Methods: Data from the 6th Korean National Health and Nutrition Examination Survey (2013-2015) were used. The study included 12,754 adults, aged ${\geq}19years$, who participated in the questionnaire survey of health related to hypertension, stroke, myocardial infarction, and angina pectoris as well as completed blood tests, anthropometry, and oral examination. Statistical analyses included complex sample frequency, complex sample general linear, complex sample cross-tabulation, and complex sample logistic regression analyses. Results: With respect to the number of Decayed-Missing-Filled-Teeth(DMFT ), patients with hypertension (DMFT 8.05), stroke (DMFT 8.66), and angina pectoris (DMFT 8.24) showed a DMFT score of >2, compared with those who did not have these diseases (p<0.05). Patients with hypertension, stroke, myocardial infarction, and angina pectoris presented an approximately 2.7, 3.5, 4, and 3 times higher incidence of periodontal diseases, respectively, than those who did not (p<0.05). Based on the analysis of the relationship between the number of DMFT and cardiovascular diseases, patients with cardiovascular diseases had a risk of 1.033 higher DMFT than those who did not (p<0.05). As per the analysis of the relationship between periodontal diseases and cardiovascular diseases, patients with cardiovascular diseases had a 2.969 higher risk of periodontaldiseasesthanthosewithout them (p<0.05). Conclusions: Two major oral diseases were found to be associated with cardiovascular diseases. Therefore, to prevent these major oral diseases in patients with cardiovascular diseases, oral hygiene management must be actively performed.

그러나, 국내에서는 심혈관계 질환과 구강건강에 관한 다양한 연구들이 진행되지는 못하였고, 주로 심혈관계 질환과 치주질환에 대한 연구[3,4,9]와 심혈관계 질환과 치아상실에 관한 연구[5-7,13]에 치우쳐 있어, 양대 구강병과 심혈관계 질환 과의 연관성에 대한 연구는 희소하였다. 이에 본 연구에서는 국민건강영양조사 제6기 원시자료를 이용하여 한국 성인의 양대 구강병과 심혈관계 질환의 연관성을 확인하여, 구강건강의 중요성을 인식시키고 심혈관계 질환자의 구강위생관리에 도움을 주고자 한다.
제안 방법
구강건강 설문조사를 통하여 칫솔질 시기를 파악한 후 칫솔질 횟수를 산출하였다. 칫솔질 시기는 ‘아침식사 전·후, 점심식사 전·후, 저녁식사 전·후, 간식 후, 잠자기 전’으로 되어 있어 칫솔질 횟수는 총 8회로 산출하였다.
국민건강영양조사의 설문조사를 통하여 성별, 만나이, 소득수준, 교육수준, 당뇨, 최근 1년간 음주경험, 현재 흡연 여부, 칫솔질 횟수, 구강위생용품 사용 개수에 관한 정보를 수집하였다. 소득수준은 가구소득을 4분위수로 나누어 ‘하, 중하, 중상, 상’으로 분류되었고, 교육수준은 최종학력수준에 따라 ‘초졸 이하, 중졸, 고졸, 대졸 이상’으로 분류되었다.
본 연구에서 심혈관계 질환은 국민건강영양조사에서 조사된 항목인 고혈압, 뇌졸중, 심근경색증, 협심 증으로 한정하였으며, 심혈관계 질환의 위험요인으로 고콜레스테롤혈증 유병 여부, 당뇨병 현재 유병 여부, 비만 여부, 최근 1년간 음주경험, 현재 흡연 여부를 변수로 사용하였다. 고콜레스테롤혈증은 장시간 진 행될 경우 동맥벽에 침착하여 죽상경화를 일으키고, 죽상경화는 심근경색이나 뇌혈전 등의 원인이 된다 [18].
대상 데이터
본 연구는 제 6기(2013 - 2015) 국민건강영양조사에 참여한 만 19세 이상 성인과 노인 중 고혈압, 뇌졸중, 심근경색증, 협심증에 관한 건강설문에 참여하고, 혈액검사(고콜레스테롤혈증 유병여부), 신체계측( 비만 유병여부), 구강검사(우식경험영구치 수, 치주질환 유병 여부)를 완료한 12,754명을 연구대상으로 하였다. 본 연구는 W대학교 생명윤리위원회의 심의면제 승인(WKIRB-201808-SB-064)을 받아 수행하였다.
데이터처리
본 연구는 국민건강영양조사 제 6기(2013 - 2015) 원시자료를 이용하여, 자료 분석 결과가 대표성을 갖도록 복합표본 통계분석을 시행하였다. 복합표본 통계분석을 실행하기 위하여 분산추정층(kstrata)과 조사구(psu) 및 건강설문&검진조사 통합 가중치를 적용하여 계획파일을 생성하였다.
연속형 변수는 평균±표준오차, 범주형 변수는 빈도(가중된 백분율)로 제시하였다. 연구대상자의 특성에 따른 심혈관계 질환, 심혈관계 질환의 종류에 따른 우식경험영구치 수와 치주질환은 복합표본 교차분석과 복합표본 일반선형분석을 시행하였고, 심혈관계 질환과 양대구강병의 연관성을 확인하기 위하여 복합표본 로지스틱 회귀분석을 시행하였다.
성능/효과
1. 우식경험영구치 수는 고혈압인 사람(8.05개), 뇌졸중인 사람(8.66개), 협심증인 사람(8.24개)은 그렇지 않은 사람보다 2개 이상 우식경험영구치아 수가 많았다(p<0.05).
2. 고혈압이 있는 사람은 고혈압이 없는 사람보다 치주질환이 약 2.7배, 뇌졸중이 있는 사람은 뇌졸중이 없는 사람보다 치주질환이 약 3.5배, 심근경색이 있는 사람은 심근경색이 없는 사람보다 약 4배, 협심증이 있는 사람은 협심증이 없는 사람보다 치주질환이 약 3배 많았다(p<0.05).
3. 우식경험영구치 수와 심혈관계 질환의 연관성을 확인한 결과, 심혈관계 질환자는 심혈관계 질환이 없는 사람보다 우식경험영구치 수가 많을 가능성이 1.033배 높았다(p<0.05).

이상의 결과를 종합해보면, 양대 구강병과 심혈관계 질환은 연관성이 있는 것으로 나타나, 심혈관계 질환자와 보호자에게 심혈관계 질환과 양대 구강병의 연관성을 인식시키고, 이를 예방하기 위한 구강위생 관리를 적극적으로 시행해야 할 것으로 생각되었다.
