The purpose of this study was to identify converged factors influencing subjective health status of patients with depression. The subjects of this study are 117 people answered that depression years of the 2013 National Health and Nutrition Examination Survey subjects. The results of this study were that the factors influencing subjective health status were education level, number of family member, quality of life, subjective body awareness, stress and they explained 55.9% of the variance. Therefore the intervention is considering various converged influencing factors should be done when the primary care for the promotion of subjective health status of patients with depression. This study identified a complex convergence of factors influence the subjective health status of patients with depression could be helpful on developing nursing intervention programs. It is necessary to identify forward more various social religious factors and disease influence the subjective health status of patients with depression.

대상 데이터

보건복지부와 질병관리본부에서 시행한 2013년도 국민건강영양조사는 총 8018명을 대상으로 하였으며, 본 연구에서는 ‘현재 우울증 유병여부’ 문항에 ‘있음’으로 명확하게 응답한 117명을 대상으로 하였다.
본 연구는 ‘국민건강영양조사 제6기 1차 년도’ 원시자료를 이용하였다.

데이터처리

• 우울증 환자의 일반적 특성, 신체적ㆍ심리적 요인들의 정도는 복합표본 빈도분석을 이용하였다.
• 우울증 환자의 주관적 건강상태와 신체적ㆍ심리적 요인 간의 관계는 복합표본 교차분석을 이용하였다.
본 연구의 자료는 SPSS 21.0 프로그램을 이용하여 복합표본계획파일(층화변수: 분산추정 층(kstrata), 집락변수: 조사구(psu), 가중치변수: 건강 설문과 검진 연관성 가중치(wt_itvex))을 생성한 후 분석하였으며, 유의수준은 .05로 하였다.

이론/모형

• 우울증 환자의 주관적 건강상태에 영향을 주는 요인은 복합표본 일반선형모형을 이용하였다.
EQ-5D(EuroQol-5Dimension)를 이용한 조사결과를 활용하였다. EQ-5D의 5가지 문항은 운동능력, 자기관리, 일상 활동, 통증/불편, 불안/우울이며, 점수가 높을수록 삶의 질이 낮은 것을 의미한다.

성능/효과

본 연구에서 우울증 환자의 주관적 건강상태에 영향을 미치는 요인은 교육수준, 가족 수, 삶의 질, 주관적 체형인식, 스트레스로 나타났고, 이들의 설명력은 55.9%로 높게 나타났다. 본 연구는 우울증 환자의 주관적 건강상태 증진을 위한 간호 중재 프로그램 개발 시 활용될 수 있는 기초자료를 제시하였다는 점에서 그 의의가 있다고 본다.

후속연구

첫째, 국민건강영양조사는 한 시점에서의 단면적인 조사 연구로서 우울증 환자의 주관적 건강상태에 영향을 주는 요인들에 대한 명확한 인과관계를 설명하기에는 어려움이 있으며, 둘째, 신체적‧심리적 요인 외에 가족의 지지와 같은 사회적 요인에 대한 부분은 반영되지 않았다는 점이다. 따라서 향후 우울증 환자의 주관적 건강상태에 영향을 주는 요인에 대한 후속 연구에서는 연구대상자 수의 확대와 사회적 ‧ 종교적 요인의 고려뿐만 아니라 특정질환 등의 신체적 요인을 첨부한 반복연구가 이루어질 필요가 있다.
이에 대해 Lee, Seong과 Sin[27]은 여자가 사회‧경제적인 지위와 신체 및 정신건강에 있어 열악한 상황에 놓여있기 때문에 여자가 더 우울증에 이환되는 비율이 높다고 지적하고 있다. 본 연구결과에 따르면 61-70세 나이 구간에서 우울증 비율이 18～19%에서 28.2%까지 급증하는 것을 나타내므로 특히 이 시기의 여성에 대한 보건의료계의 지속적인 관심과 함께 우울증 예방 중재에 대한 다양한 방안이 모색될 필요가 있다.
본 연구는 다음과 같은 몇 가지 제한점을 가지고 있다. 첫째, 국민건강영양조사는 한 시점에서의 단면적인 조사 연구로서 우울증 환자의 주관적 건강상태에 영향을 주는 요인들에 대한 명확한 인과관계를 설명하기에는 어려움이 있으며, 둘째, 신체적‧심리적 요인 외에 가족의 지지와 같은 사회적 요인에 대한 부분은 반영되지 않았다는 점이다. 따라서 향후 우울증 환자의 주관적 건강상태에 영향을 주는 요인에 대한 후속 연구에서는 연구대상자 수의 확대와 사회적 ‧ 종교적 요인의 고려뿐만 아니라 특정질환 등의 신체적 요인을 첨부한 반복연구가 이루어질 필요가 있다.
