This study examined emotional labor, fatigue, and musculoskeletal pain in hospital nurses. The study sample included 235 hospital nurses. The data were collected by questionnaire and analyzed with descriptive statistics, t-test, ANOVA, Scheffe test, Pearson's correlation coefficients, and multiple regression.The average emotional labor score was 3.27, and of fatigue was 2.04. The musculoskeletal pain score was 4.43. Emotional labor and fatigue are significantly positively associated with musculoskeletal pain(r=.180, p=.006; r=.278, p<.001). Emotional labor was positively associated with the degree of fatigue(r=.353, p<.001). The significant predictors of musculoskeletal pain for nurses were physical fatigue(${\beta}=.324$, p<.001) and gender(female)(${\beta}=.171$, p=.009), explaining 13.7% of the variance in musculoskeletal pain(F=16.271, p<.001). The results suggest that engaging in emotional labor related to nursing tasks increases the risks of fatigue and musculoskeletal pain. Therefore, it is important to develop a management program to relieve the emotional labor, fatigue, and prevent musculoskeletal pain for clinical nurses.

대상 데이터

본 연구는 B광역시 1개 종합병원에서 간호사로서 1년 이상 근무 중이며 연구목적을 이해하고 자발적 연구 참여에 서면 동의한 자를 대상으로 하였다. 자료수집은 기관생명윤리위원회의 승인(1040549-130911-SB-004 7-01)을 받은 후 B광역시 1개 종합병원에서 해당 기관 간호부의 협조를 받아 연구자가 각 간호부서별로 방문하여 연구내용을 설명한 후 연구참여 동의서를 받고 설문지를 배부 및 수거하였다.
95, 예측변수 11개로 계산한 결과 178명이 산출되었다. 자료수집과정에서 연구참여를 수락한 235명의 간호사에게 구조화된 설문지를 배포하였으며 배포된 설문지는 100% 회수되었고, 불성실하게 작성된 것 없이 총 235명의 자료가 최종 분석에 사용되었다.

이론/모형

감정노동은 Morris와 Feldman [30]의 감정노동도구에 기초하여 Kim[31]이 개발한 도구를 사용하였으며 도구사용에 대한 허락을 받았다. 본 도구는 실제 감정숨기기, 친절한 표현, 억지웃음에 대한 감정노동의 빈도 3문항, 실제 감정표현의 제한이나 미소응대 및 나쁜 감정을 숨기는 것에 대한 어려움을 다룬 감정표현의 주의성 3문항, 실제 감정의 왜곡, 감정과 일치하지 않는 표현법 및 그에 따른 혼돈에 대한 감정의 부조화 3문항 등 총 3개 하위영역 9문항으로 구성되어 있다.
피로는 일본 산업위생학회 산업피로연구위원회가 표준화한 피로자각 증상표를 사용하여 측정하였다. 본 도구는 총 30문항으로 신체적 피로 10문항, 정신적 피로 10문항, 신경감각적 피로 10문항으로 구성되어 있다.

성능/효과

간호사들은 다양한 간호업무를 수행하는 중 감정노동, 피로 및 근골격계 통증을 경험하였고 이들 변수간에는 유의한 정적 상관성이 있었다. 따라서 간호사의 감정노동과 피로도를 높이는 간호업무를 분석하여 관련되는 간호업무에 효과적으로 대처할 수 있도록 돕는 프로그램의 개발이 요구된다.
본 연구는 종합병원에 근무하는 간호사의 감정노동, 피로 및 근골격계 통증수준을 파악하고 그 관계를 확인하기 위해 시도하였다. 본 연구에 참여한 간호사들의 감정노동, 피로 및 근골격계 통증수준을 최저 0점에서 최고 100점으로 환산하면 감정노동은 57점(3.27점/ 범위 1~5점), 피로 35점(2.04점/ 범위1~4점), 근골격계통증은 44점(4.43점/ 범위0~10점)으로 나타나 해결해야 할 건강위험요소가 있음을 확인하였다. 간호사들은 업무수행 시 규정된 양식에 부합하는 정련된 감정표현을 해야 하는 심리적 압박을 경험한다.

	의료기관들은 고객만족도를 높이기 위해 어떤 노력을 하고 있는가?	최근 다양한 매체를 통한 의료지식과 건강정보의 사회적 확산은 양질의 의료서비스에 대한 소비자들의 요구에 부응할 수 있는 의료산업의 경쟁을 가속화 시키고 있다[1]. 의료기관들은 고객만족도를 높이기 위해 물적, 인적 자원의 확보와 서비스개선을 위한 제도를 마련하여 지속적으로 관리하고 있으며[2-3] 특히 간호사는 의료기관내에서 능숙한 실무수행과 친절한 환자응대에 대한 요구가 높은 대표적인 직무군이다[4]. 이에 간호사는 서비스를 제공하는 동안 현장에서 요구하는 규범에 맞추어 대상자가 우호적인 느낌을 갖도록 실제 자신의 감정을 숨기거나 왜곡해야 하는 경우가 많다[5].
