Objectives: This study used data of urban and rural elderly living alone to compare the general characteristics of the elderly who have had suicidal ideation, and to investigate the suicidal ideation of urban-rural elderly with varying degrees of emotional support status, health status, and health behavior. Methods and materials: The research participants included 2,750 elderly aged 65 or above, of which 1,487 were from dong unit and 1,263 were from eup myeon units according to the raw data of 2011 Survey on Elderly Status. All collected data were analyzed using cross tabulation of SPSSWIN(ver 21.0) program and multivariate logistic regression. Results: The results were as follows. First, the factors that influenced the suicidal ideation of urban and rural elderly living alone included degree of depression, experience of abuse, and nutritional management status. Second, the suicidal ideation of urban elderly living alone was affected by the emotional support of their children who live apart and their grandchildren. Third, the suicidal ideation of rural elderly living alone was affected by the emotional support of their kin (including siblings).

대상 데이터

본 연구는 “2011년도 노인실태조사”자료를 이용하여 연구하였다. “2011년도 노인실태조사”는 2011년 8월 31일∼2011년 11월 30일 기간 중 현장조사로 진행되었고, 총 11,542명에 대한 조사가 완료되었다.
이 중 65세 이상은 10,674명이었다. 본 연구의 대상은 65세 이상 독거노인으로 동부 1,487명과 읍·면부 1,263명인 2,750명의 자료를 이용하여 연구하였다(Figure 1).

데이터처리

연구대상자의 일반적 특성과 자살생각의 유무는 교차분석을 실시하였다. 또한 정서적 지지상태, 건강상태, 건강행태가 자살생각에 영향을 미치는 요인은 독립변수 간의 영향요인을 보정하기 위해 다변량 로지스틱 회귀분석(multiple logistic regression)을 실시하여 교차비(odds ratio,OR)와 교차비의 95%신뢰구간을 구하였다. 모든 통계학적 유의수준은 p<0.

우울증은 한국판 SGDS (Short form of Geriatric Depression Scale) 척도를 사용하였는데, SGDS는Sheikh와 Yesavage가 기존의 GDS에 대한 진단적 타당도 연구를 바탕으로 해서 GDS의 문항중에서 우울증상과 상관관계가 가장 높은 문항을 15문항 선택해서 축소 제작하였는데 GDS와 SGDS가 상당히 높은 상관관계가 있음을 보고하였다(r=0.84, p<0.001). 이 척도의 가장 적절한 절단점은 6∼7점으로 추정되었다[19,20].

성능/효과

도시와 농촌 독거노인의 자살생각은 학대 경험이 없는 집단 보다 경험이 있는 집단에서, 우울정도가 정상인 집단보다 우울 집단에서, 영양상태가 악화될수록 유의하게 높았다. 이는 노인층이 비 노인층보다 심신이 더 쇠약하여 우울과 영양상태가악화될수록 더 많은 자살생각을 하는 것으로 보인다.
반면에, 농촌 독거노인은 친인척(형제·자매 포함)간의 정서적 지지가 많을수록 자살생각이 유의하게 낮았다. 농촌노인의 경우 도시지역보다 자녀와 별거하는 비율이 높고 이에 따라 자녀의 부양기능이 낮다는 점을 감안할 때[22], 농촌 독거노인은 비동거 자녀, 손자·손녀간의 정서적 지지보다 친인척간의 정서적 지지가 많을수록 자살 생각이 낮아질 것으로 사료된다.
자살생각을 한 도시 독거노인은 비동거 자녀, 손자·손녀간의 정서적 지지가 적을수록 자살생각이 유의하게 높았으며, 이는 가족 간의 강한 유대관계가 있는 한국의 특성을 볼 때 노인이 되어서도 이러한 지지체계가 지속되어야 하나 가족 간의 구조가 빠르게 변화하고 있고, 노인들이 인식하는 가족구조와 현실적인 가족 구조간의 차이가 발생하고 있다. 결국 핵가족화 등으로 인한 가족 결속도의 감소는 독거노인을 더 고독하게 하고 독거노인들은 무관심 속에 방치된 채 자살이라는 극단적인 방법을 생각하게 된다[21].

후속연구

끝으로, 본 연구는 횡단적 연구방법을 사용하였기 때문에 변수들 간의 인과관계를 규명하는데 제약이 있고, 영양관리상태 지표는 객관적인 지표가 아닌 주관적인 지표를 사용하였기에 지표의 정확성이 다소 떨어질 가능성이 있다.
