, but in some instances they may herald a more perilous course of decline to a state of neurological disease. This study investigated the correlation between subjective and objective tests on cognitive-linguistic abilities in older adults and the predictors of objective performances. Healthy elders over 65 years of age (n=63) and their informants (n=63) completed the subjective and objective cognitive-linguistic tests (ISCOLE and CAPTBI) from July of 2015 to February of 2016. The main findings were as follows: performance on the self-report test was not significantly different from that on the informant-report test. Additionally, eight domains in older adults group and 15 domains in the informants group were significantly associated with performance on the objective test. Finally, language on the informant-report test was a predictor of several abilities including problem solving and memory on the informant-report test predicted executive function and language. The present study demonstrates that two groups have significant differences in correlation between subjective and objective tests on the cognitive-linguistic abilities, and there are more relevant domains in rating by informants. These findings have implications for the use of cognitive-linguistic evaluation and preventive intervention in clinical settings.

대상군 전체를 선별하기 위해 1차적으로 K-MMSE를 시행하였다. 이후 노인 집단을 대상으로 ISCOLE과 CAPTBI를 적용함으로써 주-객관적 인지-언어 능력을 평가하였다[18][23-25].

대상 데이터

본 연구는 2015년 7월부터 2016년 2월까지 서울 및 경기 지역에 거주하는 만 65세 이상의 정상 노인 63명(남:여=19:44) 및 노인의 정보제공자 63명(남:여=34:29)을 대상으로 하였다. 이들은 서울시 성북구 소재 노인복지관 1곳, 서울 및 경기 지역 노인정 3곳으로부터 표집된 노인과 정보제공자였으며, 기타 개별적인 자원자도 포함되었다. 대상자의 제외 기준으로는, (1) 신경학적 질환의 병력이 있는 경우, (2) K-MMSE(Korean version of Mini-Mental State Examination) 상 정상군 규준[20]에서 1 SD 미만의 수행력을 보이는 경우였다.

이론/모형

노인 집단의 객관적 인지-언어 능력에 대한 평가도구로서 CAPTBI(Cognitive-pragmatic language ability Assessment Protocol for Traumatic Brain Injury)[23-25]를 적용하였다. CAPTBI는 정상 노년층 및 신경학적 뇌손상 환자의 인지-언어 능력을 평가하는 도구로, 주의력, 지남력, 시지각력, 기억력, 조직화능력, 추론력, 문제해결력, 집행기능, 화용언어 등 9개 영역 및 22개 하위 범주에 해당하는 총 57개 문항으로 구성되어 있다.
대상 노인의 주관적 인지-언어 능력에 대해 노인 및 정보제공자 집단 간의 평정 결과를 비교하기 위해 두 집단 모두에게 ISCOLE(Informant-report Scale on Cognitive-Linguistic abilities for the Elderly)[18]을 시행하였다. ISCOLE은 노년층의 인지-언어 능력을 평가하는 정보제공자 보고형 도구로서 개발되었으며, 주의력(지속 및 분리 주의력), 지남력(사람, 시간, 장소), 시지각력(시공간력, 시각구성력), 기억력(장기, 단기, 즉각 기억 및 학습능력), 조직화능력(범주화, 순서화), 추론력(귀납적 및 수렴적 사고), 문제해결력(문제 추론, 의사 결정), 집행기능(계획화, 실행화), 언어능력(이해, 표현, 이름대기, 읽기, 쓰기, 계산, 화용) 등 9개 하위 영역의 27개 문항으로 구성되었다.

성능/효과

상관성의 정도는 인지-언어의 하위 영역별로 다르게 나타나기도 한다. 본 연구 결과에서 자기 보고형 평가는 문제해결력-지남력, 언어능력-기억력 등 총 8개 영역 간에, 그리고 정보제공자 보고형은 기억력-문제해결력, 총점-시지각력 등 총 15개 영역 간에 유의한 상관성을 보였다. 하위 영역별로 상관성을 분석한 연구들을 살펴보면, 정보제공자 보고형의 경우 기억력, 언어능력 등 전반적인 인지-언어 능력과의 비교를 통해 이미 상관성이 입증되었다[30][31].
이는 임상 현장에서 직접적으로 활용될 수 있는 증거 기반적 자료가 될 수 있다는 점에서 매우 중요하다. 본 연구를 통해 노인의 기억력은 자기 보고형의 언어능력을 통해 유효한 예측이 가능하며, 집행기능과 언어능력은 정보제공자 보고형의 기억력이 주요 예측 변인임을 알 수 있었다. 이같은 결과는 기억력과 언어, 집행기능 간의 연계적 기제로서 설명될 수 있다.
본 연구에서도 정보제공자 집단이 대상 노인의 수행력을 보다 엄격하게 평가하는 경향이 있었으나 자기 보고형 평가와의 차이는 미미하였다. 이를 통해 주관적 평가의 유형에 따른 차이가 노인의 인지-언어 능력을 파악하는 데 있어 영향력이 크지 않음을 알 수 있었다.

후속연구

셋째, 본 연구는 정상 노년층의 인지-언어 능력에 중점을 두었다. 향후 연구를 통해 MCI, 치매 등 환자군에 대한 적용을 추가함으로써 정상 노년층의 주관적 인지-언어 능력을 신경학적 질환의 예측 요인으로 보다 확대시킬 수 있을 것이다.
