Objectives There is growing interest in the management of cancer survivors due to the increase in long-term survivors. But no literature research focusing on the management of cancer survivors has been done in Sasang Constitutional Medicine despite the superiority in comprehensive management of patients. Therefore, we planned to look into the perspective of health preservation in Sasang Constitutional Medicine.Methods We investigated the 『Donguisusebowon-Sasangchobongwon(東醫壽世保元四象草本卷)』. We classified the contents which might be applicable to cancer patients. After classifying, we compared the contents with clinical experience and health preservation methods of Western Medicine.Results and Conclusions The health preservation is mentioned mostly as Joyang(調養). The Sasang Constitutional Medicine classifies the lives of people in 8 stages. Depending on the stages, there is a difference in the prognosis and management. Cancer patients can be regarded as being in the stages of Noeok(牢獄) and Wiegyeong(危傾). In these stages, patient's health preservation is important. In order to achieve long-term survival and to prevent disease progression, it is important to maintain requisite energy(保命之主). The attitudes that patient must have are simplicity(簡約), attentiveness(勤幹), vigilance(警戒), knowledgeability(聞見) for keeping one's own health and fulfilling the social obligations of human relations. The concept of health preservation in Sasang Constitutional Medicine covers the wide range of health. But detailed methods for patients to understand and practice are lacking. Moreover, it is hard for cancer patients to access the information. Therefore, further researches should be done to make the concept more specific and easy to access for cancer patients.

따라서 본 연구는 이제마의 저서 중에서 養生에 대해 풍부하게 나와있는 『東醫壽世保元四象草本卷』17을 중심으로 『東醫壽世保元甲午本』11과 『東醫壽世保元辛丑本』11을 살펴보고, 임상 경험을 토대로 하여 암 환자의 사상의학적 養生法에 대하여 초점을 맞춰서 고찰해보고자 한다.

성능/효과

결과적으로 사상의학은 서양의학과 병을 바라보는 관점이 다르기 때문에 養生에 접근하는 방식도 다르다. 서양의학에서 養生을 바라보는 관점은 암의 직접적 치료보다는 암 환자의 불편함을 감소하기 위한 보조적인 개념으로 발전하여 왔다.

후속연구

또한 이제마가 사상의학을 창안한 시대와 달리, 현대에서는 수술, 항암, 방사선 치료를 병행하는 암 환자가 많기 때문에 기존의 서양의학적 치료 관점 및 방법과 치료로 인해 유발되는 증상, 암의 종류별로 환자가 주의해야 할 사항 등에 대하여 숙지한 뒤에, 한의학적 관리 방법에 대해 큰 틀에서 고민하고 환자의 몸(行身)과 마음(用心)의 관리 방법을 구체적으로 세워나가야 할 것이다.
하지만 사상의학적 養生法에서는 구체적인 실천 방법이 부족한 점이 있으며 암 환자들이 쉽게 이해하기 어렵고, 환자 스스로 찾아볼 수 있는 책 또는 웹에 기반한 가이드라인이 부족하여 접근성이 떨어진다. 이에 대하여 이38 등, 김39 등, 박40의 연구에서는 심리치료를 性情 조절의 방법으로 활용하기 위한 접근 방법에 대해 연구되어있지만, 사상의학적 養生法을 암 환자에게 쉽고 구체적으로 접근하기 위해서는 앞으로도 많은 연구가 이루어져야 할 것이다.
마지막으로 이제마는 한 개인의 건강이 가정과 사회의 건강과 연관되어 발생할 수 있는 폐단에 대하여 언급하며, 養生의 중요성에 대해 강조하였다. 하지만 현대 사회의 구조 상, 암 환자가 養生을 하기 어려운 부분에 대하여는 한의계에서도 암 환자를 위한 사회 시스템 구축(事務) 및 보호자 지도 및 관리를 위한 프로그램 마련(居處)에 대해 노력하여야 할 것이다.
