Purpose: To identify the effects of a smart program for the patients who underwent percutaneous coronary intervention (SP-PCI) on coronary disease-related knowledge, health behaviors, and quality of life. Methods: A nonequivalent control group with a non-synchronized design was utilized and 48 participants (experimental=22, control=26) were recruited from a university hospital in Gyeongsang area from May to December, 2016. The 12-week SP-PCI consisted of self-study of health information using smart phone applications (1/week), walking exercise (>5/week) using smart band, feedback using Kakao talk (2/week), and telephone counseling (1/week). Patients in the control group received usual care from their primary health care providers and a brief health education with basic self-management brochure after the PCI. Data were analyzed using the SPSS 21.0 program through descriptive statistics, $x^2$ test, and t-test. Results: After the 12-week SP-PCI, the experimental group showed higher levels of coronary disease-related knowledge (t=2.43, p=.019), heart-related health behaviors (t=5.96, p<.001), regular exercise (Z=-4.47, p<.001), and quality of life-MCS (t=3.04, p=.004) and showed lower levels of stress (Z=-3.53, p<.001) and sodium intake (t=-4.43, p<.001) than those in the control group. There were no significant group differences in medication adherence and food intake in total energy, lipids, and cholesterol. Conclusion: The suggested SP-PCI provided easy access and cost-effective intervention for patients after PCI and improved their knowledge of the disease, performance of health behaviors, and quality of life. Further study with a wider population is needed to evaluate the effects of SP-PCI on disease recurrence and quality of life for patients after PCI.

본 연구의 목적은 관상동맥중재술 환자를 위한 스마트 프로그램(Smart Program for patients who underwent PCI [SP-PCI])이 환자의 질병관련 지식, 건강행위 이행(심장질환 관련 건강행위, 약물복용 이행, 규칙적 운동량, 식이섭취, 스트레스 관리) 및 삶의 질에 미치는 효과를 검증하는 것이다.
가설 설정
가설 1. 관상동맥중재술 환자를 위한 스마트 프로그램(SP-PCI)을 시행한 중재군(이하 중재군)은 시행하지 않은 대조군(이하 대조군)보다 질병관련 지식이 향상될 것이다.
가설 2. 중재군은 대조군보다 건강행위가 향상될 것이다.
가설 3. 중재군은 대조군보다 삶의 질이 향상될 것이다.
제안 방법
중재군에게는 퇴원 후 2주차 외래 방문 시 프로그램에 대한 질의응답과, ‘우리가족 심장건강 지킴이’앱을 활용하여 관상동맥질환 관련 자가학습 내용과 방법을 재교육하였고, 매주 전화상담 시 해당내용에 대해 질문함으로써 자가 학습여부를 확인하였다. 걷기운동은 스마트 밴드와 스마트 밴드 전용 앱을 활용하여 주 5회 이상 하루 10,000걸음 이상 걷는 것을 목표로 하여 5,000걸음으로 시작하여 매주 1,000걸음씩 늘이고, 운동 일수는 2주마다 1일씩 추가하여 저강도에서 중등도 이상의 강도로 점진적으로 늘려가도록 하였다. 걷기 운동 후 운동기록은 스마트 밴드 전용 앱에 연동하고, 스마트 밴드의 오작동을 대비하여 추가적으로 운동일지를 작성하도록 하였다.
대상 데이터
본 연구의 대상자는 Y시 소재 P대학병원에서 관상동맥중재술을 처음 시행하고 입원한 환자로 연구의 목적과 방법을 이해하고 아래의 선정기준을 만족하고 연구 참여에 동의한 자이며, 구체적 대상자 포함기준은 1) 나이 만 40세에서 70세 사이, 2) 의사소통에 장애가 없으며 국문해독이 가능한 자, 3) 걷기 등 신체 활동에 영향을 줄 수 있는 만성 폐쇄성 폐질환이나 관절질환이 없는 자, 4) 심각한 부정맥(특히 심실성 부정맥)이 없으며, 흉부통증이나 불편감 등의 증상이 없는 자, 5) 스마트폰 운영체제가 Android 4.3 또는 IOS 5.1 또는 그 이상 버전을 사용 중인 자이다. 제외기준은 1) 다른 운동 중재프로그램이나 연구에 참여하고 있는 자, 2) 스마트폰 사용에 주관적인 어려움을 호소하는 자이다.

이론/모형
1) 질병관련 지식(Disease-related knowledge) 질병관련 지식은 Kim [19]이 제작한 관상동맥질환자의 질병관련 지식측정도구를 Kim과 Park [20]이 수정․ 보완한 지식측정 도구를 사용하여 측정하였다. 지식측정 도구는 질병의 특성 4문항, 위험요인 8문항, 식이 7문항, 투약 7문항, 운동과 일상생활 7문항 등 총 33문항으로 구성된다.
삶의 질은 SF-12 (12-Item Short-Form Health Survey)로 측정하였다. 이 도구는 Ware 등[27]에 의하여 개발되었으며 8개 영역 12문항으로 구성되며, 응답자의 건강상태에 대한 주관적 지각을 측정한다. 신체적 건강관련 삶의 질(Physical Components Score [PCS])는 신체적 기능(Physical functioning), 신체적 역할 제한(Role physical), 통증(body pain), 일반 건강(General health)의 4개의 소영역 6문항으로 구성되고, 정신적 건강관련 삶의 질(Mental Components Score [MCS])은 정신 건강(Mental health), 감정적 역할제한(Role emotional), 사회적 기능(Social functioning), 활력(Vitality)의 4개의 소영역 6문항으로 구성된다.
