This study was conducted to investigate the effects of oral health awareness and oral health behavior on cardiocerebrovascular prevention behavior in a cardiocerebrovascular diseases risk group and provide basic data for a nursing intervention program for the group. Data were collected from 131 people in the risk group of cardiocerebrovascular diseases living in J Province in February and March of 2018. The obtained data were analyzed using descriptive statistics, t-tests, Pearson's correlation, and stepwise multiple regression analysis. The results revealed that preventive acts were positively correlated with subjective oral health state (r=0.261, p=0.003), importance of oral health (r=0.250, p=0.004), and practice of oral health (r=0.303, p<0.001). Moreover, acts to prevent cardiocerebrovascular disease were influenced by oral health (${\beta}=0.29$, p<0.001), age (${\beta}=0.27$, p=0.001), and subjective oral health state (${\beta}=0.24$, p=0.003). Specifically, scores of preventive acts for cardiocerebrovascular diseases increased with higher degree of oral health behavior, older age, and better subjective oral health state. These variables had an explanatory power of 19.3%. The results of this study imply a need to prepare measures to enhance the oral health level of the risk group of cardiocerebrovascular diseases and conduct ongoing follow-up studies of the perception, attitude, and behavior of individuals toward oral health, as well as the state of oral health.

이에 본 연구는 심뇌혈관질환 위험군의 주관적 구강건강 상태와 구강건강의 중요성, 구강건강 행위가 심뇌혈관질환 예방행위에 미치는 영향을 파악하고, 이들에게 구강건강의 중요성을 인식시키고, 구강건강 행위 수준을 향상할 수 있는 간호중재 프로그램 개발의 기초자료로 제공하고자 한다.
제안 방법

독립변수는 일반적 특성 중 심뇌혈관질환 예방행위에 유의한 차이를 보인 연령(세), 당뇨병약투여 여부 (아니오 기준)등 2개와 상관분석에서 심뇌혈관질환 예방행위에 유의한 관계가 있는 구강건강상태, 구강건강의 중요성, 구강건강실천 등 총 5개를 투입하였다.

