Purpose: The purpose of this study was to identify the factors affecting the perceptions of patient safety culture, safety care knowledge, and safety care activity among nurses at orthopedic hospitals. Methods: Data were collected during Feb. 16 and Feb. 26, 2017, from 195 nurses of 9 small to medium sized orthopedic hospitals. Questionnaires about patient safety culture, safety care knowledge and activity were used. Data were analyzed by descriptive statistics, t-test, ANOVA, $Scheff\acute{e}$ test, Pearson's correlation coefficient, and stepwise multiple regression. Results: The safety care activity was positively correlated with perceptions of patient safety culture (r=.50, p<.001) and knowledge (r=.48, p<.001). Factors that had influence on the safety care activity were the patient safety culture (${\beta}=.30$, p<.001), age (${\beta}=.27$, p<.001), and knowledge of the safety activity (${\beta}=.21$, p=.004). The patient safety care activity was explained 36.6% by those factors. Conclusion: To enhance the patient safety care activity, it should be provided the environment and open communication for the perceptions of patient safety culture and the in service education program for safety care knowledge.

따라서 본 연구는 중소 정형외과병원 간호사의 환자안전문화인식, 환자안전간호지식과 환자안전간호수행수준을 파악 하고, 이들의 관계를 분석하고, 환자안전간호수행에 영향을 주는 요인을 탐색하여 중소 정형외과병원 간호사들의 환자안전간호활동수행을 증진시키기 위한 맞춤형 전략을 수립하기 위한 기초자료를 제공하고자 한다.
제안 방법
자료수집 전 경상대학교 생명윤리심의위원회(GIRB-A17-Y-0001)에서 승인을 받은 후 연구자가 각 병원의 간호부에 방문하여 연구 목적을 설명한다. 협조에 대한 동의를 얻은 후 연구참여동의서에 서명한 간호사를 대상으로 설문지를 배부하고 직접 작성하도록 하였으며, 연구대상자의 자료는 무기명으로 처리할 것과 설문지 응답과정에서 자유롭게 연구참여를 중단할 수 있음을 알렸다. 작성한 설문지는 연구자가 각 의료기관을 방문하여 회수하였다.
대상 데이터
본 연구의 대상은 B시에 소재하고 있는 100병상 이상 300병상 미만의 9개의 중소 정형외과병원 병동간호사를 대상으로 하였다. 구체적으로는 이들 병원의 병동간호사로서 본 연구에 참여하기로 동의하고, 3개월 이상의 업무경력을 가진 간호사를 대상으로 하였다.
15 (Kim & Lee, 2015)로 하여 가능성 있는 독립변수 12개를 적용한 대상상자는 157명이다. 탈락률을 고려하여 200명에게 설문을 배부 하여 미회수와 응답이 누락된 설문을 제외하고 195부를 분석하였다.
데이터처리
• 대상자의 일반적 특성에 따른 환자안전간호문화인식, 환자안전간호지식, 환자안전간호수행의 차이를 분석하기 위해 t-test, ANOVA를 이용하여 분석하고, 사후 분석은 Scheffé test로 분석하였다.
• 환자안전간호수행에 영향을 미치는 요인을 파악하기 위하여 단계적 회귀분석(stepwise multiple regression)을 실시하였다.
• 환자안전문화인식, 환자안전간호지식, 환자안전간호수행의 상관관계를 알아보기 위하여 Pearson’s coefficient로 분석하였다.
이론/모형
환자안전간호수행을 측정하기 위하여 Park (2008)이 보건복지부 산하 Korea Health Industry Development Institute (2007)에서 개발한 안전평가항목 총 119항목 중환자안전간호 수행을 중심으로 개발한 도구를 Lee (2014)가 수정·보완한 도구로 측정하였다. 환자안전간호수행과 관련된 총 9개 영역의 72문항 중 중소병원에는 신생아실이 없으므로 신생아실과 관련 문항 1문항을 제외한 총 71문항을 이용하여 측정하였다.
환자안전간호지식을 측정하기 위해서 Jang (2012)의 환자안전역량 측정도구 중 지식에 해당하는 영역을 Yang (2015)이 수정한 도구를 간호학과 교수 1인과 정형외과 병동 수간호사 2인, 정형외과 전문의 1인에게 내용타당도를 확인받아 측정하였다. 지식은 자가평가형식으로 10문항으로 구성되었다.
환자안전문화인식을 측정하기 위하여 Agency for Healthcare Research and Quality (AHRQ) (2004)에서 개발한 환자 안전문화 측정도구(hospital survey on patient safety culture) 를 Kim, Kang, An과 Sung (2007)이 한글로 수정 번역한 ‘환자의 안전문화조사도구’를 수정·보완한 Lee와 Kim (2011)의 환자안전문화인식의 도구로 사용허가를 받은 후 측정하였다.
성능/효과
본 연구에서는 100병상 이상 300병상 미만의 정형외과병원의 병동간호사를 대상으로 환자안전문화인식, 환자안전간호지식과 환자안전간호수행의 정도를 조사하였다. 연구결과 중소병원 정형외과 간호사들의 환자안전문화인식은 5점 만점에 3.44±0.39점으로, 보통 수준으로 인식하는 것으로 나타났다. 중소병원 간호사를 대상으로 한 Lee와 Kim (2011)의 연구에서는 3.
