Purpose: To investigate the effect of a maternal role adjustment program on first-time mothers. Methods: The research was quasi-experimental with a non-equivalent control group non-synchronized design. Participants were first-time mothers admitted to two postpartum clinics at women's hospitals. The experimental group had 38 mothers and the control group had 35 mothers. A maternal role adjustment program was applied individually to the experimental group between the 1st and 2nd weeks after childbirth. Assessing Adaptation to Motherhood, Semantic Differential Scale-Myself as Mother, and Edinburgh Postnatal Depression Scale were used to measure effects of the program. A pre-test was conducted in the 1st week after childbirth while post-tests were conducted in the 4th and 6th weeks. Data were analyzed with $x^2$ test, Fisher's exact test, t-test, and repeated measures ANOVA using SPSS 24.0. Results: Maternal role adjustment (F=6.17, p=.015) and maternal identity (F=6.63, p=.012) were significantly increased in the experimental group compared to those in the control group. However, the difference in postpartum depression (F=1.11, p=.335) was not statistically significant between the two groups. Conclusions: The maternal role adjustment program can be utilized as an effective nursing intervention program to enhance maternal role adjustment and maternal identity for first-time mothers.

제안 방법

자료수집과 실험중재를 연구자가 직접 시행하였으며, 연구대상자에게는 연구의 목적, 절차, 수집된 자료의 관리, 자발적 참여, 연구 참여의 중단에 대한 내용을 설명하고, 연구 참여에 대한 서면 동의서를 받은 후 연구를 진행하였다. 실험의 확산을 방지하기 위해 대조군의 자료수집이 종료된 후에 실험군을 대상으로 모성 역할적응 프로그램을 적용하고 자료 수집을 진행하였다. 대조군은 2016년 12월 26일부터 2017년 6월 20일까지, 실험군은 2017년 6월 21일부터 2018년 2월 20일까지 자료수집이 이루어졌다.

대상 데이터

본 연구는 B 광역시 소재의 M여성병원(입원실 64병상, 조리원 39실)과 W여성병원(입원실 26병상, 조리원 50실)의 산후조리원에 재원하고 있는 임신 37~42주 정상질식분만 초산모를 대상으로 하였다. 임신 및 분만과 관련한 산과적 합병증, 정신과 질환이나 갑상선 질환, 신생아에게 선천적인 결함이나 질병이 있고, 산후 우울 검사에서 고위험군(13점 이상)에 속하는 자는 제외하였다.
연구대상자는 M여성병원에서 실험군 49명과 대조군 30명, W여성병원에서 대조군 19명을 모집하였고, 연구 진행과정에서 일부 대상자가 직장복귀 또는 불특정 이유로 사후 조사에 참여하지 않아 M여성병원에서 실험군 11명과 대조군 9명, W 여성병원에서 대조군 5명이 탈락하였다. 실험군에서 11명 (22.

모성 역할적응을 측정하기 위해 Sheehan [22]이 개발한 Assessing Adaptation to Motherhood를 Chae [7]가 번역․ 수정한 도구를 사용하였다. 도구는 개발 당시 총 46문항으로 임신 또는 모성으로서의 인식, 여성으로서의 자아인식, 모성으로서의 역할 인식의 3개 하부척도로 구성되었다.
모성 정체성을 측정하기 위해 Walker [23]가 개발한 Semantic Differential Scale-Myself as Mother을 Kim과 Tak [24]이 번역한 도구를 사용하였다. 이 도구는 총 11문항으로 구성되었고, 모성으로서의 자신에 대한 부정적 의미의 형용사와 긍정적 의미의 형용사를 양 극단에 배치하고, 7개의 구간으로 나눈 형용사 어의 구별척도로 이루어져 있다.
산후 우울을 측정하기 위해 Cox 등[25]이 개발한 Edinburgh Postnatal Depression Scale을 Yeo [26]가 번역한 도구를 사용하였다. 이 도구는 총 10문항으로 구성되었고, 지난 한 주 동안 대상자가 느낀 감정과 가장 가까운 것에 응답하도록 되어 있다.

성능/효과

	출산 후 산모는 여성으로서의 역할에서 여성과 어머니로서의 역할로 전환하는 과정에 있어서 중요하게 고려돼야 할 요소는 무엇인가?	그리고 출산 후 산모는 여성으로서의 역할에서 여성과 어머니로서의 역할로 전환하는 과정을 경험하게 되는데, 이 과정에서 여성으로서의 역할과 모성으로서의 역할을 성공적으로 통합함으로써 내적 갈등을 최소화하고 모성 역할에 적응하게 된다[22]. 따라서 출산 초기의 산모가 모성 역할에 적응하기 위해서는 여성으로서의 자기와 모성으로서의 자기에 대한 인식 사이에서 갈등을 적게 경험하는 것이 중요하다. 이에 본 연구에서는 모성으로서의 인식과 여성으로서 인식 및 모성 역할에 대한 인식을 포괄적으로 측정하는 도구를 사용하였고, 연구결과를 통해 본 연구의 모성 역할적응 프로그램이 연구대상자의 여성과 어머니로서의 역할 인식에 긍정적인 효과가 있음을 확인하였다.
