This study is a descriptive study that investigates the factors that affect the frailty of the elderly in the late period. The data were collected using a questionnaire during the period from 1 to 31 of December 2013. The subjects were 301 elderly people aged 70 years living in M city. The data were analyzed using a $x^2$ test, t-test, and ANOVA with the SPSS Statistics 18.0 program. Hierarchical regression analysis was performed to examine the factors that affect the weakness of elderly people. The results showed that 15.3% of the elderly living at home were frail. The factors that affect the frailty of the elderly were sex (p<0.01), nutritional status (p<0.01), cognition function (p<0.01), ADL (p<0.01), IADL (p<0.05), visual acuity (p<0.05), and chewing discomfort (p<0.05). A higher the degree of frailty was associated with women, poorer nutritional status, higher impairments to perform the daily activities (ADL and IADL), poorer hearing, and more feeling of chewing discomfort. These results can explain the frailty of elderly people over 70 years of age in a local community and the variables of health. The results suggest that the development of a comprehensive program for the elderly with weak physical, emotional, and functional health should be given priority.

데이터처리

연구대상자의 인구학적 특성, 신체적 건강관련 특성, 정서적 건강관련 특성, 기능적 건강관련 특성을 살펴보기 위하여 빈도분석을 실시하였고, 각 특성별 허약점수의 차이를 분석하기 위하여 t-test, ANOVA를 실시하였으며, 사후검증은 Scheffe test를 실시하였다. 노인의 허약에 영향을 미치는 요인을 살펴보고, 이들 요인의 상대적 영향력의 크기를 파악하기 위하여 허약점수를 종속변수로 하고 인구학적 특성 및 신체적 건강, 정신적 건강, 기능적 건강을 독립변수로 한 위계적 회귀분석(hierarchial multiple regression)을 실시하였다. 또한 신체적 건강을 매개로 하여 노인의 허약에 미치는 직⋅간접효과는 측정모형과 이론모형을 동시에 고려한 구조방정식모형을 이용하였으며 AMOS 21.

이론/모형

노인의 정신건강을 측정하기 위하여 인지기능을 측정하였으며, 본 연구에서 인지기능을 측정하기 위하여 Folstein, Folstein과 McHugh[21]가 개발한 Mini-Mental Status Exam (MMSE)을 토대로 개발된 한국판 간이정신상태 검사를 보완하여 개발된 치매선별용 간이정신상태검사(Mini-Mental Status Exam -Dementia Scale, MMSE-DS)[22]를 사용하였다. MMSE-DS는 시간지남력 5문항, 장소지남력 5문항, 기억 등록 및 회상 2문항, 주의집중력 1문항, 언어기능(이름대기, 따라 말하기, 명령시행) 3문항, 구성능력(도형모사) 1문항, 판단력 2문항의 총 19문항으로 구성되어 있다.

성능/효과

연구대상자 중 15.3%가 허약노인으로 분류되었으며, 남자노인보다 여자노인의 허약비율이 높았으며, 연령이 증가할수록 허약비율이 증가하였다. 후기 노인의 허약정도에 영향을 미치는 요인은 성별, 영양상태, 인지기능, 일상생활수행능력, 수단적 일상생활수행능력, 청력, 저작불편감으로 나타났으며, 남자보다 여자일 경우, 영양 상태가 불량할수록, 일상생활수행능력에 장애가 있을수록, 수단적 일상생활수행능력에 장애가 있을수록, 청력이 나쁠수록, 저작불편감을 느낄수록 허약점수가 높았다.
3%가 허약노인으로 분류되었으며, 남자노인보다 여자노인의 허약비율이 높았으며, 연령이 증가할수록 허약비율이 증가하였다. 후기 노인의 허약정도에 영향을 미치는 요인은 성별, 영양상태, 인지기능, 일상생활수행능력, 수단적 일상생활수행능력, 청력, 저작불편감으로 나타났으며, 남자보다 여자일 경우, 영양 상태가 불량할수록, 일상생활수행능력에 장애가 있을수록, 수단적 일상생활수행능력에 장애가 있을수록, 청력이 나쁠수록, 저작불편감을 느낄수록 허약점수가 높았다.

후속연구

본 연구를 통하여 70세 이상 지역사회 거주노인의 허약수준을 파악하고, 이에 영향을 미치는 건강차원의 변수를 파악할 수 있었다. 향후 신체적, 정서적, 기능적 건강이 모두 저하되어 있는 허약노인에 대한 포괄적인 연구가 활발히 이루어질 것으로 기대되며, 허약으로의 진행을 예방하기 위한 적극적인 건강증진 방법이 모색되어야 할 것이다.
둘째, 대상자가 농촌지역 재가노인으로 한정하여 삶의 환경을 고려하지 않았다. 후속연구로 도시지역 재가노인과의 비교를 통한 연구가 진행되어야 할 것이다. 셋째, 연구에 사용된 설문지가 원칙적으로 자가기입방식 설문이지만 시력의 문제로 스스로 하기 어려운 경우나 문항에 대한 이해가 부족한 경우 조사원이 대신하여 읽어주거나 이해를 도운 경우가 있어 조사원에 의한 오차가 존재할 가능성이 있다.
