치매특별등급서비스는 어떤 서비스인가? 이에, 2014년 7월 경증치매 환자들의 인지기능 유지 및 악화 방지, 가족의 부양부담 감소를 목표로 한 치매특별등급서비스가 도입되었다. 이 서비스는 장기요양등급 중 5등급 수급자 즉, 치매진단을 받고 요양인정점수 45점 이상 51점 미만인 자에게 운동, 미술, 음악활동, 회상훈련과 같은 인지활동형 프로그램을 제공하는 것으로, 대상자들이 주 ‧ 야간보호시설에서 서비스를 이용하는 것을 원칙으로 하되, 접근성 등 다양한 사유로 주 ‧ 야간보호시설을 이용할 수 없는 경우에 한해 가정에서 인지활동형 방문요양서비스를 이용할 수 있다[2,12]. 주 ‧ 야간보호시설에서는 1일 8시간, 월최대 22일간 주 ‧ 야간보호와 1일 1시간 이상 인지활동형 프로그램을 제공하며, 인지활동형 방문요양서비스는, 주 3회 이상, 1회 1시간 이상 최대 2시간의 인지자극활동 프로그램을 제공한다[2,12]. 그리하여, 2014년 치매특별등급서비스 이용자가 5,133명[1]이던 것이, 2015년 6월 기준 16,295명[12]으로 급증하였고, 2017년 6월 기준 치매특별등급(5등급) 인정유지자가 35,879명으로[13] 이용자수가 꾸준히 증가하고 있다.
치매는 어떤 질환인가? 치매는 뇌신경세포의 손상으로 발병 후 10년 이후까지도 생존이 가능하지만 만성적으로 점점 악화되어가는 소모성 질환[4]으로, 환자와 가족에게 큰 부담이 되고 있다. 실제로 치매노인을 돌보는 가족들은 장기적인 부양으로 인한 역할 과중과 신체적 피로로 인해 체력이 소진되고, 건강상태가 매우 좋지 않다고 평가하고 있으며[4,5], 치매노인들이 나타내는 여러 정신행동증상들로 인해 우울, 정신적 부담과 지침 같은 심리적 어려움을 겪는다[4,6].