스트레스는 Linn [24]이 개발한 Global Assessment of Recent Stress (GARS) Scale을 Koh와 Park [25]이 번역한 한국어판 도구를 도구 번역자에게 도구 사용 승인을 받은 후 사용하였다. 최근 일주일간 경험한 스트레스에 대한 인지반응을 평가하기 위한 도구로 총 8개 문항으로 구성되며, 일/직장/학교생활, 대인관계, 관계의 변화, 질병 및 상해, 경제적 문제, 비일상적인 사건, 일상생활의 변화 유무, 전반적인 스트레스 지각에 대한 평가로, 각 항목마다 0~9점으로 점수화하였고, 점수가 높을수록 스트레스 수준이 높음을 의미한다.
심장질환 관련 건강행위 측정도구는 Walker 등[21]의 건강증진 생활양식 도구를 바탕으로 Song 등[22]이 심장질환 환자에게 맞게 수정한 건강행위 도구를 도구 수정자에게 사용 승인을 받아 사용하였다. 본 도구는 다음의 5가지 심장질환 관련 건강행위 영역에 대한 개인의 실천 정도를 측정하는 것으로 건강책임(health responsibility) 6문항, 운동(exercise) 4문항, 식이습관(diet habits) 7문항, 스트레스 관리(stress management) 5문항, 금연(smoking habits) 3문항으로 이루어져 있으며, 본 연구에서는 약물복용 이행 도구(Morisky Medication Adherence Scale [MMAS])와 중복되는 건강책임 영역에 포함되는 약물복용과 관련된 1문항(“처방된 약을 빠뜨리지 않고 복용한다”)을 제외한 24문항을 사용하였다.
약물복용 이행정도는 Morisky 등[23]이 개발한 Morisky Medication Adherence Scale (MMAS)로 측정하였으며, MMAS는 건강전문가의 처방에 따라 정확하고 규칙적으로 약물을 복용하는지를 측정하는 8첨 척도의 자가 보고형 도구이다. 원 개발자에게 도구 사용 승인을 받은 후 구입하였으며, 2명의 연구원이 원 도구를 한글로 번역하였고, 영어와 한글 두 언어에 능숙한 2명이 역번역한 자료를, 연구팀에서 원 도구와 내용을 검토하여 완성본을 구성하였다.
성능/효과
본 연구는 관상동맥중재술을 받은 환자를 대상으로 12주간의 SP-PCI를 개발하고, 프로그램의 적용이 질병관련 지식, 건강행위, 삶의 질에 미치는 효과를 검정하기 위해 시도한 비동등성 대조군 전후 시차설계의 실험연구이다. 본 연구에서 제공한 SP-PCI는 관상동맥중재술을 받은 환자의 질병관련 지식, 건강행위 중 심혈관질환 관련 건강행위, 운동량, 삶의 질 MCS는 유의하게 향상하였고, 스트레스 정도와 나트륨 섭취량은 유의하게 감소하였다. 본 연구는 관상동맥중재술을 받은 환자의 질병관련 지식, 건강행위와 삶의 질을 향상시키기 위해서 스마트 중재프로그램을 간호사 주도하에 적용하였으며, 이미 개발된 모든 대상자들이 접근 가능한 관상동맥질환자의 건강교육 앱을 활용하였다는 점에서 의의가 있다.
후속연구
사전에 조사된 식이정보를 중심으로 좀 더 개별화된 집중적인 식이관리가 필요하며, 음식 사례나 요리방법, 재료선택을 포함하여 좀 더 구체적인 내용에 대한 교육이 필요할 것으로 생각된다. 국내 연구에서 관상동맥중재술을 받은 환자의 식이를 집중적으로 분석한 연구는 드물며, 관상동맥질환자에게 권고되는 적정 열량, 지질, 콜레스테롤, 나트륨섭취는 관상동맥질환자의 예후와 관련되며[27], 관상동맥중재술을 받은 환자들이 운동만 시행한 경우보다 운동과 식이관리를 병행하였을 경우 심근경색의 재발, 사망률 감소에 더 효과적이므로[29], 향후 관상동맥중재술을 받은 환자를 위한 식이중재개발과 적용이 필요할 것으로 생각된다.
스마트폰 앱은 시간과 장소에 제약 없이 접근할 수 있으며, 간단한 전화상담과 카카오톡을 통한 문자메시지를 통한 대상자관리는 짧은 입원 치료 이후에도 지속적이며 효과적인 생활습관 변화와 질병의 추후관리, 삶의 질 향상에 도움이 되었고, 의료비 절감에 도움이 될 것으로 기대된다. 추후 원격건강관리의 적극적 활용과 간호사의 참여는 대상자의 긍정적 건강결과와 의료비 절감에 필수적 요소일 것으로 생각되며, 추후 스마트 장비를 활용한 간호사 주도 중재프로그램의 개발과 효과검증이 다양한 영역에서 필요하다.