첫째, 심뇌혈관질환 위험군의 일반적 특성, 구강건강 관련 특성, 주관적 구강건강상태, 구강건강의 중요성, 구강건강행위, 심뇌혈관질환 예방행위에 대해 파악한다.
대상 데이터
10%의 탈락률을 고려하여 총 140부의 설문지를 배부하여, 응답이 불충분한 설문지를 제외하고 131부을 분석하였다.
대상자는 전라남도에 소재한 2개의 종합병원, 보건소, 보건진료소 및 노인복지관을 통해 만 20세 이상의 성인중 심뇌혈관질환 위험군(비만, 고혈압, 당뇨병, 고지혈증, 협심증)의 선행질환 1개 이상을 가지고 본 연구의 목적을 이해하고 의사소통에 문제가 없으며 연구 참여에 동의한 자로 하였다.
데이터처리
대상자의 일반적 특성과 치주질환 인식은 실수, 백분율, 평균과 표준편차를 하였고, 구강건강행위와 심뇌혈관질환 예방행위의 차이 검증은 t-test로 분석을 하였다.
변수 간의 상관관계는 피어슨 상관계수(Pearson‘ correlation coefficient)를, 심뇌혈관질환 예방행위에 미치는 영향 요인 파악은 다중회귀분석으로 분석하였다.
이론/모형
구강건강행위를 측정하기 위해 Kim[24]의 연구에 사용된 문항을 기반으로 본 연구의 목적과 대상에 맞게 수정, 보완하여 사용한 Won[23]의 도구를 사용하였다. 구강건강관리에 있어서 기본 요소에 해당하는 칫솔질 방법, 구강위생용품, 정기적 치과 방문에 관한 문항과 식이조절 및 교육, 관심에 관한 문항으로 구성되어있다.
심뇌혈관질환 예방행위를 측정하기 위해 건강보험공단에서 제작한 만 40세용 생애전환기 건강진단 생활습관 평가도구를 Park[25]가 수정·보완한 도구를 사용하였다.
치주질환 인식을 측정하기 위해 No[21], Jung[22]의 연구에서 사용된 문항을 기반으로 심뇌혈관질환 위험군에게 맞도록 수정·보완하여 사용한 Won[23]의 도구를 사용하였다.
성능/효과
구강을 건강하게 가꾸는데 가장 중요한 요인은 규칙적인 칫솔질 45.8%이었으며, 가장 심각한 구강건강문제는 치주질환이 66.4%로 가장 높았으며, 구강건강이 사회활동에 미치는 영향 정도는 영향이 있다가 47.3%이었다.
대상자의 심뇌혈관질환 예방행위는 주관적 구강건강상태(r= .261, p= .003), 구강건강 중요성(r= .250, p= .004), 구강건강행위(r= .303, p < .001)와 유의한 양의 상관관계가 있는 것으로 나타났다[Table 4].
본인의 구강건강상태는 보통 45.0%, 현재 구강관리를 위해서 하고 있는 사항에 대한 다중응답 분석 결과 스케일링이 42.7%로 가장 많았으며, 자신의 구강건강의 문제점에 대한 다중응답 분석 결과 치주질환이 64.1%로 가장 많았으며, 구강건강문제의 원인은 원래 치아 또는 잇몸이 약해서가 52.8%로 가장 높았다.
심뇌혈관질환 예방행위에 영향을 미치는 변인으로 구강건강행위, 연령, 주관적 구강건강 상태가 있었고, 구강건강행위 정도가 높을수록, 연령이 높을수록, 주관적 구강건강상태가 좋을수록 심뇌혈관질환 예방행위 점수가 높아진다고 나타났다.
연구결과 심뇌혈관질환 예방행위는 치주질환 인식인 주관적 구강건강상태, 구강건강 중요성, 그리고 구강건강실천과 양의 상관관계가 있었고, 심뇌혈관질환 예방행위에 영향을 미치는 요인은 구강건강행위, 연령, 주관적 구강건강상태로 나타났으며, 이들 변수의 설명력은 19.3%이었다.
현재 치과치료 여부는 아니오 63.4%, 지난 1년간 구강상태를 알아보기 위해 치과를 방문한 횟수는 없다 39.7% , 칫솔질하는 이유는 충치와 치주질환의 예방을 위해서가 66.4%로 가장 높았으며, 칫솔 이외 사용하고 있는 구강 위생용품에 대한 다중응답 분석 결과 치간 칫솔이 40.5%로 높았다[Table 2].
후속연구
따라서 구강건강에 대한 인식 및 행위와 심뇌혈관질환 예방행위를 위해서는 선행질환을 가진 대상자의 구강건강 인식 및 행위의 실태조사가 필요하며 대상자들의 구강건강 위험인자에 대한 지식 정도와 건강 행위 실천의 관련성을 연구해볼 필요가 있다.
즉, 구강건강행위 정도가 높을수록, 연령이 높을수록, 주관적 구강건강상태가 좋을수록 심뇌혈관질환 예방 행위 점수가 높아진다고 할 수 있다. 따라서 본 연구의 결과는 심뇌혈관질환 위험군의 구강건강인식의 평가와 심뇌혈관질환 예방행위의 실천 정도, 심뇌혈관질환 예방행위에 대한 중재와 그에 따른 예후 향상에 기대 여할 수 있는 연구의 기초자료로 활용될 것으로 기대된다. 본 연구를 근거로 심뇌혈관질환 위험군의 구강건강 수준을 향상하는 방안을 마련하기 위해 구강건강인식, 태도 및 행동과 구강건강상태 등에 관한 계속적인 연구가 필요하고, 대상자를 확대하여 반복적인 연구를 할 것을 제언한다.
본 연구는 일개 지역에 국한된 선행질환을 가진 성인에 대한 연구로 충분한 표본수를 확보하지 못하였고 본 연구의 결과를 적용하여 일반화하는데 제한점이 따르므로 반복적인 연구가 필요하다.
따라서 본 연구의 결과는 심뇌혈관질환 위험군의 구강건강인식의 평가와 심뇌혈관질환 예방행위의 실천 정도, 심뇌혈관질환 예방행위에 대한 중재와 그에 따른 예후 향상에 기대 여할 수 있는 연구의 기초자료로 활용될 것으로 기대된다. 본 연구를 근거로 심뇌혈관질환 위험군의 구강건강 수준을 향상하는 방안을 마련하기 위해 구강건강인식, 태도 및 행동과 구강건강상태 등에 관한 계속적인 연구가 필요하고, 대상자를 확대하여 반복적인 연구를 할 것을 제언한다.
이를 토대로 심뇌혈관질환위험군의 주관적인 건강인식 상태를 높이고, 효과적인 구강건강 실천을 할 수 있도록 구강보건교육 프로그램을 개발하고, 구강건강교육에 참여할 수 있는 여건 조성이 필요하다.
